1. EXECUTIVE OVERVIEW
The purpose of this contract is to facilitate the construction of all electrification work in
the LimLanga Cluster in the Distribution Business. All work shall be carried out to the
specified requirements, standards and quality as set out by Eskom while observing and
complying to all safety and environmental requirements at the agreed rates. All project
specifications will be included in the relevant Project Specific Agreement for each
project.

2. PURPOSE OF THIS SHE PLAN


The purpose of this document is to establish a plan for Implementing SHOMO KA MATLA TRADING
ENTERPRISE Safety, Health and Environmental Program. It shall form part of our SHE tender
returnables for this project. It is at this stage that we have to demonstrate competence to
comply with the applicable legislation and also the ability to do so. Compliance with the
provisions of the Occupational Health and Safety Act and associated regulations,
including the Construction Regulations 2014 is mandatory and this plan with the
necessary additions and amendments, shall be evaluated and approved by Eskom prior
to commencing work.

3. SCOPE OF THIS SHE PLAN


This document details how health and safety issues shall be managed at Shomo Ka
Matla Trading Enterprise. It demonstrates how resources and processes shall be
managed in order to ensure that activities for this Eskom contract is executed within
budget and on time. The Occupational Health and Safety Act No 85 of 1993 as amended
and the Construction Regulations 2014 respectively mandate our organization, Shomo
Ka Matla Trading Enterprise to provide and maintain a safe environment that is free of
risks for all our employees, to provide and demonstrate to ESKOM a suitable, sufficiently
documented and coherent site specific health and safety plan, based on their
documented health and safety specifications as contemplated in regulation CR 5(1)(b).
To that affect, our organization commit to do everything reasonably practicable in
ensuring compliance with the aforementioned.

4. BRIEF SCOPE OF WORKS


The scope of work for the electrification project within the Limlanga Cluster -Limpopo &
Mpumalanga covered by this contract includes:

1. Overhead Support and Excavations:


• Excavate backfill and compact as per Eskom Standard and import backfill soil
where required as per Eskom Standard for holes and trenches for poles, stays,
struts, flying stays and earth electrodes.
• Planting of structure, flying stay and strut poles are included in this section. Poles
are Eskom free issue material. Pole top diameter ranges from 140mm to 220mm.
• Supply and install stays, flying stays, struts including accessories. Accessories
include stay wire, stay rods; stay plates, soil anchors, stay insulators, guy grips stay
brackets, hardware, anti-climbing devices, stay guards and danger labels.
• Take delivery and store Eskom free supply materials safely (poles, x-arms).

• Transport and deliver Eskom free supply materials from Eskom Stores to the Site
Camp, and vice versa (poles, x-arms).
• Supply strut pole brackets. • Blasting and drilling of rock holes (to be paid on
verification of the Clerk of Works).

2. MV overhead system
• Install Eskom issued marked conductor. Material quantity to allow for 5% sag in
addition to actual conductor length quantity. Installation includes handling, stringing
and final sagging.
• Take delivery and safely store Eskom free issue material (conductor).
• Transport and deliver conductor from Eskom Stores to the Site Camp, and vice
versa.

3. MV equipment and switchgears


• Install transformer / recloser / voltage regulator and MV metering units as per
relevant Eskom DDT 1800 series assembly drawing and DX OUs, SI Engineering
instructions. All auxiliary equipment to include station and Distribution MV, LV surge
arrestors, control boxes, metering kiosks, jumper terminations, anti-climbing devices,
LDPE pipe covered jumpers as per 02TB-023 and danger labels, channel irons,
cradles, stand-off insulators, conductor bus bars and suitable equipment labels.
Transformers / reclosers / voltage regulators and MV metering units will be Eskom
free issue material. • Supply and install MV isolators according to Eskom DDT 1800
series, where applicable. Accessories to include solid inserts, expulsion type fuses,
jumper terminations, hardware, LDPE pipe covered jumpers as per 02TB-023.
Disconnectors to be supplied with suitable labels. Phase-phase isolators to only
have one covered jumper. The jumper support insulator is excluded and if required,
it may be selected as miscellaneous item. Cross arms are included.
• All relevant earthing shall be included as per relevant Eskom DDT 1800 series,
DDT 0600 and DDT 0200 series assembly drawing and OU specific SI Engineering
instructions.
• Allowance shall be made for the complete testing and commissioning of Medium
Voltage equipment. Tests to include earth electrode resistance measurement.
Transformer to include LV earth electrode resistance measurement.
• Take delivery and safely store Eskom free issue materials (transformers, reclosers,
VR’s and MV Metering units, control boxes and earthling kits).
• Transport and deliver from Eskom Stores to the Site Camp, and vice versa.

4. MV cable work, trenching, installation, and removal


• Take delivery of and safely store on site miniature substations, ring main units, MV
cable and pre-cast plinths as supplied as free issue from Eskom. Transport to site
and install as per Eskom DT Standards. All civil work to be included.
• Install as per Eskom Standard and DDT 0854.XPLE Insulated Type A3 core cable
300mm and 185mm sq. stranded copper, PVC bedding, galvanized steel wire.
Allowance is made for crossing the road, plantations, 2m slack on both sides and
9m up the MV pole.
• Allow for railway, road and driveway crossings as indicated below. Included are
cutting and/or removal of hardened surfaces, excavations to required depth,
160NBx3.2mm wall thickness PVC pipes (number of per crossing as indicated on
drawing), backfilling and reinstatement of hardened surfaces to, at least, as good a
condition as before removal.

• Supply and install all materials for the complete earthing of transformer structures
for ABC networks as specified. Included in the rate shall be all required spikes,
insulated copper conductor, galvanized conducts, staples, bare copper, excavations,
backfilling, etc. The rate shall allow for one MV earth electrode only, consisting of
four earth spikes, in accordance with the specifications.

5. House service connections (infills)


• House connections (Type A and Type B). Accessories include house labels, pigtail
bolts, strain clamps, cable saddles, passive base / ready board hardware, threaded
rods. Meters, Customer Interface Units, ready boards, service cable, poles and
excavations are measured elsewhere.
• Install Customer Interface Units, ECU’s, ready boards, passive base units.
Accessories include hardware. Allow for suitable sealing and testing of meters for
COC, OHS Act and Eskom requirements. Eskom meters and CUI’s are free issue.
• Install Eskom issued Eskom marked conductor. Material quantity to allow for 5%
sag addition to actual conductor length quantity. Installation includes handling,
stringing and final sagging. The quantity shall be conductor length.
• Completing of PCS file for uploading of connections to be done immediately after
energisation and registering of all connected customers on the Eskom SOC Limited
customer management system within 7 days after energisation under dead or live
conditions.
• Abridged certificate of compliance or installation of certificate to be completed by a
competent resource and to be submitted within 7 days after energization. 3.7. LV
cable work, trenching, installation, and removal
• Take delivery of and safely store on LV cables on site as supplied as free issue
from Eskom. Transport to site and install as per Eskom DT Standards and DDT
0854.
• Take delivery of and safely store LV cables on site as supplied as free issue from
Eskom. Supply and install joint kits for jointing of LV PVC / SWA / PVC cables as per
Eskom DT Standards.
• Allow for LV cable trenching as indicated on the drawings. Excavation and levelling
of cable trenches as per Eskom DT Standards and DDT 0854. Any services (e.g.,
other cables, water pipes, etc.) damaged by the Contractor shall be made good by
him at his own. Crossings and trench backfilling measured elsewhere.
• Allow for the complete testing and commissioning of the LV underground system,
include earth readings, feeder continuity tests, phasing, etc. Included are the test
certificates and all documentation required to hand-over the system.

6. Dismantling
• Dismantle MV line (and cables, if any) including all accessories and auxiliary
equipment. • Dismantle LV line (and cables, if any) including all accessories and
auxiliary equipment.
• All leftover assets and scrap materials to be returned to Eskom RDC (using IM104
Process).
• Contractor to sign the handover certificate on completion of the project.
• All tests done during construction are to be handed over to the COW before outage
day / project completion.

7. Underground cable installation


• Trenching, pulling, laying, jointing, termination, and backfilling.

6. DEFINITIONS
The definitions below are taken directly from the Construction Regulations 2014:

TERM/PHRASE MEANING
"Agent" A competent person who acts as a representative for a client
"Angle of repose" The steepest angle of a surface at which a mass of loose or
fragmented material will remain stationary in a pile on the surface,
rather than sliding or crumbling away;
"Bulk mixing plant" Machinery, appliances or other similar devices that are assembled in
such a manner so as to be able to mix materials in bulk for the
purposes of using the mixed product for construction work;
"Client" Any person for whom construction work is being performed;
“Competent person" A person who -
(a) has in respect of the work or task to be performed the required
knowledge, training and experience and, where applicable,
qualifications, specific to that work or task: Provided that where
appropriate qualifications and training are registered in terms of the
provisions of the National Qualification Framework Act, 2000 (Act
No.67 of 2000), those qualifications and that training shall be
regarded as the required qualifications and training; and

(b) is familiar with the Act and with the applicable regulations made
under the Act;
"construction manager" means a competent person responsible for
the management of the physical construction processes and the
coordination, administration and management of resources on a
construction site;
"Construction site" A work place where construction work is being performed;
"Construction supervisor" A competent person responsible for supervising construction
activities on a construction site;
"Construction vehicle" A vehicle used as a means of conveyance for transporting persons
or material, or persons and material, on and off the construction site

for the purposes of performing construction work;
"Construction vehicle" Vehicle used as a means of conveyance for transporting persons or
material, or persons and material, on and off the construction site for
the purposes of performing construction work;
"Construction work" Any work in connection with -
(a) the construction, erection, alteration, renovation, repair,
demolition or dismantling of or addition to a building or any similar
structure; or
(b) the construction, erection, maintenance, demolition or dismantling
of any bridge, dam, canal, road, railway, runway, sewer or water
reticulation system; or the moving of earth, clearing of land, the
making of excavation, piling, or any similar civil engineering structure
or type of work;
"Construction work permit" A document issued in terms of regulation 3;
"Contractor" An employer who performs construction work;
"Demolition work" A method to dismantle, wreck, break, pull down or knock down of a
structure or part thereof by way of manual labour, machinery, or the
use of explosives;
"Design" In relation to any structure, includes drawings, calculations, design
details and specifications;
"designer" means –
(a) a competent person who -
(i) prepares a design;
(ii) checks and approves a design;
(iii) arranges for a person at work under his or her control to prepare
a design, including an employee of that person where he or she is
the employer; or
(iv) designs temporary work, including its components;
(b) an architect or engineer contributing to, or having overall
responsibility for a design;
(c) a building services engineer designing details for fixed plant;
(d) a surveyor specifying articles or drawing up specifications;
(e) a contractor carrying out design work as part of a design and
building project; or
(f) an interior designer, shop-fitter or landscape architect;
"Excavation work" The making of any man-made cavity, trench, pit or depression
formed by cutting, digging or scooping;
"Explosive actuated fastening device" A tool that is activated by an explosive charge and that is used for
driving bolts, nails and similar objects for the purpose of providing
fixing;
"Fall arrest equipment" Equipment used to arrest a person in a fall, including personal
equipment, a body harness, lanyards, deceleration devices, lifelines
or similar equipment;
"Fall prevention equipment" Equipment used to prevent persons from falling from a fall risk
position, including personal equipment, a body harness, lanyards,
lifelines or physical equipment such as guardrails, screens,
barricades, anchorages or similar equipment;
"Fall protection plan" A documented plan, which includes and provides for –
(a) all risks relating to working from a fall risk position, considering
the nature of work undertaken;
(b) the procedures and methods to be applied in order to eliminate
the risk of falling; and

