The undersigned are Humanities and Social Sciences Students who are currently taking
Practical Research II subject as part of the Senior High School Curriculum. They are
currently pursuing their research study entitled “Level of Implementation, School
Practices, and Problems Encountered on Disaster Risk Reduction Management
(DRRM) by the Secondary Schools in Lucena City: Basis for Action Plan”.

In view of this, they would like to request your permission in conducting the investigation
through remote platform by using Facebook Messenger, Google Forms, and Phone Interview
to observe safety measures among teachers and learners in Quezon National High School.

The undersigned believe that the study will be beneficial for the school since it will give a
clear view and grasp of the Disaster Risk Reduction Management of the secondary schools.
Also, it will improve the DRRM that the respondents currently have.
