Term of Reference

BACKGROUND

The Subic Bay Metropolitan Authority (SBMA) is a government agency that was organized and
established by virtue of Republic Act 7227, otherwise known as the Bases Conversion Development Act
of 1992. In recognition of the efforts of its employees who played a major part in the current auspicious
financial position standing of the agency.

OBJECTIVE

To give SBMA Loyalty Awardees souvenirs on the third quarter of year 2022.

SCOPE OF SERVICES

ITEM 1: 10 KARATS GOLD RING, CUSTOMIZED - FOR LOYALTY AWARDEES WITH


TWENTY (25) YEARS OF SERVICE

Total Number of Awardees : 173


Unit Price / Item : P6,000.00
Total Amount : Php 1,038,000.00

PARTICULARS
Ring : 10 Karats Gold, embossed SBMA Logo and wordings of Subic Bay Metropolitan
Authority, and year 1997-2022 on top of the ring; embossed picture of “Volunteer’s
Monument” on the right side (Side A) and number 25 on the left side (Side B); and,
engraved SBMA at the back & front (Side C & D).

SUBMISSION OF PROTOTYPES

1. The supplier/contractor with the lowest calculated bid shall submit prototypes of the attached item’s
design within three (3) days from receipt of the Notice of Post Qualification. If recommended for
award, SBMA shall retain custody of all submitted prototypes. Otherwise, the prototypes shall be
returned to the bidders.

2. The approved prototype of the winning bidder shall become the property of SBMA. Said prototype
shall be the basis of comparison when the items are delivered.

3. Alternative samples of the souvenir items shall not be accepted.

DELIVERY SCHEDULE

The winning bidder shall deliver the prescribed quantity and quality of items within forty five (45)
working days from the signing of contract. Delivery shall be from Mondays to Fridays ONLY. No
deliveries shall made on Saturdays, Sundays, and Holidays.

WARRANTY

Items that are found to be defective shall be replaced or repaired by the winning bidder without additional
charge to SBMA within fifteen (15) working days from the return of defective items.

PAYMENT SCHEDULE

Payment shall be made within thirty (30) days upon completion and final acceptance by the Authorized
signatory of the PRAISE Committee.