(c) a rescue plan and procedures;

"Fall risk" Any potential exposure to falling either from, off or into;
"Health and safety file" A file, or other record containing the information in writing required by
these Regulations;
"Health and safety plan" A site, activity or project specific documented plan in accordance
with the client's health and safety specification;
"Health and safety specification" A site, activity or project specific document prepared by the client
pertaining to all health and safety requirements related to
construction work;
"Material hoist" A hoist used to lower or raise material and equipment, excluding
passengers;
"Medical certificate of fitness" A certificate contemplated in regulation 7(8);
"Mobile plant" Any machinery, appliance or other similar device that is able to move
independently, and is used for the purpose of performing
construction work on a construction site;
"National Building Regulations" The National Building Regulations made under the National Building
Regulations and Building Standards Act, 1977 (Act No. 103 of 1977),
and promulgated by Government Notice No. R.2378 of 30 July 1990,
as amended by Government Notices No's R.432 of 8 March 1991,
R.919 of 30 July 1999 and R.547 of 30 May 2008;
"Person day" One normal working shift of carrying out construction work by a
person on a construction site;
"Principal contractor" An employer appointed by the client to perform construction work;
"Professional Engineer or Professional Certificated A person holding registration as either a Professional Engineer or
Engineer" Professional Certificated Engineer in terms of the Engineering
Profession Act, 2000 (Act No. 46 of 2000);
"Professional Technologist" A person holding registration as a Professional Engineering
Technologist in terms of the Engineering Profession Act, 2000;
"Provincial director" The provincial director as defined in regulation 1 of the General
Administrative Regulations, 2003;
"Saffold" A temporary elevated platform and supporting structure used for
providing access to and supporting workmen or materials or both;
"Shoring" A system used to support the sides of an excavation and which is
intended to prevent the cave-in or the collapse of the sides of an
excavation;
"Structure" (a) Any building, steel or reinforced concrete structure (not being a
building), railway line or siding, bridge, waterworks, reservoir, pipe or
pipeline, cable, sewer, sewage works, fixed vessels, road, drainage
works, earthworks, dam, wall, mast, tower, tower crane, bulk mixing
plant, pylon, surface and underground tanks, earth retaining
structure or any structure designed to preserve or alter any natural
feature, and any other similar structure;

(b) Any falsework, scaffold or other structure designed or used to


provide support or means of access during construction work; or
(c) any fixed plant in respect of construction work which includes
installation, commissioning, decommissioning or dismantling and
where any construction work involves a risk of a person falling;
"Suspended platform" A working platform suspended from supports by means of one or
more separate ropes from each support;
"Temporary works" Any falsework, formwork, support work, scaffold, shoring or other
temporary structure designed to provide support or means of access
during construction work;
"The Act" The Occupational Health and Safety Act, 1993 (Act No. 85 of 1993);
"Tunneling" The construction of any tunnel beneath the natural surface of

the earth for a purpose other than the searching for or winning
of a mineral

8. MANAGEMENT COMMITMENT AND LEADERSHIP

Shomo Ka Matla Trading Enterprise is fully committed to the SHE Policy and will provide
leadership through:

- Planning the work in a safe manner prior to executing the tasks, conducting Risk
Assessments for all activities and equipment;
- Providing safe equipment, Personal Protective Equipment and safe systems of work;
- Provision of suitable training, supervision, information and instruction to all personnel
engaged on the construction phases of the project;
- Acceptance and accountability of the responsibility for accidents and incidents in the
workplace;
- Introduction of systems to encourage active involvement of management, workforce and
the local community in providing suggestions and proposals for improvements in SHE
performance;
- Providing active support for the construction team by way of specialist advice in safety
and quality areas;
- A continuous effort, driven by top management, will be made throughout the construction
phases to motivate Shomo Ka Matla Trading Enterprise personnel to take an active part
in safety issues.

9. ROLES, ACCOUNTABILITIES AND RESPONSIBILITIES

9.1. Our Construction Manager


As an 8(1) appointee, our construction manager carries accountability and responsibility
of ensuring a safe working environment that is without risk for both our employees and
subcontractors. It is to that end therefore that our 16(2) (Contracts Manager), in
appointing him in writing, vested certain roles and responsibility on his shoulders.
Our construction manager is fully aware of his primary responsibility to manage all the
construction work on a single site that includes ensuring the overall occupational health
and safety compliance, Upon having considered the size of the project, he shall and in
writing, appoint one or more assistant construction managers for different sections on
site: He shall not manage any construction work on or in any construction site other than
the site in respect of which he or she has been appointed.

After consultation with the client and having considered the size of the project, the
degree of danger likely to be encountered or the accumulation of hazards or risks on the
site, he shall appoint a full-time or part-time construction health and safety officer in
writing to assist in the control of all health and safety related aspects on the site. He shall
ensure that any health and safety officer to be appointed is registered with the statutory
boy namely, the SACPCMP. Furthermore, he shall be satisfied that he has necessary
competencies and resources to assist us.

As contemplated in CR 8(7), he shall and in writing, appoint a construction supervisors


responsible for construction activities and ensuring occupational health and safety
compliance on the construction site. Once again, upon having considered the size of the
project, he shall and in writing appoint one or more competent employees for different
sections thereof to assist the construction supervisor. Similarly to the appointed assistant
construction manager, our construction manager shall ensure that no construction
supervisor appointed as in CR 8(7) may supervise any construction work on or in any

construction site other than the site in respect of which he or she has been appointed,
unless if a sufficient number of competent employees have been appropriately
designated on all the relevant construction sites.

9.2. Our OHS Practitioners and Sub-contractor OHS Practitioners


Construction regulations 2014 states very categorically that any contractor that appoints
or want to appoint a health and safety officer shall ensure that he is registered as a
Construction Health and Safety Officer(CHSO) with a statutory body, namely the
SACPCMP in this regard. Our organization shall further ensure, as necessary that we
also appoint a Construction Health and Safety Manager (CHSM) who will be reporting
directly to the Project Manager/Contracts Manager/CEO. We will also ascertain that the
Sub-contractors comply with the legislation as well, i.e. appointing registered
practitioners.

9.3. Our employees on the project


Section 14 of the Act clearly outlines the duties of the employees and we will ensure as
far as reasonably practicable that they fully comply with all national legislation
requirements and do conform to Eskom requirements and SHE Specifications. The
aforementioned is motivated by the notion that they are responsible for their own safety
and that of their co-workers.

9.4. Our client (Eskom)


CR6 on the responsibilities of the Client or Client Agents are broken down into
exact details of what is expected to commence any construction work. The
responsibilities are clearly defined and lie squarely on the client’s onus and leave no
room for grey areas in future. It is on that background therefore that we expect Eskom’s
hand to be more tighter on PC’s than ever before hence our continous commitment to
comply fully to any instructions and requirements Eskom may acquire arising from its
duties, which such duties include amongst others:

- Preparation and provision of a baseline risk assessment to us, where applicable provide
the same to the DoL.

- Preparation and provision of the designer with the health and safety specifications which
is included in the tender documents.

The client shall, amongst its responsibilities also:


- Verify/evaluate competencies and our resources before appointment i.e. during technical
evaluations.

- Ensure that the Owner of the structure shall undertake assessments once every 6
months for first 2 years and thereafter yearly.

- Ensure that the we have made provision of H&S cost in the submission of tender
documents

- Ensure that our safety file is kept and maintained..

- Investigate all incidents, accidents or near misses – with main focus on leading
exposures and lagging implications.

- Agree on and formal approval of our OHS plan before work may start

- Undertake at least monthly compliance assessments to ensure that the agreed OHS
plan is implemented and maintained

- Appoint us in writing

- Communicate existing and emerging risks to us as well as the changes to the project.

9.5. Our Responsibilities as a Principal Contractor


As one of the potential Principal Contractors for this project, we hold the safety of all
employees on site at high esteem and our primary obligation is that our employees,
sub-contractor’s, visitors and the public at large are provided with maximum level of
safe, risk and occupational diseases free environment. In accomplishing this
commitment we shall ensure that in all our construction undertakings the following
prevail:

We shall, as required by legislation and Eskom,

- provide and demonstrate to Eskom a suitable, sufficiently documented and coherent site
specific health and safety plan based on their safety specifications

- open and keep on site a health and safety file and make it available on request to an
inspector, Eskom and sub-contractors

- fully comply with the provisions of CR 7(1)(c) and CR 7(1)(d - g) and that includes
ensuring that all employees on site have a valid medical certificate issued by a
registered occupational health practitioner. Refer to our SHE returnables for proof of
valid medical certificates or to our SHE File.

- in writing, appoint a full-time competent construction manager per site

- not appoint a safety officer/risk assessor that is not registered with a statutory body
approved by the Chief Inspector (emphasis on registration & qualifications)Conduct H&S
induction for all contractor prior to commence work

- Ascertain that all sub-contractor construction H&S personnel are registered with
SACPCMP

- Ensure that all our employees have a medical certificate of fitness verified by

10. LEGAL AGREEMENT
Upon Eskom appointing us as per CR 5(1) (k), though not compulsory, our organization
shall, in terms of section 37(2) of the OHS Act, sign “Agreement with Mandatory”prior to
commencing with the construction work on site. This agreement is generally viewed by
some as narrative by a client to indemnify itself in terms of liability should there be any
incidents on site. Refer to our SHE File to view the template.

11. NOTIFICATION OF CONSTRUCTION WORK


CR 4(1) states thus:
A contractor who intends to carry out any construction work other than work
contemplated in regulation 3(1), shall at least 7 days before that work is to be carried
out notifythe provincial director in writing in a form similar to Annexure 2 if the intended
construction work will-
(a) include excavation work;
(b) include working at a height where there is risk of falling;
(c) include the demolition of a structure; or
(d) include the use of explosives to perform construction work.

The fact that will run for 5 years, Shomo Ka Matla Trading Enterprise shall, at least 7 days
before this work is to be carried out, notify the provincial director of the DoL in writing.
This shall be done through submitting Annexure 2A form to the nearest DoL office and
the completed form will then be signed and stamped by the receiving official on behalf
of the Provincial Director. Refer to our SHE fileon Annexure 2A (Notification of
Construction Work).

