AFFIDAVIT

I, Mohd Farooq S/o Behram Din R/o Kalaban Tehsil mendhar Distt
Pooch at present Upper Roopnagar Sector 07 near Bachpan School
Gouse No. 117 Janipur Jammu do hereby state an oath and declare as
under-

1. That I am the permanent resident of U.T of J&K and Ladakh at

Jammu.
2. That I am serving as gardener in Govt CD Hospital Jammu.
3. That my wife namely Hamida Bi is a housewife.
4. That I am a father of two children namely Wahid Iqbal Chouhan
(DOB- 06-06-2007, studying in 10th standard) and Wajid Iqbal
(DOB-06-06-2011, studying in 7th standard).
5. That I am intending to seek child education allowance as per the
norms laid down by the govt.
6. That I am soliciting this affidavit in support of child education
allowance.
7. That this affidavit is being solicited for necessary refrence and
record of the concerned department.
8. The statement made above are true and correct.
