Job planned was not intimated to NCL officials 2 LOTO system of shutdown was not followed. In earlier cases of diversion of OH lines inside the mine, LOTO system was strictly followed by NCL. 3 Action should be initiated against concerned supervisor of the contractor concerned and report should be submitted to NCL so that 4 SKSL should inform NCL and all concerned their daily job program one day ahead or in the morning.
