BRIEF DESCRIPTION

On Tuesday 21 March 2023 at 09:42 WIT, a work accident occurred involving Marthen
Korwa, an employee of the electrical department of PT Redpath Indonesia working at
the Metal Picker area, Haulage Level 2525 DMLZ UG Mine. Marthen Korwa, together
with his co-workers Budi Prihandoko and Juanda, were assigned to install OKONITE
4/C #14 AWG supply electrical cables for rotary lights with the use of Telehandler
(TL943D) 730-#558 equipped with man basket that was operated by Pak Juanda.
When Marthen Korwa was securing the wire cable to the protruding section of cable
bolts with the use of cable zipties the operator elevated the man basket resulting in
the worker’s finger being pinched between the crash bar and the protruding cable bolt
at the back of the drift.
As a result of this accident, Marthen Korwa suffered an open fracture to the tip ring
finger of his left hand. This incident is classified as a serious lost time accident (LT 60
days).

EVENT
1 Mr. Marthen Korwa gave a signal to the operator
Mr. Juanda to raise the man basket while Mr. Budi
Prihandoko was still outside the man basket securing
the cable at West Empty 02.

2 Mr. Juanda elevated the man basket to 8 meters


(the distance between the work floor and the top of the
crash bar). Mr. Budi Prihandoko returned to the
telehandler's work area, he observed Mr. Marthen
Korwa working in the man basket.

• 3 Mr. Juanda continued elevating man basket until


reached 8.3 meters (the distance between the work
floor and the top of the crash bar), without receiving
any signal from Mr. Marthen Korwa.
At this elevation Mr. Marthen Korwa was able to
reach the protruding cable bolt to which he secured
power supply cable to the cable bolt using Zip Ties. 3
EVENT
4 Mr. Marthen Korwa started securing power supply
cable to the protruding cable bolt. He held the power
supply cable against cable bolt with his left hand while
his right hand fastened the two together using Zip ties

• 5 When Mr. Juanda continued to raise the man


basket, Mr. Marthen Korwa’s left ring finger was
pinched between the cable bolt and the crash bar. Mr.
Marthen Korwa shouted out while pulling his hand
away quickly leaving his glove trapped and resulting in
an injury to the left tip ring finger.

IMMEDIATE CAUSES
UNSAFE ACT
 Juanda decided to raise man basket without any clear instructions from
Marthen Korwa.
 Marthen Korwa decided to work alone in the man basket where he deviated
from original work instruction given in the line up.
 Marthen Korwa failed to recognize the need of maintaining clear and specific
means of communication between himself and the operator which was only
one initial signal to raise the man basket without giving a signal to stop.
 Budi Prihandoko failed to recognize the deviation and apply stop and correct
measure.
 Both hands of Marthen Korwa were exposed to a pinch point between the
crash bar and cable bolt.

UNSAFE CONDITION
 The configuration of the crash bar exposes workers to a pinch point between
the crash bar and other objects (line of fire).

BASIC CAUSES
PERSONAL FACTOR
 Juanda chose to accept the risk of raising the man basket without receiving
clear instructions from Marthen Korwa.
 Marthen Korwa, Budi Prihandoko and Juanda had not received initial HIRA
training.

JOB FACTOR
 Procedure of Installation and Connection of Facilities in Underground Mine – Air,
Water or Power Cables No. UM.1.00-EL-7.05.08/13 has not yet been reviewed.
 Procedure of Using Work Platform No. SOP-6.01-UG-N01 has not identified the
risk with the addition of a crash bar to the man basket and has not included a
standardized signal between the workers in the man basket and the operator

MANAGEMENT CONTROL

1. The Change Management identification system for man baskets equipped


with crash bars has not been implemented.

2. Procedures related to the work of laying power supply cables in underground


mines have not been regularly reviewed.

MITIGATION

1. To conduct assessment of the crash bar configuration and continue with adequate
management of change for man baskets equipped with crash bars process.
2. Register Marthen Korwa, Budi Prihandoko and Juanda to complete HIRA training.
3. Revise Procedures of Installation and Connection of Facilities in the Underground
Mine – Air, Water or Power Cables No. UM.1.00-EL-7.05.08/13 to include:
a. Risks of pinch points when installing cables in underground mines using a
man basket and determining controls.
b. The number of workers needed perform the task utilizing the man basket refer
to the type of cable be installed.
4. Conduct socialization of Procedures for Installation and Connection of
Facilities in the Underground Mine – Air, Water or Power Cables No. UM.1.00-
EL-7.05.08/13 which has been revised to PTRI Electrical employees.

5. Conduct PJO refer to Procedure of Installation and Connection of Facilities in the


Underground Mine – Air, Water or Power Cables No. UM.1.00-EL-7.05.08/13
which has been revised to PTRI Electrical employee.
6. Revise Procedures Using Work Platform No. SOP-6.01-UG-N01 to include:
a. Identify risks with the addition of a crash bar to the man basket.
b. Standardization of signals between workers in the man basket and the
operator.
7. Socialize Procedures Using Work Platform No. SOP-6.01-UG-N01 which has been
revised to all PTRI DMLZ Electrical workers.
8. Conduct PJO refer to Procedure Using Work Platform No. SOP-6.01-UG-N01
which has been revised to PTRI Electrical workers.
9. Review the HIRADC of PTRI Electrical Department regarding the risks of being
pinched by crash bars on man baskets.

2. Proper hand signals are essential for a group working with use of mobile
equipment. The spotter training then mandated for PTRI UG employees.
Currently about 65% of PTRI manpower had attended Spotter training.
3. Need to review wider aspects of the manbasket used in UG.
 Door (open in or out, issue when cable in the basket
 Coloring (red: no-hand zone, green: handrailing, proper anchor points)
 Screen to prevent man climbing the manbasket, etc.
