LOYALTY AWARDEES’ SOUVENIR

Term of Reference

BACKGROUND

The Subic Bay Metropolitan Authority (SBMA) is a government agency that was organized and
established by virtue of Republic Act 7227, otherwise known as the Bases Conversion Development Act
of 1992.

In February 5, 2018, during the meeting of SBMA Program on Awards and Incentive for Service
Excellence (PRAISE) Committee at the SBMA Board Room, amended the Policy on the Grant of Loyalty
Award with new cash value for souvenirs to all qualified SBMA employees who have rendered ten (10),
fifteen (15), twenty (20) and twenty five (25) years of continuous and satisfactory service in the
government. Said policy was approved by the SBMA Board of Directors under Resolution No. 18-05-
0697 dated May 10, 2018.

OBJECTIVE

To give SBMA Loyalty Awardees souvenirs on the third quarter of year 2022.

SCOPE OF SERVICES

ITEM 1: TROLLEY BAG - FOR LOYALTY AWARDEES WITH TEN (10) YEARS OF

Trolley Bag : Soft case, secured with a number lock; one short handle on top and on one side, has
a trolley with retractable handle on the top and corner-mounted 360 degree inline

with click clasps, zip lining; two external zip pockets on the front; zip detailing to
adust the size; with SBMA Logo and wordings of “Subic Bay Metropolitan

SUBMISSION OF PROTOTYPES

1. The supplier/contractor with the lowest calculated bid shall submit prototypes of the attached item’s
design within three (3) days from receipt of the Notice of Post Qualification. If recommended for
award, SBMA shall retain custody of all submitted prototypes. Otherwise, the prototypes shall be
returned to the bidders.

2. The approved prototype of the winning bidder shall become the property of SBMA. Said prototype
shall be the basis of comparison when the items are delivered.

3. Alternative samples of the souvenir items shall not be accepted.

DELIVERY SCHEDULE

The winning bidder shall deliver the prescribed quantity and quality of items within forty five (45)
working days from the signing of contract. Delivery shall be from Mondays to Fridays ONLY. No
deliveries shall made on Saturdays, Sundays, and Holidays.

WARRANTY

1. For the Wrist Watch award, the Warranty shall be for a period of one (1) year from date of
acceptance of the item.

2. Items that are found to be defective shall be replaced or repaired by the winning bidder without
additional charge to SBMA within fifteen (15) working days from the return of defective items.

PAYMENT SCHEDULE

Payment shall be made within thirty (30) days upon completion and final acceptance by the Authorized
signatory of the PRAISE Committee.
