1. INTRODUCTION

This standard applies to protect personnel from actual or potential workplace hazards, identified
through risk assessment that cannot be controlled by other acceptable means, i.e., engineering
or administrative practices.

The use of Personal Protective Equipment (PPE) lies at the bottom of the hazard control
hierarchy because it does not remove the workplace hazard. Every reasonable effort shall be
made to reduce workplace hazards through the use of engineering and administrative controls
(e.g. process/facility/equipment design, operating procedures and safe work practices). PPE
shall be used in conjunction with these established standards/practices but only as the last step
in employee protection.

This standard shall ensure that appropriate types of PPE for each workplace hazard is selected
and made available to personnel and that PPE are appropriately used and maintained in fit for
purpose condition.

This standard provides the guidelines for Personal Protective Equipment as per the
requirements of the OSHA and the OE system.

2. SCOPE & PURPOSE

This PPE standard is applicable to all departments and organizations performing work within the
Q-Chem operated Facilities. (Q-Chem, Contractors, Sub-Contractors and Visitors).Specifying Q-
Chem in this document refers to Q-Chem, Q-Chem ll, RLOC and Salam Tower.

Specialized PPE requirements for emergency response and firefighting purposes are not in the
scope of this standard.

3. DEFINITIONS

 Basic/Minimum PPE : Common PPE that is required to visit or work in any of the PPE
Designated Area in the plant.
 Contractors: Workers brought to site to visit or work on project specific jobs. This includes
all other workers not fitting the category of Employee.
 Employees: For the purpose of this PPE Standard, the persons referred to as Q-Chem
Direct Hire, Secondees and Base Load Contractor.
 Hazard: A workplace condition that presents the opportunity for occupational injury or
illness (e.g. chemical, physical and biological hazards).
 Hazard Assessment: The process used to identify hazards in the workplace for the
purpose of selecting appropriate PPE.
 JSA : Job Safety Analysis
 Personal Protective Equipment: Consists of devices worn by workers to help protect
them against hazards in the work place, including stressors to the human body that are
physical, chemical, biological and ergonomic in nature.
 PPE Designated Areas: Areas inside the perimeter fence where, at a minimum, Basic PPE
is required to be worn at all times as per the PPE Matrix Section 1.

 Specialized PPE: Special purpose PPE that is required to address a high risk job/task
specific hazards. Specialized PPE shall be identified in PPE Matrix Section 2+3, a work
Procedure or Program, a Permit to Work (PTW), JSA or a Safety Data Sheet.(i.e
Hydrojetting PPE, Grit blasting hood, half/full face respirators, welding shield, Chain saw
cutting leg/body protection, etc..,)
 Visitor: A person entering the plant for visitation purposes only and may or may not be
visiting at PPE Designated Area.
 Worker: A generic term to include both Employees and Contractors.
 Workplace: The location and environment in which the worker shall perform the required
tasks of the job. Workplace also includes locations outside the facility, if an employee is
required to travel as part of the work requirements (e.g. visiting another site or facility).

4. STANDARD

4.1. Understanding the Hazards

i. The first critical step is to identify workplace hazards which may be achieved through
methods such as Hazard Assessments, IH Qualitative & Quantitative Assessment, HazCom
Program, SDS, JSA and Permit to Work (PTW).
ii. Workplace hazards Assessment and selection of PPE are completed through Safety
Section.
iii. Use the Guideline for Workplace Hazard Assessment and PPE Selection – Attachment 6:
Guideline for Workplace Hazard Assessment and Selection of PPE.
iv. The hazard assessment shall be reassessed for:
a) Any changes in conditions, equipment, materials or operating procedures that could
affect occupational hazards.
b) As part of a Management of Change (MOC) process.
c) As part of incident investigations involving personal injury or exposures.
v. The hierarchy of control shall be applied to control/mitigate the identified hazards.PPE shall
be considered as the last resort option to bring the risk to the ALARP (As Low As
Reasonably Practical) level.

The acceptable hierarchy of control used in risk management is:


a) Eliminate: i.e. the hazard can be removed completely.
b) Substitute: i.e. the hazard can be substituted.
c) Isolate: i.e. the hazard can be isolated.
d) Engineering: i.e. engineering controls.
e) Administration: i.e. restricted access, signs/barriers, job rotation, training, procedures.
f) PPE.

