AFFIDAVIT OF CONSENT

I, EMELITA CAMANSO LABOR of legal age, Filipino, single, with residence

address at San Juan St. Brgy. Poblacion ,Inopacan, Leyte, after having duly
sworn to in accordance of law ,do hereby depose and state that:

1. I the grandchild of KIM VILLACORTE , I am the person exercising authority

of the said minor since her mother died ( my daughter ) and his father
abandoned him;
2. The child will travel together with his aunt – CHONA V. LIM to Taiwan for
leisure ;
3. The above mentioned minor will be leaving on August 28, 2023 and will stay
in Taiwan until September 5 ,2023 together with his aunt who will take care
of him;
4. I can not personally travel with the child because I have work commitments ;
5. I am giving my full consent for the minor child to travel abroad accompanied
by CHONA V. LIM ;
6. I am executing this Affidavit to attest to the foregoing facts and for whatever
legal purpose it may serve.
