Re: Reprimand of Insubordination

Dear Junriel,

This is to inform you that your immediate supervisor has complained regarding you work attitude
in the workplace. In her complain, (1) you refuse to follow instructions, all of which are related to
your job functions (2) doing unauthorize tasks upon yourself without her approval.

Please explain in writing within three (3) days why you should not be terminated for doing such
actions which may be viewed as insubordination.
