Assessment Instructions:
 In Assessment 1 you created a plan by identifying your own personal stressors. Now you
need to translate this into meaningful strategies for you as an individual.
 In this assessment you will develop your own personal Wellbeing Strategy with the support of
your mentor or manager. You will capture this strategy in the template provided by completing
the tasks outlined below. You will identify an overarching goal and then break this down into
SMART goals and the activities you need to engage in to achieve these goals.
 Ensure that your personal strategy is enabling you to achieve a healthier work life balance.
Consider how it will assist you to achieve your work goals and objectives.
 You will also be required to answer some questions that relate to your plan.

Due Dates and Assessment Support:

Please refer to the Learning and Assessment Plan (LAP) for details of your lecturer and specific
assessment due dates.

Addressing and submitting your assessment:


1. This assessment is to be completed by students individually.
2. All questions in this assessment must be addressed fully to meet assessment requirements.
3. Ensure that your assessment question responses are professionally presented with
correct grammar, formatting and punctuation and are typed in black (not red), size 11 font
Arial, using the template provided in this assessment tool.
4. Check your uploads each time as there could be times when the Word document does not
upload well on Blackboard Dropbox, and please upload a PDF version, it may save your text
and graphs from the “floating” syndrome as this can at times overlap text on Blackboard that
makes it difficult to decipher your answers.
5. Address each question fully and adhere to the recommended response instructions given.
6. Remember to include List of References as per APA referencing methods.
7. Include your name, student number and assessment number in the footer of each page of
your Assessment.
8. Save your file with good digital file practice ie. BSBPEF401 PHWB_Assm#2_your
name+student ID
9. Assessments must be submitted on or before the due date as indicated in the LAP.
10. Once you have completed your responses and tasks, please submit the assessment through
the appropriate assessment Dropbox, in Blackboard. Detailed instructions are included in
Blackboard, within the assessment tab.
11. Students working on assessments off campus must ensure that all work submitted is their
own (as outlined in the Blackboard assessment tab). To ensure this, and prior to providing an
outcome mark, lecturers may ask students additional questions related to the assessment
task.
12. If you are unsure about any of these instructions or require clarity on any of the assessment
questions, please contact your lecturer via email, without delay.

1. From the plan you created in Assessment 1, create your Wellbeing Strategy
own Wellbeing Strategy using the template below.
Identify at least three (3) overarching goals that you have
for your own health and wellbeing that will also positively
impact your work environment. Write these goals using
the SMART goal model (specific, measurable,
actionable, reliable and timetabled). From this create the
strategy to meet those goals. Consider the activities that
will enable you to complete the strategy, the resources
required and the timeframes for each goal.

2. Now you have developed your strategy you need to Meeting notes/minutes
communicate this with your mentor and/or any other
relevant personnel to gain feedback and ensure that the
support you require is actioned. Arrange a meeting and
discuss the strategy and seek feedback. Document this
