DEED OF VOLUNTARY SURRENDER

WITH QUITCLAIM

We, ELSON R TABERNA and LANIE C TABERNA , of legal age, Filipino citizens and residents of
_________________________________________________________________, after having been duly sworn to in
accordance with law, hereby depose and say:

We are the registered owner of property located BLOCK 15 LOT 7 PHASE 1 CAMELLA KORONADAL
GENSAN DRIVE NATIONAL HIGHWAY BARANGAY CARPENTER HILL KORONADAL CITY with an
area of Forty Four Square Meters (44.00 sqm) and covered by Transfer Certificate of Title No. 145-
2020000124 of the Registry of Deeds of KORONADAL CITY (“Subject Property”);

That We hereby surrender and turn over to BDO the physical possession of the Subject Property on
_____________ devoid of any occupant or tenant, furniture, movable equipment/machines and personal belongings,
and in good tenantable condition and without any claim or lien from any third party, and shall forever hold BDO, its
stakeholders, directors, officers, employees, assigns or representatives, harmless and free from any civil, or criminal
liability or claim arising from or consequent to this voluntary surrender of property.

That We shall further assume any and all liability as regards to any loss, damage or alteration incurred or
sustained by the Subject Property, as well as any loss, damage or removal of any permanent improvements or fixtures
therein pending turn over/surrender of possession of the Subject Property to BDO;

That all utility charges and/or association dues (if any) that have accrued up to actual turnover date shall be
for my account.

That We are waiving and relinquishing our right of redemption under the law within the one-year redemption
period.

This Deed of Voluntary Surrender with Quitclaim is issued this ________ day of _____________ for all legal
intents and purposes.
