This is to justify for late submission of IPCR ACCOMPLISHMENT July to December 2022 and IPCR
TARGET January to June 2023, we submitted the abovementioned last December 15, 2022, but it was
return on the same day to our office because the PEEMO was asking for additional copy and wasn’t sign
by our PG Head because she’s already on leave by December 19, 2022 and resume on January 3, 2022.,
that is why were not able to submit the IPCR on the said date.

I humbly apologize for the inconvenience it may have caused your office.
