A RESOLUTION CONCURRING TO THE APPOINTMENT OF BARANGAY HEALTH WORKER

(BHW) OF BARANGAY ____, ZONE _____, DISTRICT ______, CITY OF MANILA

WHEREAS, Chapter 2 Section 387 (b) of the Local Government Code of 1991 provides
that the Sangguniang Barangay may form community brigades and creates such other positions
of offices as may be deemed necessary to carry out the purpose of the barangay government in
accordance with the needs of public service, subject to the budgetary limitations and personal
services prescribed under Title Five, Book II of this Code;

WHEREAS, Republic Act No. 11223 or the Universal Health Care (UHC) Act provides
that the Local Government Units (LGUs) shall endeavor health care delivery sysystem that shall
afford all Filipinos a primary care provider, Hence, the State shall ensure an adequate and
equitable number of human resources for health at all levels. Including the Barangay Health
Workers (BHWs), to assist in the service delivery of comprehensive and quality health care
service delivery;

WHEREAS, ____________________________, Punong Barangay of this Barangay appointed the

below listed names as Barangay Health Workers (BHW) on ______________________ and submitted
the same appointment to this august body for review and concurrence, to wit;

WHEREAS, after thorough study and deliberations, this Sanggunian found that the
appointee satisfies the minimum qualification requirements as provided for under the Local
Government Code of 1991;

NOW THEREFORE, on motion of Kagawad ___________________________ duly seconded by

Kagawad _____________________ and all members present; BE IT RESOLVED AS IT IS HEREBY
RESOLVED, to concur to the appointment issued by Punong Barangay for the above-listed
name/s as Barangay health Worker/s;

RESOLVED FURTHER, to send copies of this Resolution to all the officials and offices of
the government concerned for their information.
