ENVIRONMENT ASPECTS PROCEDURE


1.0 PURPOSE AND SCOPE
The purpose of this procedure is to provide a system and instructions to identify
environmental aspects of ABE’s activities, products and services in order to determine
those which may have a significant impact on the environment.
This procedure covers all activities, products and services associated with ABE.

2.0 DEFINITIONS AND ABBREVIATIONS


ABE : Albanna Engineering
EMS : Environmental Management System
EO : Environmental Officer
MR : Management Representative
PO : Process Owner
CEO : Chief Executive Officer
Environmental Aspect : Element of ABE’s activities, products or services
that can interact with the environment
Environmental Impact : Any change to the environment, whether adverse
or beneficial, wholly or partially resulting from
ABE’s activities, products, or services

3.0 RESPONSIBILITY
3.1 The Chief Executive Officer is responsible for implementation of EMS within the
company.
3.2 Environmental Officer, with the assistance of department heads is responsible
for identifying ABE’s activities, products and services which may have
significant impact on environment.

4.0 PROCEDURE
4.1 Identification of Environmental Aspects
4.1.1 Environmental Officer will identify environmental aspects of operations
and activities that are controlled or influenced. The aspects categories
include but not limited to
 Air Emissions
 Liquid and Solid waste
 Noise
 Natural Environment
 Waste water discharges
 Energy usage
 Water usage
 Material usage
 Land condition

4.2 Identification of Significant Environmental Aspects


Environmental Officer will review and evaluate the aspects identified and
determine the significance using the criteria given below in sections 4.4 and 4.5
4.3 A register will be maintained in the respective department listing all aspects/
impacts and their significance.
4.4 An environmental aspect may be considered as “significant” where the aspect:
4.4.1 is subject to relevant legislation, regulation, permit requirements and/or
other requirements.
4.4.2 is subject to potential accidental release of solid, liquid or gas that is of
sufficient quantity to cause environmental concern.
4.4.3 pertains to energy usage
4.4.4 is subject to high environmental loading due to one or more of following
criteria.
 toxicity
 amount of solid, liquid and gas release
 amount of usage of natural resources
 frequency of episodes
 severity of actual or potential impacts
4.5 Significance of each aspect identified in the Environmental Aspects Register
will be calculated using the following formula.

EMERGENCY PREPAREDNESS AND RESPONSE PROCEDURE

1.0 PURPOSE AND SCOPE


The purpose of this procedure is to describe appropriate response to identified
environmental emergencies, which can have a significant impact on environment.
The scope of this procedure covers all activities, products and services of ABE.

2.0 DEFINITIONS AND ABBREVIATIONS

3.0 RESPONSIBILITY
3.1 CEO is responsible for ensuring that an emergency preparedness program is
implemented and for approving the emergency response plans (ERP).
3.2 Environmental Officer is responsible for development and implementation of
ERP.
3.3 Department heads are responsible for identifying potential environmental
emergencies relevant to their areas of operation.
3.4 Operations Manager and Project Manager are responsible for dealing with
immediate threats to ABE employees, the public and to the environment.
3.5 Project Engineers and Supervisors are responsible for taking immediate
actions on site based on the instructions given by their respective Project
managers and/or Operations Manager.
3.6 HR Officer is responsible for ensuring employee welfare during a potential
emergency situation.

4.0 PROCEDURE
4.1 Environmental Officer will initiate the process to develop emergency
preparedness and response plans (ERP) based on the identified potential
emergency situations. These emergency situations will be identified by
department heads, based on the data input from Project Engineers and
Supervisors. As a minimum, the following will be addressed in the ERP.
4.1.1 Emergency Response personnel and their roles and responsibilities.
4.1.2 Evacuation routes and response procedures.