4.2. PPE Selection

Once the hazard assessment establishes that PPE is needed for designated job / location,
the following selection criteria shall be followed:
i. Provide a level of protection which is greater or at least minimum required to protect
the employees from actual/potential hazards.
ii. Comfortable to wear.
iii. Fit properly.
iv. Meets accepted specifications/standards (ANSI, BS, EN or equivalent international
standards).
Refer to PPE Matrix and Attachment 6: Guideline for Workplace Hazard Assessment and
Selection of PPE.

NOTE 1
PPE selected as per the Workplace Hazard Assessment Form is the
minimum requirement. Use of higher level of protection of PPE is
acceptable.

4.3. PPE Approval and Evaluation

i. The Safety Section shall approve the PPE for use within Q-Chem.
a) Refer to ‘Approved PPE and Safety Equipment List’ (HSE-SAF-LST-00-0005) for
approved PPE.
ii. Safety section shall evaluate PPE as necessary as a part of continuous improvement
measure.
iii. Any workplace hazard that requires Specialized PPE not already on the approved PPE List
shall be brought to the attention of the Safety Section for evaluation and approval.
a) Safety Section shall analyze the job task and workplace hazards and recommend
appropriate level of PPE.
b) Safety Section shall submit technical specifications to Procurement Department and
approve samples before purchase.
iv. Employees shall ensure that when wearing PPE not supplied by Q-Chem, that the PPE
meets the minimum technical specifications outlined in the Approved PPE and Safety
Equipment List (HSE-SAF-LST-00-0005).
v. Contractor PPE Approval: PPE used at Q-Chem site by contractors shall be as per the
Approved PPE and Safety Equipment List or alternate specification approved by Safety
Section.
vi. Contractor to submit the PPE specification along with sample(s) to respective DSR who
ensures it meets Q-Chem technical specifications.
vii. DSR may use Contractor PPE Form for guidance and documentation of Evaluation(See
Attachment 8).

viii. DSR may get assistance from Safety Section for any clarification while evaluating on PPE
specification, while conducting the evaluation of PPE used by Contractors.

4.4. Basic PPE

Basic PPE is the minimum PPE required for entry into PPE designated area of the plant.

Basic PPE includes:


i. Body protection
ii. Head protection
iii. Eye protection
iv. Foot protection
v. Hand protection
vi. Hearing protection
The basic PPE for various PPE designated areas are summarized in Section 1 of PPE
Matrix.
The Q-Chem Safety Requirements Plot Plan, QCC-A1-01-MP-0182 (Mesaieed) and RC-CIV-
DWG-30-0001 (RLOC) provide pictorial representation of PPE designated areas.

NOTE 2
Following are exceptions from basic PPE requirement in the PPE
Designated Areas. All attempts should be made to comply with
requirements prior to allowing exceptions:
a) Escorted Visitors may travel, inside vehicle on non-classified
roadways, without designated PPE, as long as they remain in
the vehicle. Example, plant tour.
b) Truck driver, driving on non-classified roadways, to a non-
classified area in the facility, for deliver or pick-up.
c) During emergency conditions personnel may travel by foot to a
safe location/assembly area without basic PPE.

4.5. PPE for Common Tasks

PPE shall be required for common tasks even when they are carried out in a non-PPE
designated area. PPE requirements for such common tasks are summarized in Section 2 of
the PPE Matrix.

4.6. Specialized PPE (Task specific PPE)

i. Over and above Basic PPE, a specific job / task may require the use of specialized PPE e.g.
disposable coveralls, full face or half face respiratory protection equipment, etc.
ii. Specialized PPE may also be required outside the PPE designated areas when identified by
a Procedure, Permit to Work (PTW) or an SDS.

iii. Task specific PPE requirements for routine tasks are summarized in Section 3 of PPE Matrix.

