AFFIDAVIT OF PARENTAL
CONSENT OF MINOR CHILD
TO TRAVEL ABROAD

I, FROILAN BAYOT AMBAT, of legal age, married, Filipino Citizen, and

resident of 167 Brgy. Taywanak Ilaya, Alfonso, Cavite, after having duly sworn to
in accordance with the law hereby deposed and say that:

1. I am the father of minor ALEXANDRE ABANILLA AMBAT, 11 years old,

born on May 11, 2004 at Pasay City.

2. That I am giving my consent to my said minor daughter ALEXANDRE

ABANILLA AMBAT to travel to Thailand;

3. That I am allowing my elder daughter DAPHNE ROSE ABANILLA

AMBAT to accompany my said minor child during her travel to Thailand,
with the purpose of spending her Christmas vacation together with their
mother NOEMI ABANILLA AMBAT;

4. I am executing this affidavit for informing proper authorities that I am giving

my parental consent to my said minor daughter, ALEXANDRE ABANILLA
AMBAT to travel to Thailand on December 2015 in order to meet their
mother;

5. Moreover, I am executing this sworn statement to attest to the truth of the

foregoing facts and not for any unlawful reasons.
