SIM card for the TNT Mobile Sim No. 09631064452 registered under my
name.

Due to an unknown person pick-pocketed my cellphone while on my way

along Public Market Cebu City, I am unable to personally undertake the
process. Lyn O. Obial is my partner, and I trust her to act on my behalf in this
matter.

Please find enclosed copies of my identification documents : Affidavit of Loss

and Seafarers Identification Documents (SID) as well as Lyn O. Obial's
identification for your verification.

I authorize Lyn O. Obial to sign any necessary documents and provide any
required information during the SIM replacement process. I also acknowledge
that any actions taken by Lyn O. Obial regarding the replacement of my SIM
card will be considered binding and valid.