4.7. Distribution/Issuing of PPE

i. Q-Chem employee: All approved PPE (except safety foot wear and prescription safety
glasses) are available within Material warehouse for Q-Chem employees. To obtain PPE,
employees shall complete the PPE Request Form and deliver the form to the Materials
Warehouse staff and the staff will issue PPE from the store.
ii. Contractors: Shall provide approved Basic PPE as well as specialised PPE over and above
the basic PPE when the job requires which meets Q-Chem technical requirement as
specified in Approved PPE List or acceptable international standard such as ANSI, BS or EN
specification approved by safety section.
iii. Q-Chem Employees shall prevent/stop any Contractors, visitors entering or performing work
not in possession of required and acceptable PPE.
a) Report all Contractor and Visitor PPE related issues to the Safety Section.
iv. Basic PPE can be issued to Visitors who do not have their own PPE if the visit includes going
to a PPE Designated Area.
a) All PPE issued to visitors shall be returned when leaving the Q-Chem Facility.
b) The Q-Chem Designated Site Representative (DSR) is responsible for ensuring visitors
and/or contractors are in possession of the required PPE.

4.8. Purchasing of PPE

i. The PPE is purchased through the standard Q-Chem procurement process; however the
Safety Section shall approve the PPE sample(s) before purchase.
ii. All purchased PPE shall meet the Q-Chem Technical Specification.
iii. The safety Section will determine if the PPE item will be maintained as a stock item in the
material warehouse.
iv. Employees may purchase their own hearing protection/Safety glasses at their own expenses.
These PPE shall comply with Q-Chem requirement as specified in Approved PPE List or
acceptable international standard such as ANSI, BS or EN and approved by safety section.
a) Minimum NRR of 24 dBA is must for hearing protection.
b) However, it is recommended to wear Q-Chem supplied hearing/Eye Protection.
v. Purchase of Prescription Safety Glasses for Q-Chem employee:
a) Employees shall fill the attached Prescription Safety Glasses Voucher (Attachment 3)
and get approval from Q-Chem supervisor.
b) It is the employee’s responsibility to maintain these glasses and request replacement if
damaged or eye prescription changes.
c) Employees may select one pair clear and/or one pair of tinted glasses.
d) Employees shall have an eye examination by approved vendor and select the frames.

a) At the commencement of employment, every employee is given a voucher authorized


by his/her department to purchase Q-Chem approved safety footwear, which meets the
Q-Chem specifications.
b) In the event that the employee’s safety footwear is damaged at work or is deemed unfit
for use, the employee’s department can authorize the purchase of a new pair.
c) Employees have the option to visit any vendor on the Q-Chem approved vendor list.
d) Employees must ensure the original documents are submitted to vendor with their
completed cost centre and G/L account information.
e) Approved safety footwear vendors are required to submit invoices to Q-Chem Finance
for payment.
f) Employees can purchase their own safety foot wear at their cost from the market;
however the safety foot wear must comply with Q-Chem requirement as specified in
Approved PPE List or acceptable international standard such as ANSI, BS or EN and
approved by Safety Section.

4.9. Maintenance and Care of PPE : General Requirements

i. PPE shall be inspected before wearing to ensure the equipment is clean, good and in
operating condition.
ii. PPE shall not be shared between employees unless it has been properly cleaned and
sanitized.
a) Employees shall maintain PPE properly to ensure the designed level of protection is
achieved.
b) Do not use dirty, modified, damaged or defective PPE which cannot provide the
required level of protection.
c) Do not modify/temper with PPE or misuse PPE.
d) Contaminated PPE, which cannot be decontaminated, shall be disposed of in a manner
that protects employees from exposure to hazards. Use the proper waste collection
bins for disposal.

4.10. Eye and Face Protection

i. Eye and face protection protects the eyes and face from both physical and chemical hazards
(i.e. Flying particles, hazardous chemicals, hazardous light radiation).
ii. Eye and face protection shall meet ANSI Z-87.1, BS EN 166 or equivalent international
standard as approved by the Safety Section.
iii. Safety Glasses
a) Safety glasses are part of basic PPE.
b) All personnel shall use safety glasses in the PPE Designated area or as specified in
hazards assessment of specific job.
c) Employees can get clear and dark safety glasses from the materials warehouse.

d) Employees can obtain approved prescription safety glasses as outlined in Section


4.8.5.
e) Safety glasses with tint(silver, amber, mirror or grey) are not to be worn after dusk or in
low lighted areas (e.g. buildings).

