Causes of the Gas Leak:
Mechanical Failure: Investigate the mechanical integrity of the plant's equipment,
including storage tanks, valves, and pipelines, to identify any potential weaknesses
or malfunctions that could have led to the release of methyl isocyanate (MIC) gas.
Safety Systems Failure: Evaluate the effectiveness of safety systems, including
alarms, emergency shutdown procedures, and ventilation systems, to determine if
there were any deficiencies or failures that contributed to the severity of the leak.
Human Error: Examine the role of human factors such as operator error,
inadequate training, and communication breakdowns in the events leading up to
the gas leak.
Management Oversight: Assess the management practices and decision-making
processes within UCC, including maintenance procedures, risk assessment
protocols, and compliance with regulatory standards, to identify any systemic
failures or lapses in oversight.