12. DESIGN AND SPECIFICATIONS


We shall adhere to all applicable Eskom Standards, Specifications and latest designs

13. WORK PERMIT


Shomo Ka Matla Trading Enterprise and its Sub-contractor’s shall adhere to the
approved Eskom permit-to-work system to control identified high-risk activities. There
will be only one permit-to-work system (Eskom) on the construction site.If the type of
work requires that contractors shall be trained, competence-assessed, and authorised in
writing to perform the duties of an authorised or responsible person as contemplated in
the applicable. Eskom regulations, for example:

- Operating Regulations for High-voltage Systems;


- Plant Safety Regulations;
- Hot work;
- Radiation; and
- Confined space work.

Eskom representative is to provide more details on the permit-to-work system for the
specific work to be conducted by Shomo Ka Matla Trading Enterprise. Note: No
construction work shall commence or carried out without an authorized Construction
Work Permit as required by CR regulation 2(7).

Our organization, shall ensure that all site-related legal/statutory appointments are in
place and that they are site specific and indicate for which areas within the OHS Act and
regulations individuals are responsible.

We shall certainly ensure that all our appointees are made aware of their accountabilities
and responsibilities in terms of their appointment, they understand them before
accepting such appointments in writing so to assist them in the execution of their duties.

16. TRAINING

Our belief in doing it right the first time enables us to recognize the first things first kind
of a philosophy, i.e. starting with the basics in whatever we do. Training forms part of our
basic preventive measures to combat hazards and associated risks on site. The first line
of defence will thus be Induction for all employees. Training shall be divided into:

16.1. General Training


We shall ensure that all our employees and Sub-contractor’s employees working on the
site are adequately trained in the type of work/tasks to be performed. The training shall
extend to include relevant procedures, hazard identification, incident/accident
investigation and risk assessment. Such employees shall be trained by accredited
training providers and shall, at the end of training, be provided with appropriate
qualifications/certificates and shall be under continuous competent supervision. Copies
of records of appropriate training and qualifications for all employees shall be kept and
maintained in our SHE file.

16.2. Site Induction


It is imperative that Shomo Ka Matla Trading Enterprise shall ensure that all our employees,
agents and Sub-contractor employees undergo the Eskom induction programme prior
to commencing any work on site, and or proof of Eskom two days induction completed
by Contractors permanent construction employees and simultaneously provide
necessary PPE’s (CR 7(6) to all our employees free of charge. Shomo Ka Matla Trading
Enterprise shall ensure that appropriate time is set aside for training (induction and other
appropriate training) of all employees. A copy of the verified certificate of fitness CR
7(8)shall be presented for permanent record at the induction centre and kept at site
offices for permanent record.All employees and visitors on site shall have proof of
induction training. Equally so, we shall ensure that all casual employees prior to any
work on site, are accorded with the same treatment as if they were permanent staff.

16.3. General construction site induction


Shomo Ka Matla Trading Enterprise shall ensure that all its employees and Sub-
contractor employees undergo general work induction with regard to the approved SHE
plan, general hazards prevalent on the construction site, construction risk assessment,
rules and regulations, and other related aspects. Job specific induction shall be
conducted to ensure that before an employee commences work on the project the
supervisor in charge with responsibility for the employee has informed the employee of
his/her scope of authority, any hazards associated with the work to be performed, as
well as the control measures to betaken. This will include man-task specifications and
the discussion of any standard task procedures or hazardous operational procedures to
be performed by the employee. Shomo Ka Matla Trading Enterprise shall ensure that
the supervisor has satisfied himself/herself that the employee understands the hazards
associated with any work to be performed by conducting task/job observations.

16.4. Visitors and Suppliers to the site


Visitors to the site shall be required to undergo and conform to Eskom site-specific
safety induction requirements prior to being allowed access to the site. We shall ensure
that all visitors remain in the care and custody of a person (host) who has been properly
inducted. Novisitors are permitted to undertake any construction work of any nature.

16.5. Toolbox talk

As the name implies “ toolbox talk”, our employees shall, before work, have a talk whist
they have their toolboxes with them. This meeting will held on a daily basis and shall be
as brief and concise as reasonably practicable. Subjects/topics are applicable to the
job/task at hand. Near misses, accidents, up and-coming work are to be discussed
along with suggestions and comments and risks identified during the pre-task/work risk
assessment. Toolbox talks shall form part of a continuous risk assessment.

17. COMPETENCIES
On its electrical projects, Eskom requires Shomo Ka Matla Trading Enterprise and other
contractors to ensure that adequate and competent staff is employed to manage SHE
matters on site. Shomo Ka Matla Trading Enterprise shall ensure persons appointed in
the following positions, have the following SHE related training as a minimum
requirement:

17.1. Construction Supervisor (CR 8(7) Appointee) SHE Competency:


- 3 years applicable experience in construction management;
- Accredited Construction Supervisor training course
- Risk assessment training
- Incident investigation training;
- Technical training which shall include but not limited to:

1. Line construction course – (top build rural electrical infrastructure)


2. Electrical or civil engineering and related training course (Substation and HV line
construction)3. +5 years related work experience
- Root cause analysis training (RCat);

17.2. Team Leader/Supervisor SHE Competency:

- Attended an accredited supervisory training course;


- Risk assessment training;
- Line construction course
- Root cause analysis training (RCat); and
- OH&S Act & Construction Regulations training
- Eskom PSR and ORHVS as and when required by the contract scope of works

17.3. Safety Officer (CR 8(5) Appointee) SHE Competency:


- Registered with an approved professional body as contemplated in CR regulation 8(6)
(SACPCMP)
- Risk assessment training;
- Incident investigation training;
- Root cause analysis training (RCat); and
- OH&S Act & Construction Regulations training
- Eskom PSR and ORHVS as and when required by the contract scope of works

Shomo Ka Matla Trading Enterprise shall do everything reasonably practicable in


ensuring that all aforementioned requirements are fully met and complied with.

18. SHE STAFFING


A typical Eskom minor reticulation SHE Specifications require of Shomo Ka Matla
Trading Enterpriseto submit for approval, a schedule detailing qualifications and
experience of all key employees who will be employed on the project. As per that

requirement, Shomo Ka Matla Trading Enterprise shall ensure that the following staffing
totals are employed as a minimum:

18.1. Construction Health and Safety Manager


Though not stipulated in the construction regulations, depending on the risk, Eskom
may impose the requirement of employing a Full-time Health and Safety Manager to the
project. Details of this requirement is included in the contract works information.

18.2. Construction Health and Safety Officer


Shomo Ka Matla Trading Enterprise shall, due to the project risk profile, employ the
services of a Full-time Construction Health and Safety Officer during construction of this
line. If the risk or if the Eskom Project Manager/Inspector of the DoL requires it, Shomo
Ka Matla Trading Enterprise shall appoint additional full-time Safety Officers, as
requested, to manage safety and health matters on site. Only SACPCMP safety officer
as stipulated on CR 8(6) may be appointed for the project and we shall ascertain that
he/she has necessary competencies and resources to assist us.

18.3. Environmental Officer


Shomo Ka Matla Trading Enterprise shall, according to the project risk profile, employ
the services of a Part-time or Full-time Environmental Officer during its term of contract.
If the risk or if the Eskom Project Manager/Inspector of the DoL requires it, Shomo Ka
Matla Trading Enterprise shall appoint additional Full-time Environmental Officers, as
requested, to manage environmental matters on site.

18.4. Construction Professional Registration


The legislation and the SACPCMP’s publications direct that all persons assuming
responsibility for Construction Project Management, Construction Management and
Construction Health and Safety should be registered as professionals in the appropriate
category with the SACPCMP in order to comply with legal and statutory requirements
within South Africa. Employees who perform work identified by the councils for the
professions shall register with the relevant council as a professional. Alternatively, an
employee shall perform such work under the direction, control, supervision of or in
association with a person, registered as a professional with respect to that category of
work and who shall assume responsibility for the work performed. Shomo Ka Matla
Trading Enterprise has a registered CHSO.

19. SUBCONTRACTOR/SUPPLY SELECTION AND MANAGEMENT


The requirements related to the selection and Management of Sub-Contractors are
implemented in the principal phases:

1) Pre-qualification and inclusion in the approved vendor list;


2) Qualification for special projects;
3) Management at the work site (SHE Plan);
4) Safety performance standards (Risk Assessments);
5) Details of personnel experience;
6) Details of personnel qualifications;
7) Safe Operating Procedures;
8) Emergency response procedures;
9) Personal Protective Equipment procedures.

After awarding the contract, Shomo Ka Matla Trading Enterprise will be using the
following to monitor the Sub-Contractor:

1) Pre-Job Meetings;
2) Risk Assessments;
3) Sub-Contractor communications;
4) Sub-Contractor Induction
5) Access Control;
6) Sub-Contractor’s Performance;
7) Audits;
8) Site Establishment.

20. SHE COMMUNICATION AND AWARENESS


Effective communication in our organization is essential as it develop and promote a
SHE culture. We at Shomo Ka Matla Trading Enterprise shall provide a communication
strategy outlining how we intend to disseminating health and safety issues staff where
appropriate, our subcontractors and their staff, the mediums we will employ, and how we
will measure the effectiveness of our SHE communication. Shomo Ka Matla Trading
Enterprise shall ensure that safety representatives and a committee shall be formed and
shall perform all statutory functions as per OHSA section 19(1). Every formal meeting
conducted on site shall include SHE as a standing agenda point, and minutes of these
meetings shall be available on site at all times. Attendance lists shall be kept for all the
health and safety meetings. Matters that are discussed include, but are not limited to the
following:

- Accident/safety incidents
- Accident investigations (including near misses) and close-out of recommendations
- Audit findings and close-out
- Hazardous materials/substances
- Work procedures
- Protective clothing/equipment
- Housekeeping
- Work permits
- Non-conformances
- Emergency preparedness
- Traffic control
- Medicals
- Training
- Forthcoming high-hazard activities
- General SHE issues
- Matters arising from contractors’ SHE meetings

20.1. TOOLBOX TALKS


Objective: this is a meeting which is held prior to the commencement of the day’s work
with all relevant personnel associated with the work task in attendance. The job,
relevant procedures, associated hazards, safety measures, i.e., the task risk
assessments shall be discussed. Each employee who attends the briefing shall sign the
back of that pre-job brief form. Toolbox talks shall be included in the pre-job brief
meetings. The toolbox topics will be based on SHE issues pertaining to the construction
site. The topic contents shall be in writing.