NOTE 3
For Buildings - Dark/Tinted safety glasses may be worn while
walking through a building on ground level and on the designated
walkway only.

iv. Safety Goggles


a) Use goggles when entering the area marked with orange line and labelled “Goggles
required “or identified in PTW, JSA, PPE Matrix.
b) Goggles must be carried all the time by Operation personnel (Operators, Supervisors)
who routinely work in a unit that labelled as “Goggles required” areas and demarcated
by orange line. (e. g. Ethylene, Utilities).
c) Goggles shall not be deformed, distorted or warped to provide a good fit during use.
d) It is recommended to use the goggles pouch to carry goggles.
v. Contact Lens
a) The use of contact lenses is not recommended inside the process area, or any other
PPE Designated Area. It is recommended to use prescription safety glasses if vision
correction is required. Contact lenses are not eye protection devices, and shall not be
considered as a substitute for safety glasses or chemical splash goggles.
b) Use of contact lenses is prohibited for following:
 Dusty atmospheres which requires wearing a half-mask face piece.
 Welding operations
 When working with following :
o 1,2-dibromo-3-chloropropane (DBCP)(OSHA regulation)
o 4,4'-methylene di aniline: (OSHA regulation)
o Ethylene oxide:(OSHA regulation)
o Methylene chloride: (OSHA regulation)
o Ethyl alcohol (Study of absorption by Cerulli, et al. 1985)
o Isopropyl alcohol: (Study of absorption by Cerulli, et al. 1985)
vi. Eye Protection from light radiation
a) Use appropriate Welding glass/Shield with correct shade number of filter lens to protect
eye from harmful radiation (i.e. oxy-fuel gas welding, cutting. See ref OSHA Shade
charts as Attachment 6).
vii. Face Protection
a) Face shields are designed to protect the face, not the eyes.

viii. Combination of use of Safety glasses or goggles with face shield for eye and face protection
a) Protection of eye and face when there is potential of flying particles:
Use both face shield and safety glasses.
b) Protection of eye and face when there is potential of sprays or splashes of hazardous
chemical:
Use both face shields with chin guard + goggles.

NOTE 4
Safety glasses are not required to be worn when wearing googles.

ix. Avoid looking directly at the sun or any intense light radiation (i.e. Arc welding flash).
x. Frequent cleaning of eye/face protections is required to maintain visibility when there is
potential of mist formation.

4.11. Foot Protection

i. Workers must ensure that their safety footwear is suitable for the type of work being done for
the job.
ii. Foot protection used must be Q-Chem approved.
iii. Safety Boots must be worn as Basic PPE as per the guidance in the Basic PPE
Requirements Matrix, or when the job task requires.
iv. Foot protection for Q-Chem employees and contractor must:
a) Conform to ANSI Z41 PT99 I75 C75 PR, or EN ISO 20345:2004 S3 (previous EN 345-
S3) or ASTM 2412 and ASTM 2413 and marked accordingly.
b) Full grain or nu-buck leather upper.
c) Oil/Chemical resistant sole.p
d) Integrated arch support.
e) Ankle supporting (high-top).
f) Defined heel of 0.5”.
v. Employees must ensure they wear proper socks inside their safety boots at all times. Clean
cotton sport socks (washed every day/shift) with elastic tops are recommended
vi. Replace the damaged safety footwear if found with following damages:
a) Cracked or torn uppers
b) Separation between soles and uppers
c) Holes or cracks in soles
d) Significant loss of tread in the soles
e) Protective toe caps exposed

vii. Safety footwear must be cleaned when necessary

NOTE 5
Do not use chemicals to clean safety footwear.

viii. Q-Chem employees can purchase Safety Footwear as outlined in section 4.8.(vi).
a) Each department is responsible to ensure that their employee’s safety footwear meet the
Q-Chem specification.
b) To ensure maximum protection, properly fitted footwear must be worn .Do not select a
safety boot that does not fit properly. Visit and seek assistance from other approved
safety footwear vendors to find one that is proper size and that is comfortable.

