Tuguegarao, after having been duly sworn to in accordance with the law, hereby depose and say:

That the following documents submitted to Philippine Overseas Employment Administration

(POEA) are authentic and the data/facts contained therein are true and correct of my own personal
knowledge, namely:

 Transcript of Records (TOR) issued by International School for Culinary Arts & Hotel
Management

 Certificate of Employment as a Head Chef /Head Cook at Kape Uno Bistro Restaurant from
August 8, 2021 through April 18, 2023;

 Certificate of Employment as a Head Chef /Head Cook at Nanas Kitchen from March 8, 2017
through June 10, 2019;

 Certificate of Employment as a Culinary Instructor-Supervisor at Ocean De Azu Hospitality

School Inc. from July 25, 2019 through June 10, 2023;

I further undertake to allow POEA or any of its authorized representatives to verify the
documents submitted.

Finally, I herby DECLAIRE and ASSUME full responsibility for any falsity or misinterpretation of
any material fact in the contents of the above mentioned documents.
