Assessment Instructions:
In Assessment 1 you created a plan by identifying your own personal stressors. Now you
need to translate this into meaningful strategies for you as an individual.
In this assessment you will develop your own personal Wellbeing Strategy with the support of
your mentor or manager. You will capture this strategy in the template provided by completing
the tasks outlined below. You will identify an overarching goal and then break this down into
SMART goals and the activities you need to engage in to achieve these goals.
Ensure that your personal strategy is enabling you to achieve a healthier work life balance.
Consider how it will assist you to achieve your work goals and objectives.
You will also be required to answer some questions that relate to your plan.

1. From the plan you created in Assessment 1, create your Wellbeing Strategy
own Wellbeing Strategy using the template below.
Identify at least three (3) overarching goals that you have
for your own health and wellbeing that will also positively
impact your work environment. Write these goals using
the SMART goal model (specific, measurable,
actionable, reliable and timetabled). From this create the
strategy to meet those goals. Consider the activities that
will enable you to complete the strategy, the resources
required and the timeframes for each goal.
2. Now you have developed your strategy you need to Meeting notes/minutes
communicate this with your mentor and/or any other
relevant personnel to gain feedback and ensure that the
support you require is actioned. Arrange a meeting and
discuss the strategy and seek feedback. Document this
meeting (you can create meeting notes or minutes for
this task). You can also record your conversation as
secondary evidence.
