1. INTRODUCTION

Q-Chem records all injuries that occur within the confines of the plant perimeter fence and
Head Quarters in the city of Doha, Qatar.

Recording, trending, and reporting safety statistics is a proven way to improve safety
performance. Reported safety data is also important to benchmark with other facilities and
plays a significant role in rewarding Q-Chem employees and base load contractors through the
incentive program.

2. SCOPE

This program applies to the Q-Chem Plant site and head quarters in Doha, Qatar.

3. DEFINITIONS

Actual Man-hours is the total normal working hours plus overtime, less the man-hours lost
from vacations, and any other leave. Actual man-hour data is provided by HR Department
usually by the end of the following month (i.e. January actual man-hours are received by end of
February)

Estimated Man-hours is the total number of monthly employees multiplied by an accepted


estimation factor (i.e. estimation factor = 173.3)

DSR (Designated Site Representative) is the Q-Chem person with fulltime responsibility for a
contractor while on-site. The DSR must submit man-hour data to the Safety Section for
contractors under their responsibility as a requirement of this program

RIR is the Recordable Incident Rate calculated under the guidance from OSHA

OSHA stands for Occupational Safety and Health Administration which acts as the regulatory
body governing safety compliance in the United States of America.

Employee is a Q-Chem direct hire, CPChem Secondee or QP Secondee

Contractor is a temporary worker

Contractor (Baseload) is a contract worker supplied to Q-Chem to provide day-to-day facility


support, replace vacant permanent positions or to cover employee or other baseload contractor
leaves. Baseload contractors receive direct day-to-day work direction from a Q-Chem
Supervisor/Rep.

Contractor (Other) is a contract worker that does not receive day-to-day work direction from a
Q-Chem Supervisor / Rep. Typically, “other contractors” have a Contract Supervisor providing
the work direction.

Work Direction means specific instructions given by a supervisor in performing the most basic
tasks of a job

4. REQUIREMENTS

4.1. Recording Injury Data

4.1.1. All injuries and nearmisses are reported through the Incident & Nearmiss
Reporting and Investigation Procedure

4.1.2. Classification of all workplace injuries are determined as per the guidance in
OSHA 29 CFR 1904.7

4.1.3. The Safety Section tracks all final incident reports, injury and illness data.

4.2. Determining Baseload Contractor versus Other Contractor

4.2.1. A contractor is considered Baseload Contractor if the following conditions exist:

• Contractor takes day-to-day work direction from a Q-Chem Supervisor/Rep

• Contractor is filling a vacancy normally occupied by Q-Chem staff or other


baseload contractor

4.2.2. A contractor is considered Other Contractor if the following conditions exist:

• Contractor takes day-to-day work direction from a Contract Supervisor

• Q-Chem does not give day-to-day work direction to the contractor

4.3. Man-hours

4.3.1. The Safety Section manages a process for tracking organization man-hours for
the purpose of calculating the Recordable Incident Rate (RIR)

4.3.2. Q-Chem Senior Staff, CPChem and QP Secondees

• HR Department provides monthly report on number of employees on payroll

• Safety Section calculates estimated man-hours based on calculation #1.

• Man-hours in this category remain an estimated number because no credit is


given for extra hours worked and no hours are subtracted for vacations or any
leave, etc…

4.3.3. Q-Chem Non-Senior Staff

• HR Department provides monthly report on number of employees on payroll

• Safety Section calculates estimated man-hours based on calculation #1.

• HR Department provides actual man-hours to Safety Section by the end of the


following month.

• Safety Section corrects the estimated man-hour figure with the actual man-
hours provided by HR. Correcting for actual man-hours may adjust the RIR
calculation (up or down) to more accurately reflect the RIR.

4.3.4. Baseload and Other Contractors

• All Contractors have a DSR (Designated Site Representative), as indicated on


the Man-hour Reporting Matrix – emailed monthly

• Safety Section e-mails the previous month’s Man-hour Reporting Matrix to the
list of DSRs at the end of every month.