4.12. Hand Protection

i. The selection of the appropriate hand protection shall be based on an evaluation of the
performance characteristics of the hand protection relative to the hazards present (e.g.
chemical hazards, cut hazards, flame hazards, or contact with blood-borne pathogens).

NOTE 6

Employees cannot purchase their own hand protection.

ii. When using a chemical, refer to SDS for correct type of gloves. Note: There is no single
glove that is resistant to all chemicals.
a) Gloves (cotton or leather) must be carried with an employee at all times when in a PPE
Designated Area.
b) Chemical gloves are available in a variety of compounds and compatibility with the
material being handled is CRITICAL.
c) If the SDS recommends a glove that Q-Chem does not supply, refer to the Industrial
Hygienist.

4.13. Head Protection

i. Head protection protects the head from hazard of falling objects or overhead striking
hazards.
ii. Head protection shall meet the performance and specification requirements of OSHA
1910.135 and its reference standard ANSI Z89.1. Type l, Class E & G or BS EN 397.
iii. Metal hard hat is prohibited.

NOTE 7
Employees cannot purchase their own hardhats. Use Q-Chem
provided head protection available from material warehouse.

iv. Hardhats may not be mandatory for a specific task due to body positioning or other forms of
head protection are utilized, even though the task is conducted in a PPE Designated Area.
These exceptions include:
a) Inside a hardtop vehicle (i.e. crane)
b) Inside an office building
c) Forklift Drivers in Logistics
d) Mentioned in JSA and approved from safety Section
v. As per the Risk Assessment, hard hat may be required in a non-PPE Designated Area for
special circumstances or activities within that area.
vi. Hardhats must be kept free from holes and punctures. And must not be dropped, thrown,
painted or used as a support or seat.
vii. Do not use vented hardhat.
viii. Do not leave hardhats in direct sunlight while not in use.
ix. Use chin-strap along with hardhats for:
a) High winds or wind gusts – especially when working at heights.
b) Specified in JSA/PTW etc.
x. Workers must inspect hardhats regularly for damage to shells or the inner cradle and
suspension:
a) Inspection is done by compressing the sides by hand and looking for cracks and
observing how quick the shell returns to normal shape.
b) Replace hardhat immediately if cracks or deformities are observed.
c) Any hardhat that has sustained an impact (sudden blow) must be replaced with a new
hardhat. The old hardhat must be destroyed to avoid further use of damaged hardhat.
xi. Workers must:
a) Never wear a baseball cap under a hardhat.
b) Never wear a knitted cap under a hardhat (e.g. Took, wool cap, etc.)
c) Never paint a hardhat’s shell. Some paints will soften and weaken the shell.
d) Never drill holes, cut or carve in a hardhat’s shell. Holes will weaken the shell and
destroy its electrical insulating qualities.
e) Never carry materials inside their hardhat while wearing it (e.g. between suspension
and the shell).

f) Never cover their hardhat with stickers to the point that a thorough inspection is
impossible.
g) Monkey masks (face/head-wraps) are acceptable work clothing with following
conditions:
 Made from FRC or 100% Cotton.
 Fit smooth and tight to the head.
 Fastened with Velcro (no snaps, etc.)
h) Long Hair must not be kept below the shoulder level. It must be tied back, or in a hair
net when in a PPE Designated Area.
 Long hair must not be wrapped under the hardhat suspension
i) Do not use T-Shirts wrapped around face and under the hardhat because:
 Most T-Shirts are large and of thicker material
 Wrapping around the neck creates a choking hazard if the T-Shirt caught on
something.

