Incident Description:

At approximately 9:50 am, Mr. Francis Tallo noticed that the Automatic Transfer Switch did
not automatically switch the power from the GENSET to SAMELCO power, even though the supply
from SAMELCO was now normal. He had to manually transfer the power from GENSET to
SAMELCO, resulting in electricity fluctuations.

Action Taken After an Incident:

We conducted an inspection of the Automatic Transfer switch together with Sir Leo and Mr.
Tallo. Upon inspection, we found that the controls of the ATS have two burned relays and one
detached wire. Additionally, we noticed that the display of the ATS is no longer functioning.

Recommendation/Suggestion:

1. Contact the supplier of the Automatic Transfer Switch. (done)


2. Purchase spare relays.
3. Schedule an Annual PMS for the Automatic Transfer Switch with the supplier or a third-party
contractor.
4. Temporarily switch the ATS to manual operation mode until the PMS has been conducted.