20.2. PRINCIPAL CONTRACTORS STATUTORY MEETINGS AND MINUTES


Individual Principal Contractors shall hold a statutory safety, health and environmental
meeting at least once a month. Where a Principal Contractor has sub-contractors the
sub-contractor shall be included in Shomo Ka Matla Trading Enterprise's meetings. All
appointed safety health and environmental representatives shall be a member of at
least one committee and the meeting shall be held in accordance with the Occupational
Health and Safety Act. This meeting will be attended by Shomo Ka Matla Trading
Enterprise’s Safety Manager, Principal Contractor Safety Practitioner and the Safety
Practitioners from all other Sub-contractors and where required Shomo Ka Matla
Trading Enterprise Management on site.

20.3. STATUTORY HEALTH & SAFETY COMMITTEE MEETING


Shomo Ka Matla Trading Enterprise and its Sub-contractors shall implement a statutory
health & safety committee, as per the OHS Act, section 19. Shomo Ka Matla Trading
Enterprise shall comply with the requirements as stipulatedin the Employers latest
revision of the procedure for Health &Safety Representative Committee. Committee
meetings shall form part of our communicating strategy.

20.4. SAFETY AWARENESS SCHEMES


Shomo Ka Matla Trading Enterprise shall continuously roll out safety awareness
schemes on the site. These schemes may be in the form of posters and or talks on
specific safety topics identified as pertinent to the site and safety of every employee.
This shall form part of Shomo Ka Matla Trading Enterprise Health and Safety Plan.

20.5. GENERAL WALK-DOWNS


Shomo Ka Matla Trading Enterprise’s Project and Site Management shall take part in
site walk downs, as agreed upon, to demonstrate their commitment towards
occupational health and safety matters. These site visits will be used to identify both
strengths and areas for improvement regarding SHE issues. Shomo Ka Matla Trading
Enterprise’s Project and Site Management including all levels of supervision will be
required to do Visual Field Leadership inspection (VFL’S). Proof of all VFL”S to be
submitted to the Employer’s OHS Department within the time frame agreed upon, as
part of the reportable statistics.

21. CONTRACTOR’S SITE FACILITIES

21.1. TEMPORARY FACILITIES LAYOUT PLAN


Shomo Ka Matla Trading Enterprise shall submit a detail plan of its and its Sub-
contractors site layout for acceptance by the Employer. The location for Shomo Ka
Matla Trading Enterprise’s site establishment is detailed in the Works Information
document.

21.2. DINING ROOM FACILITIES


Shomo Ka Matla Trading Enterprise shall provide and maintain adequate dining room
facilities appropriate to the workforce size and work duration, that conform with the

requirements of the OHS Act Construction Regulations, Facilities Regulations, and the
Hazardous Chemical Substances Regulations. Furthermore, Shomo Ka Matla Trading
Enterprise shall provide, to the approval of the Employer, sheltered eating areas for use
of its own, as entire well as its sub-contractors’ personnel employed on site. The
maintenance and cleaning of eating areas shall be the responsibility of Shomo Ka Matla
Trading Enterprise. All costs involved are deemed to be included in the tender price.
Eating areas shall provide adequate shelter and shall be ventilated and lighted. Tables
and backed seating shall be provided. Suitable receptacles with lids for depositing
waste shall be provided at convenient points inside and outside the eating areas.
Furthermore, Shomo Ka Matla Trading Enterprise shall ensure compliance to all
legislation.

21.3. CHANGE ROOMS


Shomo Ka Matla Trading Enterprise shall provide and maintain adequate and suitable
washing facilities appropriate to the workforce size and work duration, that conform with
the requirements of the OHS Act Construction Regulations and Facilities Regulations.

21.4. ABLUTION FACILITIES


Shomo Ka Matla Trading Enterprise shall provide and maintain adequate and suitable
sanitary facilities appropriate to the workforce size and work duration, that conform with
the requirements of the OHS Act Construction Regulations, Facilities Regulations and
the Hazardous Chemical Substances Regulations.

21.5. LAY-DOWN AND STORAGE


Shomo Ka Matla Trading Enterprise shall include in its temporary facilities plan, a
detailed plan for all lay-down areas required for storage of materials, equipment and
machinery. Shomo Ka Matla Trading Enterprise shall provide and maintain adequate
and suitable storage facilities appropriate to the scale of the project and work duration,
that conform to the requirements of the OHS Act Construction Regulations, regulation
28, and as approved by the Employer/Client.

21.6. PRINCIPAL CONTRACTOR/SUB-CONTRACTOR SITE ACCESS


Access to Shomo Ka Matla Trading Enterprise and its Sub-contractors site areas will be
the responsibility of the Principal and Sub-contractors, and that no unauthorised access
is gained to site laydown areas. Shomo Ka Matla Trading Enterprise and its Sub-
contractors are to submit a plan to the Employer/Client, indicating how it intends to
control access to its laydown areas. Shomo Ka Matla Trading Enterprise and its Sub-
contractors shall ensure that no inadvertent access is gained to any of its materials,
equipment or machinery.

21.7. EXISTING SERVICES


Shomo Ka Matla Trading Enterprise shall give prior notice in writing to the Employer of
his intention to begin excavation work in any area. The Employer will then arrange to
have the approximate location of all known buried cables and or other existing services
indicated to the Contractor and, where practical, marked on the ground before
excavation commences. All movement and removal of existing buried services will, if
necessary, be carried out by Shomo Ka Matla Trading Enterprise. Shomo Ka Matla
Trading Enterprise shall immediately inform the Employer of any existing services
uncovered during the work. Prior to any excavation work, a scan shall be done by
Shomo Ka Matla Trading Enterprise to determine the location of any hidden services
underground. Where possible, air driven shovels are to be used for any excavation

work. Shomo Ka Matla Trading Enterprise may only make use of manual labour as a
last resort.

21.8. INSTALLATION AND MAINTENANCE OF TEMPORARY CONSTRUCTION


ELECTRICAL SUPPLY, LIGHTING, AND EQUIPMENT
Shomo Ka Matla Trading Enterprise and its Sub-contractors shall ensure that all
temporary electrical supply, light and equipment are installed and used in accordance
with the OHS Act, Electrical Installation Regulations, relevant national standards, by-
laws and regulations of the OEM and supplier concerned. Attention shall be given to the
positioning of such equipment in order to minimize pollution caused by noise and
fumes. Every portable generator shall be issued with a drip tray and refueling of these
generators shall be done in such a way to prevent any spillage. Each Portable
generator shall be fitted with an earth and/or earth pen.

21.9. ACCESS CONTROL TO THE CONSTRUCTION SITE


Shomo Ka Matla Trading Enterprise in collaboration with the Employers representative
will ensure that proper access control is in place and functional at all times on and off
the construction site. All security requirements shall be highlighted at the induction
given by the Employers representative. Shomo Ka Matla Trading Enterprise and its
Sub-contractors are to strictly adhere to all security requirements on the premises as
laid down by the Employer.

22. VEHICLE AND DRIVER BEHAVIOUR SAFETY


Shomo Ka Matla Trading Enterprise and its Sub-contractors shall ensure that:

- All vehicles operated by contractors within the area shall, in all respects, comply with the
Road Traffic Ordinance and Road Traffic Act. Designated drivers shall be in possession
of a driver’s licence, valid for the class of vehicle. The driver’s licence shall be kept by
the person so authorised, and he/she shall produce such card on request.

- All drivers of construction vehicles and mobile plant are to have medical certificates of
fitness to determine physical and where applicable psychological fitness.

- The speed limit within the bounds of the construction site is 40 km/h or as indicated by
the T raffic Management Plan.

- No drivers or operators may talk on a cellphone or two-way radio while driving, unless a
hands free kit is used.

- It is the responsibility of the driver to ensure that:

- He/she and his/her passengers, at all times, wear seat belts while the vehicle is in
motion;

- He/she complies with all safety, direction, and speed signs;

- Vehicle loads are properly secured and loaded onto vehicles; and

- Vehicles are not overloaded.

- All requirements with regard to the transportation of tools/equipment/material and
persons on the back of construction vehicles shall be adhered to:

- All contractor employees shall not be transported on the back of construction vehicles.

- Tools, equipment, and material are to be secured in a separate compartment, in order to


prevent movement.

- Fixed and firmly secured seats with seat belts - adequate for the number of passengers
being transported and SABS approved.

- Construction vehicles are to be fitted with rollover bars as per SANS standards.

- The driver and all passengers are to be seated with seat belts fastened while the vehicle
isin motion. Shomo Ka Matla Trading Enterprise shall ensure that its employees and
those of its Sub-contractors do not:
- Ride on the back of elevators, cranes, or any other mobile plant equipment;

- Leave vehicles unattended with the engine running; and

- Park vehicles in unauthorised zones/areas.

- Eskom reserves the right to search any vehicle on the premises or when entering or
leaving the premises.

Over and above:


- Shomo Ka Matla Trading Enterprise shall be solely responsible for the safety and
security of any of its vehicles (including private vehicles) on the premises.

- Shomo Ka Matla Trading Enterprise shall attach identification markers on all of its
vehicles that are permitted to enter the site. A current maintenance logbook is required
for all cranes and large plant equipment and shall be available for inspection at any time.
The logbook shall be located in the cabin of the crane orplant equipment.

- Shomo Ka Matla Trading Enterprise is to ensure that visibility (for example, switching on
of lights, reflectors, barricades equipped with lights, etc.) is enhanced on all construction
vehicles and mobile plant in order to identify the location of the vehicles or plant.

- Shomo Ka Matla Trading Enterprise shall maintain its vehicles in a roadworthy condition
and have a valid licence. These vehicles shall be subject to inspection by the
client’s/agent’s representative. Vehicles that are not roadworthy will not be allowed onto
the site.

In the event we do not own the equipment, we will still be responsible for ensuring that
all conditions are complied with by all and sundry. Drivers/operators shall be responsible
for the travel-worthiness of all loads conveyed by them. Precautions shall be taken to
lash all loads properly. Loads projecting from vehicles shall be securely loaded, and in
daytime, a red flag and, during darkness, a red light or red reflective material shall be
attached to the extreme end of such projecting material. All servicing and repairs shall
be carried out by the contractor in a designated area. Note, that any incident occurring
as a result of servicing or repairs will not be considered as work related. All waste from

servicing shall be disposed of in accordance with environmental legislation. Every mobile
machine whose vision is impaired when reversing shall have a siren/hooter that sounds
when the machine is reversing. This includes trucks, cranes, loaders, etc. Operators
have great difficulty in seeing light vehicles behind their machines. Drivers of light
vehicles shall avoid stopping or parking in the vicinity of machines. At least 30 (thirty)
meters shall be left clear between such a vehicle and such a machine.

23. WORKERS’ COMPENSATION AND OR COID


Shomo Ka Matla Trading Enterprise shall submit proof of registration and a letter of good
standing with the compensation fund or with a licensed compensation insurer for his/her
company and each of his/her subcontractors. This shall remain valid for the duration of
the contract. The letter of good standing shall reflect the name of Shomo Ka Matla
Trading Enterprise and/or Sub-contractor company, Expiry date and the registration
number. The certificate will be issued without any alterations. No company may do any
work without a valid letter of good standing.

