AFFIDAVIT
I, REYNALDO TAFALLA ILAGAN, of legal age, Filipino, with present
address at P.Paterno St. Biñan, Laguna, after having been sworn to in accordance
with law, hereby depose and state, that:

1. That I am one of the heirs of deceased ISAGANI TAFALLA ILAGAN,

being his brother for he died single without a child;

2. That my said brother died on February 13, 2015 at Sta. Rosa, Laguna;

3. That my said deceased brother has pawned at CEBUANA LHUILLER

PAWNSHOP Alfonso Branch a Lady’s ring with brill 12k 2.80 grams
with amount Php 2,884.00 which will expire on May 11, 2015;

4. That I hereby request said pawnshop to release said ring after full pay-
ment of said pawned ring;

5. That this affidavit is being executed for the purpose of attesting to the
truth of the facts above stated and for whatever legal purpose it may
serve.
