Responsibilities:
 Enters HIV clients information into NMRS Mobile Tracker Device on daily basis.
 Enters new patients’ information, enrolment, and initiation, visit/drug pick up, clinical
encounters, drug administration, adherence, and side effects, on the electronic medical
records provided for use.
 Conducts bi-monthly patients satisfaction survey.
 Daily update of existing patients’ information, including but not limited to that of
visit/drug pick-up clinical encounters, drug administration, and side effects, on the
electronic medical records (EMR) provided for use.
 Reviews daily entries for completeness and correctness of data.
 Verifies entered data by reviewing, correcting, making changes to data in electronic
medical records.
 Conducts daily back-up of electronic medical records information entered on the EMR
platform.
 Retrieves and properly re-file patients’ file/folder during and after visits.
 Ensure proper archiving of obsolete registers.
 Ensure all source documents (ART Registers, daily appointment registers, and patient
attendance logs) are properly updated at all times (as applicable).
 Support the review of monthly programmatic reports before submission to next level of
reporting including LACA focal persons and IP(as applicable)
 Generate automated weekly reports from the EMR database for clinical decision making.
 Ensure all patient-level information are discretely and securely stored away from
unauthorized persons.
 Adhere to all confidentiality procedures with respect to clients physical/electronic
medical records.
 Ensure safety of EMR hardware/software at designated fire proof safe/cabinets at the
medical records unit.
 Generate xml reports weekly for submission to the National Data Repository.
 Participate in the conduct of data collation and validation exercises in assigned facilities.
 Participate in the conduct of internal DSC and maintain logs on-site.
 Maintained data base of assigned health facilities.
 Corrected data discrepancies by contacting patients to acquire clarification on missing or
erroneous data.
 Troubleshoots entry errors and implements corrective measures in a timely manner
 Participate in the conduct of patient audit.
