1 Magnetic Separator is working without Tramp Chute
2 Coal Spillage while dumper unloading at SMCP (West)
from a height of 60 ft

1 Power Factor in West Substation has gone down to 0.45.
Capacitor Bank not connected in circuit
Letter no 991 dated 16.01.2023
1 Unsafe working condition resulting in electrocution of
workers during erection of lighting pole below 33KV HT OH
line.

Safety procedures not adhered to during working.


Job planned was not intimated to NCL officials
2 LOTO system of shutdown was not followed. In earlier
cases of diversion of OH lines inside the mine, LOTO
system was strictly followed by NCL.
3 Action should be initiated against concerned supervisor of
the contractor concerned and report should be submitted
to NCL so that
4 SKSL should inform NCL and all concerned their daily job
program one day ahead or in the morning. NCL warned
SKSL not to take up any sensitive job, specially where
safety is involved, without the knowledge of NCL
Letter no 999 dated 19.01.2023
1 It was observed that JCB No. MP 66D 0832 was carrying
manpower in mine during morning time. This is a
dangerous practice and will also spoil the safety culture.
2 At SMCP and Silo, manpower deployed by SKSL for
cleaning was found without safety gadgets. In spite of
repeated reminders by NCL, situation has not improved.
3 Some of the LVs and HVs deployed in the mines area for
various purpose in relation to construction work in Eastern
& Central Lines do not have valid gate pass
4 NCL had advised SKSL to inform them the daily job
program in the morning. It is regretted that the daily job
program is not shared by SKSL with NCL till date.

Letter no 1055 dated 03.02.2023


1 Shutdown book to be filled by competent person before
taking up any maintenance job at CHP (either mechanical
or electrical). This book/record will be maintained by
Control Room Operator (CRO)

2 Manpower distribution chart should be prepared &
maintained by CRO at beginning of every shift so that
anybody can easily figure out the duty of employee with
locations
3 It has been observed that during breakdown of any belt,
sizer or apron feeder, rest of the plant keep running
without reason which cause lots of power loss. It is
recommended to stop all unnecessary conveyors if
running idle
4 Electrical shutdown should be taken with proper permit
taken by competent person with LOTO system compilation
and having detail of person with photo along with danger
board on panel. For LOTO system which is the practice of
Jayant mines and CHP, the details may be obtained from
PE (E&M) and CHP In-charge
Letter no 1056 dated 03.02.2023
1 Employees who are deputed in O&M of western section
should comply the following statutory requirements:
a. Attendance in Form A register – mandatory
requirement
b. VTC trained
c. IME complied
d. Should maintain data on CLIP portal on monthly basis
in coordination with Personnel Deptt. For payment of
wages
e. Should wear PPE kit on duty
