TO WHOM IT MAY CONCERN:

THIS IS TO CERTIFY that Rodolfo Padillo Cabanes of legal age, and resident
of Purok 3- Candabong, Anda Bohol has manifested her intent to engage business in this
Barangay in her capacity as owner of said business with business named CANDABONG
MINIMART (/_) single proprietorship, (_) NGO , (__) joint venture.
THIS FURTHER CERTIFIES that the nature of the business does not destroy the
moral values of its residents especially the youth and the children, nor does it cause the
breach of peace and order and harmony in the Barangay, nor endangers the safety and
health of its constituents, nor unnecessarily destroys the environment to the detriment of
the residents thereof.
THIS FURTHER CERTIFIES that the applicant has paid the amount of ONE
HUNDRED pesos (P100.00) in consideration thereof by virtue of REVENUE CODE
ORDINANCE NO. 01 series of 2019, and that all pertinent ordinances of this Barangay are
hereby complied with and shall henceforth be observed with due respect and obedience to
the local authorities.
THIS FINALLY CERTIFIES that the issuance of this Business Barangay
Clearance is pursuant to Section 152 “c” of RA 7160 wherein the city/ municipality shall
never issue any permit or license for any business or activity unless a clearance is first
obtained from the Barangay where such business or activity is located or conducted.
