The criteria used to assess survey instruments in this study have been derived and

adapted from the work of Carter V. Good and Douglas B. Scales (Goods & Scates,
1972). With due respect, the researchers kindly seek your expert evaluation of the
enclosed survey questionnaire, which is a fundamental component of the research
entitled “Level of Knowledge and Attitude towards HIV, and Willingness of Nurses to
Work with PLHIV in Iloilo City”.

Instruction: Please express your level of agreement or disagreement with the


statements below by selecting the number that best reflects your judgment. The
following ratings are: (5) Strongly Agree, (4) Agree, (3) Undecided (2) Disagree, (1)
Strongly Disagree.

VARIABLE 1: KNOWLEDGE

Criteria for Validity

1. The items in the instrument are relevant to answer the objectives of

3. The items in the instrument can obtain depth to constructs being 5


measured.

4. The instrument has an appropriate sample of items for the construct 4


being measured.

5. The items are neither too narrow nor limited in its content. 4

consistent, clear, and not conflicting.

7. The terms adapted in the scale are culturally appropriate. 5

8. The layout or format of the instrument is technically sound. 5


9. The instrument is culturally acceptable when administered in the local 4
setting.

10. The responses on the scale show a reasonable range of variation.