24. HIV/AIDS AWARENESS PROGRAMME


An HIV/Aids awareness programme will be implemented and maintained by us. This will
include voluntary counselling and testing (VCT) of individuals prior to initial
commencement of work at the site and HIV/AIDS awareness training and access to
ongoing support for affected individuals. Shomo Ka Matla Trading Enterprise shall
ensure that its employees and its Sub-contractor employees are aware of this
programme. Records of awareness programmes and training are to be provided during
audits and inspections as and when required by the Client.

25. PROTECTION AGAINST DEHYDRATION AND HEAT EXHAUSTION


Shomo Ka Matla Trading Enterprise shall take into consideration and mitigate
dehydration and exhaustion of employees.

26. PROTECTION FROM WET AND COLD CONDITIONS


Shomo Ka Matla Trading Enterprise shall take into consideration and mitigate inclement
and extreme weather conditions.

27. EMPLOYEE HEALTH AND WELLNESS PROGRAMME


Shomo Ka Matla Trading Enterprise shall submit details of their employee health and
wellness programme as part of their health and safety plan, which should include a
medical surveillance programme and an employee assistance programme as detailed
below.

28. MEDICAL SURVEILLANCE PROGRAMME


Shomo Ka Matla Trading Enterprise shall ensure compliance to CR regulation 7(8) and
furthermore that:

- Its employees and Sub-contractor employees have undergone pre-entry medical


examination before starting any work on site. An exit medical examination shall be done
by all employeesbefore leaving the site. This requirements shall also be applicable to
casual employees.

- The verified certificate of fitness shall be issued before commencement of work and shall
be presented at induction. If Shomo Ka Matla Trading Enterprise does not provide proof

of valid and verified certificate of fitness for its employees and Sub-contractor
employees, then access to site will be denied.

Where required by the contract works information, all medicals conducted on Shomo Ka
Matla Trading Enterprise and Sub-contractor employees, by an external service
provider, shall be subjected to a medical verification process by the Eskom OHN/OHP
on site, prior to the validity of the Certificate of Fitness being confirmed and accepted.
Only verified certificates of fitness will be deemed acceptable when presented for
inductions. The following information is required for verification by the Employers
OHN/OHP:

- Copy of ID document;

- A valid medical surveillance certificate inclusive of: physical test, eye test, audio test,
lung function test, and where required by legislation and x-ray report and a site specific
Man Job Specification. This medical information shall be presented to the Employers
OHN/OHP.

The certificate shall be renewed at permitted legal intervals but as a minimum; annually
(for employees who are not office-bound, including drivers) and once every three years
(for employees who are office-bound) (until completion of the project), at which stage an
exit medical examination shall be conducted, unless otherwise advised by the
occupational health practitioner. All employees shall be issued with the required medical
records to prove medical status at the time of exiting the construction project. Shomo Ka
Matla Trading Enterprise shall provide a documented process for managing those
employees who are issued with a conditional certificate of fitness, i.e. medical
restrictions.

In instances where sick leave is taken for a period of one week or more, Shomo Ka
Matla Trading Enterprise shall institute an arrangement that employees need to sign a
declaration indicating that they did not suffer any illness or injuries that occurred in the
period of absence that may affect their ability to work on site. Visitors and Suppliers who
are visit site for a brief period of less than one day, will not be required to provide a
medical certificate of fitness, but will need to declare any medical restrictions.

Note: Eskom will only accept medical surveillances conducted by an Occupational


Health Practitioner who holds a qualification in occupational health.

29. NOISE
All employees who are exposed to noise shall be educated and made aware of the
harmful effects of long-term exposure to noise. They shall be provided with approved
hearing protection where necessary and record shall be kept of such issues. Noise
areas shall be demarcated with the appropriate signboards. The following areas should
be considered, e.g. where compressors, jackhammers, explosive tools, riveting tools,
etc. are used. Acuity hearing tests shall be done on all employees. Records of such tests
shall be available on site.

30. HEAT
When employees are working in hot conditions, clean cool drinking water shall be
supplied with easy access to all workers. Employees shall be made aware of the
dangers of heat exhaustion /stroke and the signs and symptoms of impending heat

collapse. Frequent and extended rest periods shall be provided in accordance with the
Occupational Health and Safety Act. Heat Stress Monitoring will be done when working
in the confined space.

31. ILLUMINATION
Adequate and sufficient illumination is essential and where it is insufficient Shomo Ka
Matla Trading Enterprise shall make additions to ensure safe-working conditions at all
times. Ensure that all roof lights, light fittings and fanlights are kept clear and
unobstructed. When use is made of artificial lighting where machinery is being used, all
moving parts shall be clearly visible. Illumination provided shall be so designed to avoid
glare, reflections and stroboscopic effects. Illumination survey shall be conducted at
required intervals

32. ERMEGENCY CARE

32.1. FIRST AID STATION/EMERGENCY SERVICES


Shomo Ka Matla Trading Enterprise shall provide a first aid facility as close to the
construction area as possible. The first aid station shall be equipped with the general
first aid equipment and in addition Shomo Ka Matla Trading Enterprise shall be
responsible for the following:

- Comply with the established communication network within the project/site or facility
(including outside sources, if necessary);
- Establishing personnel accountability systems (including visitors);
- Stopping work and controlling the affected areas;
- Defining key personnel responsibilities and duties;
- Access to appropriate emergency resources and medical personnel as dictated by the
emergency;
- Providing emergency response training sessions and exercises;
- Briefing and reporting requirements; and
- Preparing contingency plans.

Adequate measures and emergency plans shall be stipulated in writing and posted at
various locations on the site to adequately inform all personnel and visitors.

33. WELFARE
The following welfare facilities shall be provided for in a clean and suitable condition,
unless agreement with the client’s/agent’s representative has been confirmed regarding
the use of existing facilities:
- Shower facilities;
- Sanitary facilities;
- Changing facilities;
- Eating areas;
- Drinking water at strategic locations on site; and
- Safe pedestrian walkways.

Water for drinking/consumption purposes shall be drawn only from taps in mess areas
and ablution blocks and at points on site marked “drinking water”.No equipment or
system shall be connected to the drinking water system without prior approval of the

client’s/agent’s representative. Shomo Ka Matla Trading Enterprise will provide its own
accommodation for the workers.

34. HOUSEKEEPING
Shomo Ka Matla Trading Enterprise and its Sub-contractor shall maintain a high
standard of housekeeping within the site and shall ensure that:

- The prompt disposal of waste materials, scrap, and rubbish is essential and shall be
conducted as per the Environmental Management Plan (EMP) and Waste Management
Procedure, and according to legislative requirements;
- Adequate care shall be taken by the contractor to ensure that storage and stacking are
correctly and safely carried out;
- Before stacking any material, the contractor, subcontractor, or their employees shall
consult the Eskom ;
- Materials/objects shall not be left unsecured in elevated areas; falling objects may cause
serious injuries/fatalities;
- Nails protruding through timber shall be bent over or removed so as not to cause injury;
- All packaging material, including boxes, pallets, crates, etc., to be removed from the
work area immediately;
- Meal rooms shall be kept in a clean and tidy manner;
- On completion of his/her work, the contractor is responsible for clearing his/her work
area of all materials, scrap, temporary buildings, and building bases to the satisfaction of
the client/agent;
- In cases where an inadequate standard of housekeeping has developed, compromising
safety and cleanliness, everyone has the responsibility to bring it to the attention of the
Eskom Project/Site Manager. The Eskom Project/Site Manager has the right to instruct
Shomo Ka Matla Trading Enterprise and its Sub-contractors to cease work until the area
has been tidied up and made safe. Neither additional costs nor extension of time to the
contract shall be allowed as a result of such a stoppage. Failure to comply will result in
site cleaning by another cleaning contractor company at the cost of Shomo Ka Matla
Trading Enterprise; and
- It shall carry out regular safety/housekeeping inspections (at least weekly) to ensure
maintenance of satisfactory standards. The inspections shall document the results of
each inspection and Shomo Ka Matla Trading Enterprise shall maintain records for
viewing.

35. SIGNAGE
The contractor(s) shall use all symbolic safety signage that conforms to the requirements
of SANS and/or applicable legislative requirements. The display of the following signs is
mandatory:

- For all contractors with site establishment: the contractor company sign shall be posted
at their site offices to reflect the name and contact details of the Site Manager,
Construction Supervisor, Health and Safety Manager/Practitioner, First-aider, Health and
Safety Representative, and Evacuation Warden.
- Furthermore, all contractors shall display a statistical board containing: The nos. of
employees, the Days worked without a Lost Time Injury, total hours worked without a
Lost Time Injury, date of last Lost Time Injury.
- Radioactive material” symbolic signs at radioactive storage areas.
- The location of every first-aid box, fire extinguisher, and emergency exit is to be clearly
indicated by means of a sign.

- At the entrance to premises where machinery is used: restricted access on “Authorised
persononly” signs on entry.
- When in use, an explosive-powered tool shall have a sign warning people of its use.
- The contractors shall provide the signage where work is conducted and where
unauthorized entry is prohibited and/or where alerting and cautioning passers-by to be
aware of potential dangers.

36. PERSONAL PROTECTIVE EQUIPMENT (PPE)


In terms of section 8 of the OHS Act, the duty of the employer is to take steps to
eliminate or mitigate (hierarchy of control measures) any hazard or potential hazard to
the safety or health of employees before resorting to PPE. Shomo Ka Matla Trading
Enterprise and its Sub-contractor employees at the construction site, including visitors,
shall use the following SANS or the relevant internationally recognised authority-
approved risk-based PPE at all times, as a minimum:
- Head protection (hard hat), with an attached chin strap;
- Steel-toe capped safety boots (No Stilettos or high-heeled shoes will be allowed on the
project);
- Eye protection. Wearing of impact safety spectacles with side shields. Prescription
glasses hall comply with the same standard, or cover impact safety spectacles shall be
worn over them;
- Long-sleeved and long pants protective clothing (with reflective strips sowed on). For all
electrical work conducted in switchgear rooms, protective overalls with a minimum Cal
value of 8 shall be worn;
- High-visibility vests; and
- Hearing protection as determined by Shomo Ka Matla Trading Enterprise Noise
assessment.

