I am writing to inform you of my need to take a leave of absence from my position as a park
attendant at Mad Park. Unfortunately, I undergo with my surgery that will necessitate my
absence from work for a period of time.

I had my surgery last February 4, 2024, and my anticipated recovery time will take 2 to 3
months. I have attached documentation from my healthcare provider confirming the necessity
of this surgery and outlining the expected duration of my recovery.

my recovery. Additionally, I am committed to returning to work as soon as I am medically
cleared to do so.
