REQUEST FOR ENTRY AND EXIT RECORDS

FROM THE IMMIGRATION DEPARTMENT OF MALAYSIA

Malaysian citizens in Japan who wish to apply for a Japanese driver’s licence are usually
required to submit records of their entry into / exit from Malaysia for the last three (3)
months or more, as these records could not be found in their passports.

PROCEDURE:

1. A request for such records can be e-mailed to the Embassy of Malaysia in Japan
(consular.tyo@kln.gov.my) by attaching the required documents for onward
transmission to the Immigration Department of Malaysia.

2. The Embassy of Malaysia in Japan will e-mail the records to the applicant once
received from the Immigration Department of Malaysia.

REQUIRED DOCUMENTS:

1. A formal application letter addressed to the Director General of the Immigration


Department of Malaysia duly signed by the applicant (the letter must contain
applicant’s full name, complete address in Japan, telephone number, e-mail address
and the reason(s) why the records are needed by the applicant). The address of the
Director General of the Immigration Department of Malaysia is as follows:

2. Scanned copy of applicant’s passport (biodata page of the passport only);

3. Scanned copy of applicant’s Malaysian identity card (front and back); and

4. Scanned copy of applicant’s residence card (zairyu card) in Japan.