Note: Shomo Ka Matla Trading Enterprise and its Sub-contractors shall comply with the
General Safety Regulation 2 of the OHS Act, Section 23 and the Eskom Personal
Protective Equipment Standard - 240-44175132 as a minimum requirement. However, if
there are particular activities/areas/risk assessments that require a specific type of PPE,
that specific PPE requirement shall be adhered to (for example, for dusty environments -
eye goggles; for welding - welding helmet; high voltage work - category 2 overalls; etc.).
Shomo Ka Matla Trading Enterprise shall ensure that:
- Its and its Sub-contractor employees understand why personal protective equipment is
necessary and that they use it correctly.
- Strict non-compliance measures shall be administered for any employee not complying
with the use of PPE, and he/she shall be removed from the site.
- The use of safety belts (work positioning belt with a work positioning lanyard) is strictly
prohibited. Only Eskom-approved fall arrest/fall prevention equipment shall be used
when conducting work in elevated positions.
- Welders, brazers, cutters, and aiders shall wear suitable eye protection, gloves, and
apron spats, and screens shall be provided to protect onlookers and passers-by.
- Suitable impact-resistant eye protection shall always be worn for grinding, chipping, and
chasing, and screens shall be provided to protect onlookers and passers-by.
- When working with hazardous chemical substances (for example, acids or caustic
substances),suitable eye protection, gloves, and special overalls shall be worn.
- Suitable eye protection shall be worn by all persons, including visitors, to any designated
eye protection area.
- Ear protection shall be worn in any designated noise zone.

- Suitable respirators shall be provided where gas and/or dust could pose a hazard.

Securities PPE requirements


- Bullet proof vest for all security personnel on site is compulsory.
- Thermal jackets for all securities on site.

Note: No contractor employee may wear clothing that indicates any other company,
other than the company he/she is currently employed at and working for on the project.
Any employee found in contravention of this requirement, will be removed from the
project until the appropriate clothing is worn.

37. HAZARDOUS MATERIALS/CHEMICALS MANAGEMENT


Shomo Ka Matla Trading Enterprise and its Sub-contractors shall describe how
hazardous substances, as defined in the Hazardous Chemical Substances Regulations
(OHS Act), will be managed. Prior to any hazardous chemical substances (HCS) being
brought onto the site or produced on the site, Shomo Ka Matla Trading Enterprise shall
supply the Eskom Project Manager with the following:
- Material safety data sheets (MSDS) in accordance with the requirements of the OHS Act
Regulations for Hazardous Chemical Substances;
- Purpose for bringing the hazardous substance onto the site;
- Proposed arrangements for safe storage;
- Proposed methods for handling/usage;
- Proposed method of disposal; and
- Hazard communication/training plan.

This information is to be provided at least five (5) working days prior to the expected
delivery on site. The Eskom Project Manager shall approve the use of any hazardous
substance after receiving the above information. Shomo Ka Matla Trading Enterprise
shall ensure that:

- No HCS is to be brought onto the site until the Eskom Project Manager’s approval has
been received.
- All HCS containers to be clearly labelled. Containers that are not marked will not be
allowed. No HCS to be stored in food or drink containers.
- Users of an HCS shall wear/use the correct PPE as per the HCS material safety data
sheet.
- Users of an HCS shall be adequately trained in the HCS that they are handling.
- Shomo Ka Matla Trading Enterprise shall have and maintain a register with all the HCS
that they have onsite.

38. FLAMMABLE AND COMBUSTIBLE LIQUIDS


Shomo Ka Matla Trading Enterprise shall submit a proposal on how it intends to store
fuel on site, and shall have written approval from the Eskom Project Manager prior
commencing with the fuel storage. The volumes of fuel allowed to be stored will depend
on site conditions and statutory regulations. Shomo Ka Matla Trading Enterprise shall
ensure that the following is strictly adhered to:

- A maximum of 40 litres of fuel is allowed to be stored. Anything greater than 40 litres is


to bestored in a flammable/combustible liquid store.

- Adequate numbers of dry chemical fire extinguishers, each with a minimum capacity of
4.5 kg, shall be provided, installed, and maintained.
- Before a machine is refuelled, the motor shall be stopped. Refuelling shall take place at
designated safe areas, and appropriate warning signs shall be installed. Suitable drip
trays shall be used to prevent spillage at the filling nozzle. All fuel storage areas shall
comply with the following requirements:
- Storage should be well clear of buildings.
- Storage areas shall be kept free from all combustible materials.
- All danger signs shall be prominently displayed, that is:
- Flammable Liquid;
- No Smoking;
- No Naked Flames; and
- Hazchem identification.
- Adequate fire-fighting equipment shall be available.
- Diesel tanks will be installed in a bunded area; the bunded area shall be able to contain
110% oftank capacity.
- The bunded area shall be of a concrete or steel construction.
- The bunded area shall have a drain valve.
- No other material/equipment shall be stored in the bunded area. Shomo Ka Matla
Trading Enterprise and its Sub-contractors shall comply with the legal requirements for
storage of flammable and combustible liquids, as stipulated in the Construction
Regulations, regulation 25.

39. MACHINERY, TOOLS, AND EQUIPMENT


Shomo Ka Matla Trading Enterprise and its Sub-contractors shall ensure the following all
machinery, tools and equipment:

- Are identified and safe to be used and are maintained in a good condition.
- Shall be adequately guarded when driven by means of belts, gear wheels, chains, and
couplings. A machine is guarded when persons cannot gain inadvertent access to the
moving parts.
- Listed on an inventory list, which is handed to Security with a copy, kept on site.
- Are regularly inspected at least monthly or as required by legislation and risk
assessments; registers of tools shall be kept in the safety file.
- Are numbered or tagged so that it can be properly monitored and inspected.
- Shall have the necessary approved test or calibration documentation, where applicable,
prior to being brought onto the premises, and the records shall form part of the SHE
plan.
- All fuel-driven equipment shall be inspected by the Eskom SHE practitioners prior to
mobilising it on site.
- All fuel-driven equipment shall be properly maintained in accordance with the
manufacturer’s recommendations and legal requirements. Shomo Ka Matla Trading
Enterprise shall supply, at its cost, all items of plant and equipment necessary to perform
the work, unless otherwise indicated in the works information. The Employer reserves
the right to inspect items of plant or equipment brought to site by Shomo Ka Matla
Trading Enterprise and its Sub-contractors, for use on this contract. Should the
Employer find that any item is inadequate, faulty, unsafe, or in any other way unsuitable
for the safe and satisfactory execution of the work for which it is intended, the Employer
shall advise Shomo Ka Matla Trading Enterprise in writing, and Shomo Ka Matla Trading
Enterprise shall forthwith remove the item from the site and replace it with a safe and
adequate substitute. In such cases, Shomo Ka Matla Trading Enterprise shall not be

entitled to extra payments or extensions of time in respect of delay caused by the
Employers instructions. Shomo Ka Matla Trading Enterprise and its Sub-contractors will
ensure that it has all the necessary registers to record all tools and equipment. Shomo
Ka Matla Trading Enterprise and Sub-contractor employees shall:

- Be competent when operating or using machines and tools;

- Have a valid certificate; and

- Have proof of any form of task-related training.

40. WORKING AT HEIGHT


Shomo Ka Matla Trading Enterprise treat working at height very seriously as a high-risk
activity, and as such, all precautions including, but not limited to legislative requirements
shall be taken to prevent incidents while working at height. All users of height safety
equipment for working at height shall be trained, assessed, and declared competent for
the specific height safety equipment and associated structures. Shomo Ka Matla Trading
Enterprise and its Sub-contractors shall ensure that for work conducted at heights the
following requirements are met:

- A task-/job-specific fall protection plan shall be developed and approved by a competent


person for any activity where there is a risk of a fall.
- The fall protection plan and its requirements shall be integrated into the health and
safety plan.
- Adherence to the fall protection plan is mandatory.
- The fall protection plan shall be suitably amended in accordance with the risk
assessment, equipment technology, standards, and legislation. The fall protection plan
shall include a task-/job-specific risk assessment and requirements relating to the
following:

- Training programme for employees working from a fall risk position;

- Appointments and authorisations;

- The procedure addressing the inspection, testing, and maintenance of all fall protection
equipment;

- The processes for evaluation of the employees’ medical fitness necessary to work in a
fall risk position and the records thereof (medical surveillance programme);

- Equipment use and specification;

- Fall prevention, fall arrest, and fall rescue; and

- Method statements or safe work procedures/task analysis/work instruction. Furthermore,


Shomo Ka Matla Trading Enterprise shall ensure that:

- When working at height, appropriate PPE as determined by the risk assessment and
written safe work procedure/task analysis/work instruction shall be used at all times.

- The type of personal protective equipment to be used shall be appropriate to the activity
and provide adequate hand, eye, face, foot, and head protection.

- Work restraint methods shall be used before placing workers in fall arrest situations.

- “Working at height” shall be indicated on all job specifications of employees who are
expected to work at height and shall be taken into account in all medical and
psychological assessments/surveillances.

- All fall arrest protection equipment shall comply with SANS Standards and other
recognized international standards.

- All height safety equipment purchased shall conform to relevant national standards,
statutory requirements, and approved Eskom divisional-specific requirements.

- All portable ladders used on the site shall be in compliance with the OHS Act and
Regulations.

- All scaffolding used shall comply with the OHS Act and Regulations as well as SANS
10085.

41. FALL PROTECTION PLAN


CR10(1) go into details on the legal requirements of a fall protection plan. We at Shomo
Ka Matla Trading Enterprise shall ensure that a fall protection plan is compiled whenever
there is a possibility that a person can fall. A fall protection plan will be compiled to show
the responsibilities and the process to be followed to ensure that the possibility of a
person falling from an elevated position will be eliminated or mitigated. A competent
person will be appointed in writing to compile a fall protection plan. The duties of the
dedicated employer (person in charge), the supervisor and the workers will be clearly
recorded.The fall protection plan will include: -

- A proper risk assessment


- Proper training of risk assessment and the fall protection plan by a competent person
appointed in writing.
- An attendance register will include the date of training and the names and signatures of
all trainees and will be kept on site and be available for inspection at all times.
- Maintenance registers as per checklists of the fall protection equipment and fall arrestors
will be kept and inspected by a competent person appointed in writing.
- A rescue Procedure should be in place.

The supervisor supervising the relevant work will be in possession of the latest fall
protection plan. The contractor shall ensure that when working at heights:
- all openings on the floor are guarded or barricaded to safeguard any person from falling
through such openings
- no person works in an elevated position unless such work is performed safely as if
working from scaffolding or ladder- fall prevention and fall arrest equipment is suitable
and of sufficient strength for the purpose fall arrest equipment shall only be used where
it is not reasonable practicable to use fall prevention equipment
- suitable steps shall be taken to ensure that in the event of falling the fall arrest
equipment or surrounding environment does not cause injury to the person

- Working at height” shall be indicated on all job specifications of employees expected to
work at height and shall be taken into account in all medical and psychological
assessments/ surveillances.
- It is a prerequisite for workers to be medically and psychologically fit to work safely in a
fall risk position or such similar environment, and as proof thereof, those workers shall
be in possession of a medical certificate of fitness.
- Only competent persons shall be allowed to inspect, test, and maintain fall prevention
and/or fall arrest equipment.

