Respectfully forwarded to the University President through the Vice President for
Research, Development and Extension, recommending approval of the herein attached
request of PROF. MARISSA MENDOZA, College of Arts and Humanities to conduct
extension activity entitled “CAH-Adopt a Barangay Project“ at Barangay Kamuning
Puerto Princesa City on March 22, 2024, with the following recommendations.

1. Expenses such as meals (snacks, lunch) amounting to P 7,500.00 shall be charged

to UESO Fund 164.
2. Involved faculty members who are supposed to have day off during the date of the
activity will be entitled to claim service credit
3. The university vehicle will be used for the abovementioned purpose.
4. The conduct of extension activity is recommended on Official Business.
