ATTENDANCE 25 PTS
Attendance (15 pts) Refers to the presence in the workplace, meetings and
conferences, and other work-related activities. It is also
notifying the superior in advance in case of absence
Punctuality (10 pts) This means arriving on time for work, talking and
returning from breaks on time, and calling the superior
before being late.
ATTITUDE 25 PTS.
Appearance (5 pts) It calls for practicing personal hygiene, workplace attire,
and behavior appropriate for the position and duties.
Integrity (10 pts) It means reliability, honesty, and propensity to adhere to
policies of the workplace.
Initiative (10 pts) This means participating fully in a task or project from
initiation to completion by being resourceful.
PERFORMANCE 50 PTS.
Quality of Work (15 pts) Giving best effort, evaluating own work, and utilizing
feedback to improve work performance. It is also striving
to meet quality standards.
Problem-Solving /Critical Exercising sound reasoning and analytical thinking and
Thinking (10 pts) using knowledge and information to solve workplace
problems.
Response to Supervision Accepting direction, feedback, and constructive criticism
(15 pts) with a positive attitude and using the information to
improve work performance.
Workplace Culture, Policy, Complying with health and safety rules and exhibiting
and safety (10 pts) integrity honesty and good practices in the workplace.
