Obstetrical and Gynecological Society (POGS) Midyear Convention 2023 fully knowing that attending this event,
is optional.

By signing this waiver, I acknowledge and assume all the risks associated with attending the POGS
Midyear Convention 2023 and agree that Mindanao State University - College Of Medicine, Main Campus, along
with its faculty and staff, assume no liability whatsoever for personal injuries or property damage that may arise

out of my participation in the aforementioned event.
