Process name Assigning Acuity Levels to Patients


Project purpose It has been observed that there is a number of patients that are LWOT. It is
against the objectives of the organization. During the 4-day Kaizen meeting, the
team has identified few causes to the issue.
There are inconsistencies in the process through which the acuity levels are
assigned to the patients.
There have been several cases where the providers take a lot of time with the
patients despite the low acuity levels of the patients.
There is no proper provider – patient prioritization mechanism.

In this regard the team has made process improvement A3 papers to assist with
the project of improving processes.

Project scope Included:


To improve the processes as per the following objectives and the mechanisms
involved in the theories mentioned in Annex ‘A’

No further staff is to be hired for the sake of process improvement
