Fill-out Enrollment Form from the Registrar’s office
Submit the Enrollment Form to the Registrar’s office

1 Photocopy of Birth Certificate(NSO/PSA) Registration Promo
For Grades 1 to 12

Proceed to the Cashier for the Schedule of Fees and
Payment
Get Class Admission Slip at the Registrar’s Office