• DSR must calculate the contractor man-hours for workers under their
jurisdiction using one of the four approved calculations for estimating man-
hours or by tracking actual man-hours using a time sheet.

• The DSR must indicate the man-hours in the Man-hour Reporting Matrix and
respond by the last Thursday of each month with hours worked by contractors
under their jurisdiction

• Safety Section receives all monthly man-hour reports from the DSRs and
compiles the hours worked by Contractors (Baseload and Other). If the DSR
does not respond, Safety Section assumes “no-change” from previous month.

• At the end of each month, the Contracts Department provides Q-Tech man-
days for the current month and actual man-hours for the previous month to
Safety Section.

4.4. Calculating Estimated Man-hours

4.4.1. Calculation #1 - Estimated Monthly Man-hours based on an employee working


40 hrs a week for 52 weeks a year divided by 12 months

• Total # of Employees X 173.3

4.4.2. Calculation #2 – Contractors Estimated Monthly Man-hours based on a


contractor working 48 hrs a week for 52 weeks a year divided by 12 months

• Total # of Employees X 208

4.4.3. Calculation #3 – Contractors Estimated Monthly Man-hours (12hr shifts and


5 days/week) working 60 hrs a week for 52 weeks a year divided by 12 months

• Total # of Employees X 260

4.4.4. Calculation #4 – Contractors Estimated Monthly Man-hours (12hr shifts and


6 days/week) working 72 hrs a week for 52 weeks a year divided by 12 months

• Total # of Employees X 312

4.5. Calculating Actual Man-hours

4.5.1. Actual Man-hours only applies only to Q-Chem non-senior staff and some
baseload and other contractors.

• Record and report actual hours worked on a timesheet (normal working hours
plus overtime)

4.6. Calculating RIR (Recordable Incident Rate)

4.6.1. Q-Chem RIR is calculated using the recordable incident data and man-hours for
the following personnel:

• Q-Chem Direct Hire


• CPChem Secondees
• QP Secondees
• Baseload Contractors

4.6.2. Contractor RIR is calculated using the injury data and man-hours for all Other
Contractors regardless of time on-site.

4.6.3. RIR Formula is calculated based on guidance from OSHA

RIR = # of Recordables X 200,000


Total Man-hours

• 200,000 (based on 100 fulltime employees working 40hrs per week for 50
weeks per year)

4.7. Reporting

4.7.1. Q-Chem Safety Section is responsible to report injury data and RIR on a weekly,
monthly, quarterly and annual basis.

• Weekly Reports are sent every Thursday morning, which include the safety
stats for the week ending and including Thursday.

• Monthly Reports are prepared on the last day of the month and are
communicated at the Managers Monthly Safety Meeting and to CPChem Head
Quarters (CPChem Senior Safety Specialist)

• Quarterly Reports are prepared on the last day of every quarter and sent to
Qatar Petroleum via fax

• Annual Reports are prepared on the last day of the year, which includes the
overall Q-Chem safety performance, and final reported RIR. When the final
year-end RIR is calculated and reported at end of year (Dec 31), it is

permanent and must not be changed. This means that December man-hours
will be estimated based on number of employees and will not be corrected for
actual. All other 11 months, the non-senior staff and Q-Tech man-hours are
corrected for actual

4.8. Training

4.8.1. Specific annual training is required for those that administer reporting through the
Incident & Nearmiss Reporting and Investigation Procedure and man-hours
reporting within this program.

• Designated Site Representative training covers responsibilities for reporting


contractor man-hour data to Safety Section and those that administer
reporting.

5. RESPONSIBILTIES

5.1. Safety Section

5.1.2. All record keeping for RIR calculations

5.2. Designated Site Representative (DSR)

5.2.1. Calculating estimated man-hours for applicable contractors using the approved
calculation methods in this program

5.2.2. Identify calculation method on the monthly Man-hour Reporting Matrix
