I am completing for the degree Master of Public Administration at Union College of


Laguna, relevant to this I am doing a research entitled “EMPLOYEES OF LAGUNA
PROVINCIAL JAIL WORK MOTIVATION AND PERFORMANCE BASIS FOR
EMPLOYEE RETENTION PROGRAM”.

In this connection, I am conducting a survey regarding the data needed to answer the
sub problems cited in the earlier part of my thesis. I would need only a few minutes of
your time to fill out a questionnaire which forms a comprehensive part of my research.
