DR’s was not located immediately and not transmiited to accountable person

December 14, 2023

On December 13, 2023, BITS Hardware issued a demand letter claiming for the
uncollected payment P 94,250.00. It was the time of Liberty Pasion when the said
uncollected payment happen, late transmittal due to missing DR and it was only located
last December 11, 2023. PO-PREL-PCNAGULIAN dated December 20, 2022 with
attachment DR#35984/36374/35982/36376 amounting to P59,500.00 was transmitted
December 11, 2023, but unfortunately send back to procurement for Incident Report.
Upon checking on the DR date and PO date, the date did not match it means that
delivery comes first before processing PO which is under violation of our cooperative
existing policies and procedures.

Recommended Action:

Recommend to make a proper action to the involved personnels in accordance with the existing
policies and procedures.

 Notice to Explain

I apologize what happen, it so happen that I did not notice this PO, I only knew that
this was not transmited when the supplier follow-ups for payment. I only locate the
said DR’s lately when I’m checking the folders of mam Liberty.

For this, I’m willing to undergo what will be disciplinary action made by the
management.
