1. INTRODUCTION

This standard establishes the practice and the minimum requirements for the regular
communication of safety performance information, lessons learned, safety news releases,
safety notices, and similar safety-related information to all personnel working at a Q-Chem
site. To ensure the greatest potential for an injury and incident-free workplace, Q-Chem has
established a practice of regularly communicating safety-related information to all employees
and contractors.

2. SCOPE & PURPOSE

The purpose of this standard is to ensure the regular and continual communication of
necessary safety information throughout the Q-Chem organization in an organized, cascading
manner. The scope of this standard includes all employees and contractors working at any Q-
Chem manufacturing facility or administrative office.

3. DEFINITIONS

 Employee: A Q-Chem direct hire, secondee, or base-load contractor


 MSCM: Monthly Safety Communication Meeting

4. SAFETY COMMUNICATION STANDARD

4.1. Monthly Safety Communication Meeting (MSCM)


i. A Monthly Safety Communications Meeting shall be conducted at least once every
month in Q-Chem manufacturing facilities. A Manager’s Safety Communications
meeting shall be conducted at least quarterly at administrative offices.
a) The respective facility Leadership Team including Managers and Superintendents
and representatives of the Safety Department shall attend the MSCM.
b) Attendance of the meeting shall be documented and maintained by the OE-
HSE&S Administrative Assistant following RIM record-keeping practices.
ii. The Managers Safety Meeting shall be facilitated by the Q-Chem Safety Manager, or a
designate, who is responsible for determining the content of the agenda and the safety
focus topic for each meeting.
iii. The various work departments are responsible for serving as Hosts for the safety focus
topic on a monthly basis as documented in the annual MSCM Host schedule. (See
Attachment 1, HSE-SAF-TBL-00-0009). Each Department Manager is responsible for
ensuring a 15-minute presentation on the selected safety focus topic which is prepared
and delivered when required as per the annual MSCM Host schedule.
iv. The OE-HSE&S Administrative Assistant is responsible to ensure the presentation
material is stored in ‘Q Drive’ and accessible to all employees.

4.2. Department Safety Meetings


i. All departments are required to conduct a structured safety meeting each month. The
Department Manager is responsible for ensuring the monthly safety meeting is
attended by all employees under the control of the department.
ii. Each Department Manager is responsible to ensure relevant safety material is
communicated to employees in their respective departments during the monthly safety
meeting.

a) At a minimum, the material from the current months MSCM shall be discussed.
b) Additional department-relevant safety information shall be discussed at the
department discretion.
c) Attendance of the meeting shall be documented and maintained by the department
Administrative Assistant following RIM recordkeeping procedures.
iii. Employees shall attend each monthly department safety meeting.
a) Employees who miss the monthly department safety meeting shall review the
meeting material and sign the attendance sheet when completed.

4.3. Safety Alerts


i. Safety Alerts are periodically distributed via Company e-mail to affected personnel
when it is necessary to communicate an immediate safety message to the
organization.
a) Safety Alerts are maintained on the Safety Webpage
ii. Safety Alerts shall be approved by the Q-Chem Safety Manager prior to distribution.

4.4. Safety Statistics Communication


i. The Safety Department is responsible for maintaining the Safety Performance Board
located at the plant entrance.
a) Safety Performance Board shall be updated at least once per week on Thursday
b) Minimum information to be displayed on the Safety Performance Board includes
the following:
 Total Recordable Incident Rate as of the current week.
 Cumulative total of days since last recordable injury.
 Cumulative total of number of QSAFE Observations conducted.
c) Other safety metrics may be displayed on the Safety Performance Board as
deemed necessary by business need.
ii. The HSE Administrative Assistant is responsible for maintaining the Safety Statistics
displayed on the Company website.
a) The Safety Statistics shall be updated at least once per week on Thursday.

5. RESPONSIBILTIES

5.1. All Employees


i. Attend each monthly safety meeting as scheduled.
ii. Review monthly safety meeting materials for meetings where attendance was not
possible.

5.2. Department Managers/Superintendents


i. Attend Manager’s Safety Communications meeting as scheduled.
ii. Conduct a structured safety meeting each month, ensuring the material from the
current month’s MSCM is discussed.
iii. Ensure all department employees attend each monthly meeting. Ensure employees not
able to attend the monthly safety meeting review the meeting materials.