4.2 After review and approval of the plans, they will be tested as conditions allow.
Drills will be conducted to test the effectiveness of the plan and to maintain
the employee’s awareness level.
4.3 Environmental Officer along with Project Engineers/ Site Supervisors/
Workshop
Incharge will ensure emergency preparedness training for employees.
4.4 Environmental officer will prepare an impact minimizing strategy based on the
emergency response plans. This may include special training for employees,
additional containment around chemicals and hazardous waste (eg: storage
of paint and thinner), additional fire fighting equipment to deal with a fire
scenario (eg: welding and gas cutting in a live oil terminal).
4.5 As resources and facilities change, preparedness and response plans will be
updated by the Environmental Officer.
4.6 The Environmental Officer and Safety Officer will ensure environmental
protection supplies such as spill clean up material, sand bags for containment
of spills, personal protective equipment etc.
4.7 Project Engineers and Supervisors will ensure that MSDS of all dangerous
goods are made available on site. All dangerous goods will be stored in a
location which is available for use by emergency services during a potential
emergency situation. Information regarding all dangerous goods, their
quantity and storage location will be maintained on site.
4.8 Emergency contact telephone numbers will be displayed on sites and
workshop notice boards. This will include contact numbers of the CEO,
Operations Manager, Project Manager, Civil Defense, Hospital, Police, etc.
4.9 After an emergency situation is dealt with, an Incident Report and a
Corrective Action Plan will be prepared by Environmental Officer and Safety
Officer.
4.10 Effectiveness of ERP will be evaluated following an emergency or drill
incident.

LEGAL AND OTHER REQUIREMENTS PROCEDURE


1.0 PURPOSE AND SCOPE
The purpose of this procedure is to define the mechanism for identifying and maintaining
current legal and other requirements and regulations applicable to ABE operations and
activities, and for maintaining access to up-to-date editions of those requirements.
The scope of this procedure is to inform employees about the availability of all legal and
other requirements that ABE subscribes.

2.0 DEFINITIONS AND ABBREVIATIONS

3.0 RESPONSIBILITY
Environmental Officer is responsible for subscribing and maintaining legal and other
requirements in co-ordination with Process Owner.
PO will raise purchase requisition for procurement of these documents. Purchase
department is responsible for purchase of the documents.

4.0 PROCEDURE
4.1 EO will maintain up-to-date listing of applicable environmental legal and other
requirements through the maintenance, access and review of the relevant
references such as:
4.1.1 Local Legislation
 UAE Federal Law
 Dubai Municipality Local Order
 Dubai Municipality Technical Guidelines
 Dubai Municipality News Bulletins
4.1.2 International Standards
 ISO 14001 Standards
 OHSAS 18001 Standards
4.1.3 Other Requirements
 Customer Interest
 Interest of local residents
4.2 EO may undertake additional activities as appropriate to ensure that all applicable
legal and other requirements are available. These additional activities include
downloading documents from the ‘world wide web’ etc.

4.3 Legal requirements with respect to the test records shall be maintained and ensure
that the sample test is being conducted as per the local authority requirement.

4.4 The access to, or copies of applicable legal and other requirements will be readily
available for reference in ABE library.
4.5 PO & EO will communicate legal and other requirements to all areas and
departments. A matrix shall be prepared by the EO, with details of DM Regulations
and its relevance against ABE activities.

5.0 RECORDS
5.1 Following records will be generated and maintained up-to-date.
 List of applicable local legislations (with revision status).
 Environmental Standards & Allowable Pollutants Limits
 Test reports of Waste disposal samples
 List of approved waste disposal transporter.

HSE OPERATIONAL CONTROL PROCEDURE

1.0 PURPOSE & SCOPE:


This procedure describes the process involved in the hazard impact prevention, and
control of operations and activities that are associate with OH&S risk and imapcts.
The following HSE operational Procedure is intended to focus on our goals,
responsibilities, hazard & impact analysis, injury prevention, and control. It is
applicable to all Albania Engineering employees, subcontractors and subcontractors’
employees working on site.

2.0 DEFINITIONS AND ABBREVIATIONS:


ABE: Albanna Engineering
CEO: Chief Executive Officer
PSP: Project Safety Plan

3.0 RESPONSIBILITIES:
3.1 CEO: The GM is responsible for providing the support necessary for the
successful implementation of this procedure.
3.2 Managers: All managers are responsible for the prevention of injuries in their
respective organizational units and promote safety awareness in their own
Departments / sites
3.3 Project Manager: Project Manager has the responsibility for the prevention of
accidents to their personnel, impacts to the natural resources and has the
ultimate responsibility for HSE issues of the project. Project Management is
responsible for assigning responsible persons to ensure that Safety
Management is complied with.
3.4 Safety Officers: The safety officer is responsible for monitoring the
implementation of this procedure and including toolbox talks and induction
training. He will ensure that all personnel adhere to published
procedures/instruction. He is further responsible for advising project
management of any identified non-conformance.

3.5 Supervisors: Supervisors have key responsibility in the planning and co-
ordinating of work activities and informing their employees of potential safety
hazards on the job and instructing them in safe methods to accomplish the
work.
3.6 Employees / Subcontractors: Employees / Subcontractors are responsible for
their own safety and for the safety of fellow employees working with them.
Employees / Subcontractors are required to be active in safety on the job by
reporting unsafe conditions or practices to supervision and to take the time to
become aware of the hazards on the job and use safe work methods for
protecting themselves.

