PERSONAL STATEMENT

I have vast experience in collecting, entering and basic analysis of the data entered
in both dhis2 and Covax ministry of health systems for health information. I have
also participated in quantifying the HMIS tools need by the facilities where I have
served

 Listing all VHT in the designated parish per village level in Masindi
district, obtaining informed consent and implementing all research
principles, interviewing the VHT’s using both English and translated
tool on ODK, Daily reporting to the coordinator of the work progress,
attend debriefs and ensure that the completed successful surveys are
submitted on the server
 Conducting household surveys in areas where living goods VHT
operate, obtaining informed consent from both the participants, VHT’s
and house hold mothers with children aged 2 years and below and
community health worker stake holders, conduct a survey to explore
the value and impact of financial incentives on CHW’s job satisfaction,
personal income and livelihood,

duplicates of clients in the system which was causing a backlog.

as VHTs in the districts of Amudat, Moroto, Kamuli and Buyende. Where

were skilled with the knowledge of filling SPT forms as well as scanning
them.
 Supported different Districts in summarising and entry of both the daily
reports and
campaign summary reports for Covid 19 vaccination in the Epivac system.
