Engr Raveen mechanical site supervisor of STKT fall down in the cable trench. That him damage to his forehead . While now he is the hospital for some medication and treatment . Mr jay canda also accidentally hit his forehead on the edge of air ducting and that cause him injured , After that incident happen his foreman bring him on the office and treat some medication his injury . all this happen last Tuesday morning September 5,2011. On the other accident happens yesterday September 6,2011 , One of the steel columns already erected at the main process mezzanine floor area electrical control room accidentally hit by the scaffoldings & that cause him fall ll down and hit the equipment. But after few hours site top management 0f MTMM, AMANA, FIBEREX & EICO conduct meetings for the solutions how to rectify the equipments or replacing new equipments. Also in CSM area besides forming dock area amana waterproofing materials fire

Engr . Raveen is conducting surveillance inspection at main process area besides electrical cable trench . he forget the trench and accidentally step into the open trench . It was observed that the area is very dark and with safety barade on the peremeter of cable trench. Mr jay canda during installation air ducting he also accidentally hit his forehead on the edge of and that cause him injured, and because his not wearing hardhot . During erection / installation of the steel columns of amana structural steel erection team at main process area mezzanine floor electrical control room . One of the steel columns already erected accidentally hit by the scaffoldings & that cause him fall down and hit the equipment.

SAFETY MEASURES & PRECUATIONS : Saudi tkt should provide safety site officer on daily basis to determine all areas unsafe and provide safety precautions as per EICO safety manuals. Conduct daily safety tool box meeting to all site staff before starting the activities .

Provide all workers safety hardhot , safety shoes , gloves & safety harness afflicable for all workers working above 1.8m to the ground. Safety officer should provide safety warnings in all areas are unsafe to everybody working at site.

Amana safety officer should monitor carefully at site specially areas have already equipments to avoid the incidents happen again . All installation of steel structures erected should provide
