AFFIDAVIT OF EXPLANATION
We, PAT Marlon D Gatchalian, of legal age, bonafide member of the PNP and
presently assigned at Guadalupe Nuevo Police Sub-Station, Makati City Police Station
located at Antipolo St., Guadalupe Nuevo, Makati City, after having been duly sworn in
accordance with the law, do hereby depose and state;

That, I am furnished thru messenger a copy of the notice to explain from the Deputy
Chief of Police for Operation ACOPO of Makati CPS on March 29, 2022. Directing me
to explain in affidavit form on why no disciplinary action be instituted against me for
alleged abandoning/absent during the accounting of personnel detailed at the
proclamation rally of Makatizen United Party on March 25, 2022 at E. Zobel corner JP
Rizal Brgy Poblacion Makati City.

That, on March 24, 2022 I rendered duty at the Philippine Orthopedic Center located at
Maria Clara corner Banawe St Quezon City from 4PM to 12 Midnight despite the fact
that I am not feeling well. After my duty I immediately sought medical consultation were
I was diagnosed to be suffering from MYALGIA and was advice by the physician to rest.
It is my deepest regret in not attending the said detail despite my willingness however I
need to heed the doctor’s advice.

That, the undersigned has NO INTENTION to defy any lawful order from Higher
Headquarters and fervently hope and pray that this explanation be given due merits and
utmost consideration.

That I am executing this affidavit to attest the truthfulness of the foregoing facts as
attested by my signature. Likewise attached herewith is copy of my duty detail order at
Philippine Orthopedic Center and my medical certificate for your perusal.
