    Four workers were killed during a tire change on a haul truck at a mining site in Indonesia. As they were removing wheel cleats from the right rear tires, the inflation air was suddenly released from the inside tire, propelling the outside tire and rim assembly off the wheel hub. All four workers standing at the sidewall were hit by the flying assembly and died from their injuries. The incident is being investigated but appears similar to a recent fatality in Australia where both rear tires were not fully deflated according to safety procedures before loosening wheel nuts during a dual tire removal.