4.14. Hearing Protection

i. Hearing Protection equipment is designed to provide protection to a worker’s hearing during


exposure to high noise levels.
a) Hearing protection must meet the guidelines outlined in OSHA 29 CFR 1910.95.
ii. High noise zones are clearly designated as “Noise Zone” and the requirement for hearing
protection is clear at the zone boundaries (Refer to “Hearing Conservation Standard”).
iii. Workers must wear hearing protection when:
a) Working or entering inside a Permanent or Temporary Noise Zone.
b) Working within 10 meter radius of a portable tool marked with a hearing protection
symbol, when in use.
c) It is mandatory to use ear plug as primary hearing protection for noise zone.
iv. Double Hearing Protection
a) Plugs and muffs are required for double hearing protection noise zones.
b) Area required double hearing protection is identified and posted with safety sign
‘’DOUBLE HEARING PROTECTION REQUIRED’’.
c) Double hearing protection is required for continuous noise levels greater than 100 dBA.
v. Workers that work in identified noise zones are provided with:
a) Earplugs and /or
b) Ear muffs

vi. Earplugs only provide the indicated noise reduction rating (NRR) when properly inserted in
the ear canal.
vii. Earplugs must be inserted and fitted properly. Likewise earmuffs must fit properly.
a) Make sure your hands are clean before inserting the earplugs.
b) Hold the earplug between your thumb and forefinger or first two fingers.
c) Roll (don’t squeeze) the entire earplug between your thumb and forefinger or first 2
fingers to a crease-free cylinder. Continue rolling until the diameter of the plug is as
small as possible.
d) While still rolling use your other hand to reach around or over your head and pull up and
back on your outer ear. This straightens the ear canal and making way for a good fit.
e) Insert the earplug and hold for 10-20 seconds. This allows the earplug to expand and fill
the ear canal.

4.15. Respiratory Protection

i. Respiratory protection must meet the guidelines outlined in OSHA 29 CFR 1910.134.
ii. Respiratory Protection equipment at Q-Chem include:
a) SCBA (Self-Contained Breathing Apparatus)
b) Supplied Air-Line
c) Air purification/cartridge type respirators (Fullface mask and half mask)
d) Disposable Dust Mask
e) Disposable Welding Fume Mask
iii. Any worker whose duties require them to wear respiratory equipment at any point during
their normal job duties for a day/shift, must be:
a) Cleared medically fit to use the respirator
b) Trained in the use of respiratory equipment, including Self-Contained Breathing
Apparatus (SCBA)
c) Clean Shaven in the mask sealing area
d) Fit tested every 12 months

NOTE 8
Medical clearance must be done before attending the respiratory
protection training.

iv. Clean Shaven means:


a) No beards beyond a normal days growth (i.e. daily shaving)
b) Moustache must be neatly trimmed and not below the corners of the lips

c) Side burns must not extend lower than the bottom of the ear
v. When respiratory equipment is required for a job task, employees must wear Q-Chem
provided respiratory equipment only.
vi. Spectacle temple bars or straps prevent good face piece sealing and cannot be worn with a
tight fitting respirator.
a) Special adapter kits are available as a PPE item in the material warehouse which allows
prescription lenses to be worn with a SCOTT AV 2000/3000 face mask.
b) The kit allows prescription lenses to be custom fitted in frames that will fit inside a
respirator (SCBA). See RPE Program for details in ordering these lenses.
vii. The appropriate Unit Superintendent and the Designated Site Representative (DSR) must
determine if the specific job requirements for contractors meets the respiratory protection
criteria.
a) The Safety Section, including Industrial Hygiene, can assist with this required
assessment.
viii. Refer to Respiratory Protection Program (HSE-SAF-PLN-00-0004) for respirator selection
and further details.

4.16. Protective Work Clothing

i. Protective work clothing is used to provide bodily protection against workplace hazards such
as hot liquids, skin irritants, flash fire.
ii. Q-Chem Employees, Contractors and visitors who work/visit in PPE Designated Areas must
wear long sleeved FRC, unless otherwise stated in the Basic PPE Requirements Matrix.
a) Long Sleeve FRC for Q-Chem employees are provided by Q-Chem.
b) Long Sleeve FRC for contract employees shall be provided by the Contractor.
iii. Employees/contractors who are required to wear FRC must also wear the appropriate
undergarments as well.
a) 100% Cotton undergarments are recommended for the best protection against a flash
fire.
b) DO NOT wear non cotton under FRC.
iv. When FRC is mandatory, it must be worn as the outermost garment.
v. Protective work clothing must be maintained and kept clean.
a) If clothing is soaked with hydrocarbons or chemical, they must be immediately removed
and decontaminated or disposed.
b) If clothing is damaged or torn, they must be replaced immediately.
vi. FRC is normally distributed to all employees who either work or visit a PPE Designated Area.
a) Office Workers required to visit the plant (2-pairs).
b) All others (5-pairs).