42. EXCAVATIONS, TRENCHES, AND FLOOR OPENINGS


Shomo Ka Matla Trading Enterprise and its Sub-contractors shall comply with the
following requirements:

- Digging, excavation, or driving a peg, pile, or spike into the ground by the contractor may
not commence without written authorisation from the client’s/agent’s representative.
- Prior to commencing work on any excavation or trench, Shomo Ka Matla Trading
Enterprise and or its Subcontractors shall determine the location of all underground
installations, that is, sewer, telephone, water, fuel, electric, etc. Overhead hazards shall
be assessed and dealt with prior to commencement of work.
- Adequate precautions shall be taken by the contractor to prevent slumping of
excavations, as well as to prevent rocks and loose material falling onto workers.
- All excavations done by the contractor are to be clearly demarcated and barricaded to
prevent accidental access.
- Only solid barricading will be used in areas where a fall hazard is present. Solid
barricading and/or hole covers shall be provided around all holes or openings to prevent
any person being injured as a result of a fall. Danger tape may only be used as a pre-
warning to make the solid barricading more visible and to prevent persons from coming
close to the danger area.
- Barricading shall be placed as close as reasonably possible to the excavation.
- If an excavation or trench endangers the stability of buildings or walls, shoring, bracing,
or underpinning will be provided. Excavations and trenches that are adjacent to
backfilled excavations or trenches, or that are subject to vibrations from railroad traffic,
road traffic, blasting in open-cast mining, or the operation of machinery (for example,
shovels, cranes, trucks), shall be secured by a support system, shield system, or other
protective system (that is, sheet pile shoring, bracing).
- Where it is impracticable to provide fixed guard railing, effective removable barriers shall
be provided at all unguarded openings in guard railings or floors, and these shall be
maintained in position at all times until the hazard no longer applies.
- Warning signs and flashing warning lights at night shall be displayed in suitable positions
to warn any persons approaching the area of the location and extent of any excavation.
- No material to be within 3 m of the excavation edges.
- All excavations shall be on register and inspected daily before work commences and
after inclement weather by the contractor’s appointed competent person, declared safe,
and his/her findings noted in the said register. The Employer may review the said
register on a predetermined frequency not exceeding seven (7) days.
- While work is being performed in an excavation, there shall be a supervisor at all times.
- Every twelve meters, there shall be an escape ladder in all excavations.
- Requirements in CR regulation 13 of the OHS Act shall apply as well.
- No work shall commence in an excavation, unless the excavation has been declared
safe bythe competent person.

43. PERMIT TO WORK
Shomo Ka Matla Trading Enterprise and its Sub-contractor’s shall adhere to the
approved Eskom permit-to-work system to control identified high-risk activities. There
will be only one permit-to-work system (Eskom) on the construction site. If the type of
work requires that contractors shall be trained, competence-assessed, and authorised in
writing to perform the duties of an authorised or responsible person as contemplated in
the applicable Eskom regulations, for example:

- Operating Regulations for High-voltage Systems;


- Plant Safety Regulations;
- Hot work;
- Radiation; and
- Confined space work.

The Employers representative is to provide more details on the permit-to-work system


for the specific work to be conducted by Shomo Ka Matla Trading Enterprise. Note: No
construction work shall commence or carried out without an authorized Construction
Work Permit as require by CR regulation 2(7).

44. WORKING NEAR PUBLIC ROADS


Shomo Ka Matla Trading Enterprise shall ensure that:

- Necessary precautionary and preventive safety measures shall be taken where persons
are required to work on or near roadways. Consideration shall be given to the wearing of
high visibility vests, and protection by red cones or flags during daylight and use of red
or amber flashing lamps at night;

- Work areas shall be adequately barricaded so as to prevent unauthorised access; and

- Road traffic warning signs shall be placed well ahead of the work area.

45. RISK MANAGEMENT


Shomo Ka Matla Trading Enterprise shall ensure that:
- All hazards are identified, before and during project execution, and that appropriate
activity based risk assessments are carried out;
- The activity-based risk assessments shall be available where the work is performed and
communicated to the employees daily;
- Activity-based risk assessments shall be conducted by an appointed and competent
person ofthe contractor;
- All risk assessments shall be reviewed at least every six months, or sooner should site
conditions dictate; and
- Shomo Ka Matla Trading Enterprise shall submit all risk assessments to the Employer’s
representative for review and these documents shall form part of the contractor’s SHE
Plan/File; and
- Preliminary hazard identification shall be conducted by the contractor prior to work
beginning onsite.

45.1. SITE-SPECIFIC HEALTH AND SAFETY HAZARDS


In complying with the requirements of Regulation 5(1)(a) of the Construction
Regulations of the OHS Act, the Eskom Project Manager will outline the site-specific
health and safety hazards, within the baseline risk assessment, pertaining to the

environment and physical conditions that the contractor will be exposed to in performing
its work on site. The baseline risk assessment shall be reviewed by the Eskom Project
Manager, SHE Manager/Practitioner and Engineering Manager to make it project/site
specific. The Eskom Project Manager will make all reasonable efforts to ensure that the
information provided is complete and correct. However, Shomo Ka Matla Trading
Enterprise shall make its own assessment of the hazards and risks associated with the
work under the contract.

The site and project-specific hazards are listed in section 41 of this SHE specification in
order to make potential contractors aware of the hazards. It is, however, pointed out to
the contractor that the list may not be totally comprehensive, and it is the duty of each
Principal Contractor to ensure that all the hazards are identified, before and during the
project, and the necessary activity-based risk assessments are carried out. These risk
assessments shall form part of the SHE plan that will be passed on for scrutiny and
approval by the client’s/agent’s representative when updated. Furthermore, the Eskom
Project Manager may provide on request to Shomo Ka Matla Trading Enterprise, on
contract award, with the client’s site specific baseline risk assessment.

45.2. BASELINE RISK ASSESSMENT


Eskom has not provided us with their baseline risk assessment (as per CR) where all
necessary requirements are addressed. We have compiled ours in accordance though..
Please refer to PROC/BRA/01 for more on our baseline risk assessment.

45.3. PRE-TASK/WORK RISK ASSESSMENTS


Shomo Ka Matla Trading Enterprise shall ensure that on a daily basis and for every task
to be performed, a pre-task risk assessment is conducted, with all employees involved
in the task(s) present. The pre-task risk assessment will form the basis of the daily pre-
job brief/toolbox talks prior to the start of work. Proof of communication as well as
confirmation that it was received and understood by all will be noted on a standard form,
which will be kept at the job site during the job execution. The completed signed pre-
task risk assessment form will be filed in the contractor’s safety file.

45.4. RISK ASSESSMENT (ADDITIONAL GUIDELINES)


Activity-based risk assessments shall be conducted and approved by the contractor’s
competent person before any activity begins on site and shall be updated regularly to
ensure their relevance to changing scope and/or circumstances. The intent is zero
tolerance of unsafe acts and conditions on the construction site through the assessment
of risk of each operation executed by the contractor and the provision of the necessary
means to eliminate or minimise the risk to ensure a healthy and safe working
environment. The process involves input from the site manager, supervisor(s), safety
practitioner, and the specialist artisans for the job as well as the health and safety
representative for the workplace concerned. Additional operation-specific risk
assessments are required for certain tasks throughout the project. Guidelines for actual
steps involved in an operation-specific risk assessment are as follows:

- Each activity is listed;


- Specific hazards are identified and listed against each activity;
- The magnitude of each risk is rated as low, medium, or high;
- All known documentary and supervisory controls are listed, for instance, what safe work
procedures exist for scaffolds and ladders;

- The relevance, effectiveness, and sufficiency of these controls are assessed;- In the
event of deficient controls for the particular activity, actions to be taken will be recorded
and safe working procedures drawn up;
- Persons responsible for implementing and supervising the task are to be identified,
nominated, and duly assigned;
- Persons responsible for monitoring the task and carrying out the planned job observation
shall be nominated and
- The completed risk assessment shall be handed to the Eskom site/project manager’s
representative for comment and scrutiny.

Names of workmen who have received instruction on the work content and the
sequence of the activities listed in the risk assessment are to be recorded to obtain their
confirmation of comprehension of their roles (signature or other markings). This
instruction shall be done through an interpreter if required and recorded on the pre-job
brief (daily safe task instructions), with reference to applicable risk assessments.

46. SAFE WORK PROCEDURES AND PRACTICES


We at Shomo Ka Matla Trading Enterprise have written safe work procedures for all
activities. We write safe working procedure when:

- Designing a new job or task;


- Changing a job or task;
- Introducing new equipment or substances; and
- Reviewing a procedure when problems have been identified, for example, from near
miss incidents or an accident/incident investigation.

Our safe working procedures identify:

- The level of supervision required for the task;


- The training and qualifications required by the workers to perform the task;
- The supervisor for the task or job and the employees who will undertake the task;
- The tasks that are to be undertaken that pose risks;
- The equipment and substances that are used in these tasks;
- The control measures that have been built into these tasks;
- The personal protective equipment to be worn; and
- Actions to be undertaken to address safety issues that may arise while undertaking the
task.

47. HIGH-RISK ACTIVITIES


When Shomo Ka Matla Trading Enterprise and or its Sub-contractors are working in an
area where a high health and safety hazard exists, Shomo Ka Matla Trading Enterprise
shall:

- Ensure that permanent and adequate on-site supervision is available for the entire
duration of the work that is being conducted;
- Ensure the use of safety standbys in areas of high-risk activities and activities that fall
within the scope of the permit-to-work system; and
- Provide, erect, and maintain all the required barricading, lighting, flags, flashing lights, or
other safety control equipment to enable operations to proceed in a safe manner.
Furthermore, Shomo Ka Matla Trading Enterprise shall:

- Maintain, at all times, defined access ways, which are clear of objects or obstructions, so
as to allow for emergency vehicle entry; and
- Provide any temporary protective shielding required for protecting nearby operations
from the construction activities at its own cost.

48. EMERGENCY PREPAREDNESS


Shomo Ka Matla Trading Enterprise and its Sub-contractors shall develop a site-specific
emergency response plan. Shomo Ka Matla Trading Enterprise, together with its
Subcontractors, will develop their own emergency response plan (as a guideline) for
both site and offices and submit this plan to the Eskom Project Manager for approval. It
may be decided that one site-specific emergency response plan be used for all
contractors. Shomo Ka Matla Trading Enterprise will ensure that its employees and its
Sub-contractor employees are trained on this plan. Periodic emergency drills will be
undertaken by Eskom; however, Shomo Ka Matla Trading Enterprise shall initiate its
own emergency drills with permission from the Eskom Project Manager, and at its own
cost. This shall be recorded and provided on request.

