I am writing this letter to express my sincere apology for the late submission of my
reinstatement documents. Supposedly, my reinstatement date was on January 25, 2022 but
sad to say I didn’t submit a requirement immediately due to the following reason:

I – Lacked knowledge regarding the necessary steps to do on reporting back to

school.

II – Lacked assistance /follow-up from my direct superior.
