Re: Decision Based on Compromise Agreement

I, PATRICIA YOLANDA M. HOLGADO, Filipino, of legal age, and resident of Brgy.

Poblacion, Sta. Teresita, Batangas, Philippines, having been duly sworn in accordance with
law, hereby depose and state that:

1. I am one of the authorized representatives of CARLO M. HOLGADO in Civil Case

No. NC-2015-2528 to NC-2015-2531, NC-2015-2532 to NC-2015-2533 and NC-2015-
2534 to NC-2015-2540 for Annulment of Extrajudicial Foreclosure of Real Estate
Mortgage, Loan Documents and Damages which consolidated cases involving several
parcels of land located in Mendez, Cavite.

2. Due to the abovementioned cases, Notices of Lis Pendens were issued and
annotated on the lots subject of the said consolidated cases.

3. Civil Case No. NC-2015-2528 to NC-2015-2531, NC-2015-2532 to NC-2015-

2533 and NC-2015-2534 to NC-2015-2540 involves parcels of land covered by the

5. Pursuant to a Decisions Based on Compromise Agreement issued by the Regional
Trial Court Branch 15 of Naic, Cavite, the abovementioned consolidated cases involving the
subject parcels of land were DISMISSED.

6. We are in the process of cancelling the annotation of lis pendens on the titles of all
the subject lots.

7. I am executing this affidavit to declare the truth of the foregoing facts, for the
purpose of cancelling the annotation of Notice of Lis Pendens on the all of the
abovementioned titles and for whatever legal purpose this may serve.