vii. Chemical resistant clothing must be worn when handling chemicals and cleaning vessels as
per the guidance in the procedure or SDS.
a) Fire Resistant / Chemical Resistant disposable coveralls or PVC/slicker suits (FR) are
available as a specialized PPE item in the materials warehouse.
viii. Corrosive Chemicals work requires the use of Poly Vinyl Chloride (PVC) clothing or apron
complete with the appropriate hand protection.
ix. It is mandatory to tuck your shirt inside your trouser at all the times when using Fire retardant
(FR) shirt and trouser in area where basic PPE is required.
x. Use FR Rain coat in classified area when protection from rain is necessary.

4.17. Fall Protection Equipment

i. Fall protection/arrest equipment includes safety harness, lanyard and shock absorber, D
rings and snap hooks, Belts and buckle attachments, webbing.
ii. Fall arrest system must be checked before each use for any damage.
iii. Color code of the harness must be current (inspected within 3 months).
iv. (Refer HSE-SAF-PRO-00-0011 Fall Protection Section 4.11.2 (v) for inspection and color
code)
v. Contractors must provide fall protection equipment to their employees unless stated
otherwise in the contract.
a) Fall protection equipment shall comply with the requirements of ANSI Z359.1 and ANSI
10.14 or an equivalent international standard.

NOTE 9
Fall arrest equipment without the original manufacture’s tag must not
be used.

b) Contractor inspection records of fall arrest systems /equipment used by the contractor
must be kept at site for inspection by Q-Chem.
c) Refer to Fall Protection Procedure for detailed requirements on fall protection.

4.18. Training

i. All Employees must be trained in the correct use, maintenance and limitations of the PPE
before use.
ii. Q-Chem Designated Site Representative is responsible to ensure that the Contractors are
trained in the correct use, maintenance and limitations of the PPE before use while working
at Q-Chem.
a) It is the Q-Chem Designated Site Representative’s responsibility to ensure Contractors
have the proper PPE and are wearing it properly.
iii. It is required to consider retraining the employee under any of the below condition:

a) Whenever the facility has reason to believe that the employee does not have the
understanding and skill required to use the PPE properly.
b) When changes in the workplace render previous training obsolete.
c) When changes in the type of PPE to be used render previous training obsolete.

5. RESPONSIBILTIES

5.1. Employee

i. Inspect the PPE before each use and replace/report if any defect.
ii. Use PPE only for the intended application.
iii. Store the PPE correctly.
iv. Complete the annual CBT on the Q-Chem PPE standard as per Global Training Matrix.

5.2. Contractor

i. Must issue their employees PPE in accordance with this program and ensure their
employees know how to use and maintain issued PPE prior to arriving at Q-Chem, unless
stipulated otherwise in the contract.
ii. Ensure that all PPE must comply with Q-Chem requirement as specified in Approved PPE
List or acceptable international standard such as ANSI, BS or EN specification approved by
safety section.
iii. Contractors to submit the PPE specification along with sample(s) to respective DSR, who
ensures it meets Q-Chem technical specifications.
iv. All specialised PPE for Contractor to be supplied by Q-Chem or approved by Safety Section.

5.3. Q-Chem Designated Site Representative

i. Responsible to ensure the PPE requirements are being met at all times.
ii. Ensure that all PPE complies with Q-Chem requirement as specified in Approved PPE List or
acceptable international standard such as ANSI, BS or EN specification approved by Q-
Chem safety section.
iii. DSR may use Contractor PPE Form for guidance and documentation of evaluation (See
Attachment 8 )
iv. DSR may request assistance from Safety Section for any clarification on PPE specification,
while conducting the evaluation for Contractors.
v. Ensure that all Specialised PPE for Contractor to be supplied by Q-Chem or approved by
Safety Section.

5.4. Materials Warehouse

i. Store, distribute and replenish the PPE stock.


ii. Maintain the required PPE inventory.