49. EMERGENCY PLANNING AND FIRE RISK MANAGEMENT


Shomo Ka Matla Trading Enterprise shall develop a fire safety procedure for the specific
construction site prior to commencing work. The procedure shall take into consideration
the size of the site, the type of work being done (for example, cutting, welding, grinding,
etc.), combustible materials, and the Emergency Plan. It shall be developed in
accordance with the hot work permit of the Eskom Plant Safety Regulations, Eskom Fire
Risk Management requirements, and all other applicable regulations. All workers
entering and working on the construction site need to be trained in fire safety and any
duties they are required to perform. Pre-existing fire systems in buildings shall be
maintained during construction whenever possible. Any changes shall be approved by
the Employer.

49.1. Fire alarms


Fire systems shall never be removed from service in an occupied building, unless a
trained and qualified person is standing at the fire alarm panel who is capable of
reinitiating the system and who is in communication with persons performing the work
or alternate procedures are taken to ensure that all persons in the building can be
informed promptly should a fire occur, and the Fire Department, including Eskom
Security, is notified.

49.2. Fire watch


Except where the building is provided with a fire alarm system or similar equipment
acceptable to the Manager: Occupational Health and Safety; fire watch patrols with
tours at intervals of not more than one hour apart shall be provided while the fire alarm
system is not in operation.

49.3. Construction sites


Shomo Ka Matla Trading Enterprise shall ensure that the following are in place:
- Fire safety plan: prior to the commencement of construction or building alterations, a fire
safety plan shall be prepared for the construction site.
- Fire warning: a suitable means of alerting site personnel to a fire shall be provided and
shall be capable of being heard in all areas of the building.
- Provide fire beaters for the purposes of extinguishing and prevent the spread of veld
fires.

- Fire breakers around material shall be constructed to prevent exposing material to veld
fires, this shall be done in consultation with the project manager. Material prone to high
risk of fire are wooden poles, cable drums, steel pole, tower assemble etc. these
materials shall always protected from fire risk, which may result from veld fires and
arson.
- Portable extinguishers: suitable extinguishers shall be available at the construction site
and, incases of hot work, be readily available at the location.
- Combustible liquid and flammable liquid storage: storage of combustible and flammable
liquid at the construction site is not permitted unless stored in approved flammable
cabinets or outdoors away from the buildings.
- Fire watch: fire watch (with tours at intervals of not more than one hour apart) shall be
provided when a portion of a building is occupied while construction operations are
taking place, with provision for the fire watch to sound the alarm and notify the Fire
Department and Eskom Security (except where the building and construction sites are
provided with a fire alarm system or similar equipment acceptable to the Manager:
Occupational Health and Safety).
- Smoking restrictions: smoking is not permitted indoors, at entrances to buildings, or near
air intake systems as per Eskom policy and legislation requirements.

49.4. Fire protection system shutdown procedures


In the event of any shutdown of fire protection equipment or parts thereof, the manager
of maintenance and operations and the electrical foreman should be given seven days’
notice via email with confirmation of schedule within two days of the original notice.
Managers of security and occupational health and safety should be given three days’
notice via email for fire watch requests. The building occupants should be given three
days’ notice via email (all notes) of any shutdown of fire protection equipment or parts
thereof. An attempt to minimise the impact of inoperative equipment shall be made (that
is, where portions of a fire alarm system, sprinklers, and standpipe system are taken out
of service, the remaining portions will be maintained). Assistance and direction for
specific situations should be sought from the manager of maintenance and operations
and the electrical foreman and should be in accordance with the accepted Eskom fire
safety guidelines and the fire safety plan for the building. In the event of bells and pull
stations being removed from service in an occupied building, facilities management staff
(person in charge of the work) will be responsible to post procedures at affected pull
stations, elevators, and entrances. Assistance and direction for specific situations
should be sought from the manager of occupational health and safety and should be in
accordance with the accepted Eskom fire safety guidelines and the fire safety plan for
the building. Procedures to be followed in the event of shutdown of any part of a fire
protection system are as follows:

- On request, electrical staff will verify in person the work to be done, contact the
monitoring station if necessary, isolate the required fire protection system, place an
information tag on the fire alarm panel, and inform the electrical foreman that the system
has been isolated.
- The electrical foreman will notify the security supervisor to begin the fire watch.
- Security staff or another reliable person will patrol the affected area(s) at least once per
hour.
- On request, electrical staff will verify in person that work has been completed, contact
the monitoring station if necessary, restore the fire protection system, remove the
information tag, and inform the electrical foreman that the system has been restored.
The electrical foreman will notify the security supervisor to end the fire watch.

49.5. Fire Extinguishers
Shomo Ka Matla Trading Enterprise shall ensure that Fire Extinguishers are:

- Clearly labelled;
- Conspicuously numbered;- Entered into a register;
- Inspected on a monthly basis by a competent person; and
- Tested and serviced at recommended intervals by an accredited supplier, with the
results entered in the register and signed by a competent person.

50. HAZARDOUS CHEMICAL SUBSTANCES


Shomo Ka Matla Trading Enterprise shall keep a register of hazardous chemical
substances and material safety data sheets should be kept on site.

51. AUDITING
Shomo Ka Matla Trading Enterprise shall conduct monthly statutory audits of all its Sub-
contractors’ safety management systems and their work areas. The results of these
audits shall be kept on record on the site SHE files. The Employer will conduct monthly
audits on Shomo Ka Matla Trading Enterprise and may also conduct audits on all its
Sub-contractors. Shomo Ka Matla Trading Enterprise shall conduct daily and weekly
inspections of Sub-contractors’ activities and follow up that corrective action has been
implemented on findings. Shomo Ka Matla Trading Enterprise shall produce a monthly
report on all its internal audit findings with an accompanying corrective action plan to the
Employer for review. The Employer and Shomo Ka Matla Trading Enterprise’s Senior
Site Management shall also take part in site walks/inspections to demonstrate to the
workforce management commitment to occupational health and safety to the workforce.

52. SHE INCIDENT MANAGEMENT


We have an incident reporting system that is compatible with Eskom’s requirements and
all applicable legislation. Any incident or near miss involving all and sundry including
property, plant, or equipment shall be verbally/telephonically reported to the Eskom
representative within 60 min of occurrence. We will:

- Ensure compliance with Eskom incident management processes for reporting and
investigation of incidents, as stipulated in the latest revision of the Procedure Manual for
Environmental, Occupational Health and Safety Incident Management 32-95
- Ensure that all incidents are documented on the appropriate flash report template, as
provided for by the Employer, and submitted to the Eskom SHE Department before the
end of the shift.
- Keep on site/at the workplace a record of all accidents and incidents reported in the form
of the OHS Act Annexure 1 investigation form as referenced in the OHS Act (Incident
Investigation Report).
- Provide SHE-related statistics to the client at the end of each month.
- In addition to the reporting of incidents as required in terms of the Occupational Health
and Safety Act and Compensation for Occupational Injuries and Diseases Act and
Eskom Incident Management Procedure 32-95, each accident (first aid, medical
treatment, and disabling and/or fatal injury) shall be reported to Eskom Project
Manager, the Occupational Health Nurse, Eskom Safety Officer, Contractor SHE Rep or
Supervisor and the Safety Officer for contractors. Reporting of injuries shall be done
immediately or before the end of the shift.

- In incidents such as fire and damage t o p r op e r t y a n d o r a s se t s also report to
Eskom ’s Safety Department (Fire/Safety Department).
- Be in possession of an Accident Register as required by the Occupational Health and
Safety Act and shall acquaint himself with the contents of Sections 24 of the OHS Act
(Reportable Incidents). All reportable incidents shall be reported to Department of
Labour within 24hours.
- Investigate incidents within 7 days of occurrence.. Recommendations outlined during the
investigation shall be implemented and proof of implementation be submitted to Safety
Department for the incident to be closed out on “SAP OH&S Incident Management”.

53. MONTHLY/WEEKLY STATISTICAL REPORTS


Shomo Ka Matla Trading Enterprise shall submit on a monthly basis, on the last day of
the month, a statistical performance report on the Employers template for weekly and
month-end reports.

54. MANAGEMENT OF CHANGE


At Shomo Ka Matla Trading Enterprise we are recording and controlling our documents
in accordance to ISO 9001: 2008. All new documentation will be attached to a document
change proposal and circulated for review. The document will be logged onto the
document register once all changes have been made and the document accepted. It is
the duty of the IMS personnel to ensure that the applicable register is updated.

55. SHE FILE


We shall open a SHE file in which records of this specification and the SHE plan are
kept. All information required in the specification and plan, for the duration of Shomo Ka
Matla Trading Enterprise and subcontractor’s contract, will be recorded in the file. The
SHE file shall be maintained on the construction site. We shall also record in the SHE
file:

- Information about removal or dismantling of installed plant and equipment;


- Hands-on information about equipment needing cleaning and maintenance, for future
purposes;
- Nature, location, and markings of services; and
- As-built drawings.

The file shall be kept on site and shall be available on request for audit and inspection
purposes. At the end of the contract, the SHE file shall be handed over to the
client/agent.

56. HOURS OF WORK


All work conducted on site shall fall within the legal requirements in accordance with the
Basic Conditions of Employment Act. Shomo Ka Matla Trading Enterprise shall notify the
Eskom Supervisor(s) of any work that needs to be performed after hours according to
the agreed arrangements. (The notification needs to be submitted timeously).
Where applicable, the notification should include proof of application for extended
overtime to the Department of Labour and/or the letter of approval from the Department
of Labour.

57. ESKOM LIFE-SAVING RULES


Shomo Ka Matla Trading Enterprise shall comply with the requirements as stipulated in
most recent version of The Implementation of the Eskom Life-saving Rules 39-47

document. Eskom has identified five Lifesaving rules. Failure to adhere to these rules is
considered by Eskom a serious transgression. These rules have been implemented to
prevent serious injury to or death of any employee, labour broker or contractor working
in any area on Eskom premises. As always, Eskom takes a stance of zero tolerance on
transgression of these rules. Non-compliance to any one of the lifesaving rules will be
considered serious misconduct and will lead to disciplinary action, which may include
dismissal. These 5 Life-saving rules are as follows:
- Rule 1 - Open, isolate, test, earth, bond, and/or insulate before touch
- Rule 2 - Hook up at height
- Rule 3 - Buckle up
- Rule 4 - Be sober
- Rule 5 - Ensure that you have a permit to work

These Life-saving Rules are non-negotiable health and safety rules that shall not be
broken under any circumstances. Where additional Life-saving Rules have been
implemented as part of a site specific requirement, Shomo Ka Matla Trading Enterprise
and its Sub-contractor will comply accordingly.

We commit ourselves to adhere to these rules at all times.

Consultant representing Shomo Ka Matla Trading Enterprise, hereby confirm that the
content of this OHS Plan is been developed by myself and is in compliance with the
provisions of CR7(1)(a), Eskom Specifications and OHSA. Any deviations and/or omissions
that may be divulged shall be promptly addressed in consultation with the client.
