AFFIDAVIT

PETS J VETERINARY PRODUCTS INC., a corporation, duly organized and existing

under the laws of the Philippines, with Office Address at Unit K, 5F Westgate Tower,
Investment Drive, Brgy. Ayala Alabang, Muntinlupa City 1780, duly represented by its
President, Jill B. Platon,1of legal age, Filipino, and with a residence at 30A United States St.,
Don Bosco, Betterliving Subd., Paranaque City, Metro Manila, after having sworn to in
accordance with law, do hereby depose and state:
1. That, Defendant CCK AGRICULTURAL TRADING, represented by its owner Celso
Nava,
has a remaining unpaid total balance of EIGHT HUNDRED FIFTY-NINE THOUSAND SEVEN
HUNDRED THIRTY-EIGHT PESOS (P859,738.00) from DAIF cheque dated October 11, 2022,
and utilized goods delivered August 25, 2022 and September 05, 2022, is hereto attached as
“ITEM 3”.
2. That, on April 19, 2023, we have sent a demand letter to MR. CELSO NAVA
With an address at 14 Purol III, Tikay, Malolos, Bulacan, a copy of which is attached herein
as “Evidence”.

3. That, despite repetitive demands, MR. CELSO NAVA

still failed to pay the above-mentioned obligation.

4. That I am executing this affidavit to attest the truth and veracity of the foregoing
Facts and circumstances in support of the small claim complaint against defendant MR. CELSO
NAVA.
