This letter serves to inform you of an opportunity to be in collaboration with Friendly Smiles
Dental.

We are offering the services of Dental Cleaning & Examination to the students of your
institution at a cost of $3000 per student.

We will travel with our portable unit and provide the service at the school on the day we agree
on.

The school will be in charge of submitting the names of the students who are desirous of the
service and the monies collected.

We would require a reasonable sized area to set up and somewhere to wash our hands and
instruments.

Please note, 10 % of the monies collected will be given to the school as a donation.
