Review of Previous Site Safety Observation

1.Manpower entry at security need to be done as per DPR report. There is less manpower
reporting at security compared to the reported manpower. - All

2. material stacking and housekeeping need to be done near the site office. -DTL
3. Bare power cable used on DB to be rectified with an industrial socket. -DTL
4. An Improper 3- phase cable joint was found at B1 shed for mastic boiler and need to be rectified using a jointer
kit. -CPBPL
5. Mastic waste and gunny bags are lying haphazardly at B1 and B2 floors need to be removed -CPBPL
6.Wild grass around bath pit near the labour hutment need to be cleaned. -CPBPL
7.No access to B1 shed from west side due to drain excavation. alternate access needs to be provided. -DESPL
8. Power Cable laying Infront of canteen to be routed properly. -DESPL
9.All open cable trenches that have been completed need to be hard barricaded and surplus soil kept on the
banks should be removed -ABB
10.Empty cable drums are found in many locations at site need to be removed or kept at designated place. -ABB
11.Chequered plates are open in many places in PMCC cable cellar need to be kept properly placed to cover
openings, and cable management should be done properly -CPBPL
12.No (induction), supervision for new commers and Civil workmen were found without PPE at MSV area-GPIPL
13.Deployment of safety personnel for supervision of upcoming major activities. -GPIPL

New Agenda Points

1.All (Workmen and Supervisory staff) are requested please ensure PPEs (Safety Helmet and
Shoes) inside plant premises. -Info to all.

2. Three Phase power cable laid in ground to be removed or make it over head for excavation clearance. -CPBPL
3. Unwanted warping material, scraps found at site. Housekeeping to be done at B2, canteens and DG shed area. -

4. Scaffolding found without railing and stability people are working on it without wearing safety harness. -DTL
5. Bare cable and AG 4 grinding machine used for mechanical works at TLD area. -DTL
6.People are found without helmet below ladder lift arrangement provide at MSv for sand filling. -GPIPL
7.Unwanted scrap /materials and DB found near wall of MSV to be removed. -GPIPL
8. Power Cable laying Infront of canteen to be routed properly. -DESPL
9.Matalic scrap and wooden scrap dumped near island area to be removed or kept in a proper manner. -DESPL
10.Empty cable drums are stacked at STP are to be removed to designated location. -ABB
11.Damaged Earth strip/surplus soil found back side of admin building to be removed. -ABB
12.No (induction), supervision for new commers and Civil workmen were found without PPE at MSV area-GPIPL
13.Proper Material stacking and Housekeeping to be maintained in side plant building. - RAPL