4 PROCEDURE:

4.1 Hazard Prevention and control


The project manager will prepare the project safety plan (PSP) and he will
specify safety procedures that employees/ subcontractors are required to
follow.

Personal protective equipments such as hard hats, and safety footwear are
required in all the projects. Additional safety equipment may be required such
as a harness, safety goggles, hear protection and respirator & etc for specific
hazards.
Administrative controls may be used to protect employees from hazardous
situations as such by using signs, tags or barricades to identify hazards
and warn employees.

Emergency planning will be addressed in the PSP that identifies phone


numbers and response teams for fires, medical emergencies, and other
accidents.

First – Aid box shall be clearly labelled and one box per site locations shall be
maintained. The medicines shall be checked periodically to ensure the required
quantity as well as the date of expiry is verified.

4.2 Work Location Analysis


The safety office will conduct baseline surveys for safety and health regularly
by carrying safety inspection report. Results will dictate what type of protective
equipments may be necessary.
Work site inspections and surveillance will be conducted by the safety officer
regularly. Corrective Actions and Improvement Notices will be issued to
improve safety performance and prevent unnecessary hazards fro occurring.
All accidents/incidents and injury/illness cases will be investigated to determine
the root cause and to analyses cases for lessons learned
Information to prevent a recurrence.

4.3 Construction Safety Goals


Albanna Engineering’s Construction Safety Goals include the following:
 Zero Fatalities
 Keep data gather, record data
 Analyze information
 Determine worst risk areas
 Reduce worst cases to 0 %
 Safety Performance to be continuous review and adjust.

4.4 Causes of Accidents


Supervisors have responsibility for the safety of employees who are assigned
to them and must exercise good judgment in the administering of safety rules
and procedures. They must see to it safety rules and procedures are followed
and must take corrective action when these are not complied with. They must
investigate accidents and make recommendations and take corrective actions
and preventive actions when these are not complied with.
All employees are responsible for performing their activities in a safe workman
like manner. Each employee must know, obey and follow safety rules and
procedures relevant to his activities.

Employees are encouraged to report the incidents / near miss as soon as it


happens. Concerned sites shall have incident / near miss reporting box in
order to capture maximum number of incidents.

4.5 Safety Violations


All employees are held accountable for their actions or inaction pertaining to
safety and health issues on the job. Employees unable or unwilling to follow
Safety rules, procedures, or safe work practices, will not be allowed to continue
to work for the company. Disciplinary actions will be taken where it is deemed
appropriate.
Repeated safety violations will not be tolerated. Employees who violate safety
requirements should be disciplined under the following progressive disciplinary
policy:
5.5.1. First Violation: Documented verbal instructions and reprimand by the
supervisor.
5.5.2 Second Violation of same offence: Written reprimand to personal file.

5.5.3 Third violation of same offence: Employee may be permanently


dismissed at the discretion Company.
4.6 Compliance with safety regulation
The workforce, whether direct hired employees or contractors personnel, are
required to comply with procedures and local rules that may be appropriate to
them, Awareness of these procedures, and the requirements are adhere to
them, will be brought to the workforces attention by a variety of methods.
These methods include:
- Induction- for general rules
- Job orientation for specialist regulation & procedures
- Safety notice board
- Tool box talk

4.7 Maintenance and inspection of plant and equipment


Operational, inspection and maintenance procedure are established. These are
controlled documents, and they are available at relevant locations.
All plant and equipment will be maintained in accordance with ABE
maintenance s, procedure and manufacturers recommendation as appropriate.

Operation Manager will ensure the availability of resources and competent


maintenance teams to ensure that plant and equipment are maintained and
safe to operate.

4.8 Alcohol & Non-Alcohol Prescription Drugs

Alcohol and addictive drugs will not be permitted to ABE Workshop premises,
Project sites and any other operating facility under the control of ABE. Persons

4.9 Shift Handover
A system in place for shift and crew handovers to ensure the safety of ongoing

4.10 Safety Awards & Appreciations

The Project Managers / Project In-charges shall identify the safety conscious
Employee who is contributing for Safe working practices and award them on a
monthly basis. The award program shall contribute in improving the safety
system compliance as well to act as a motivating factor to other employees.

4.11 Environmental Operational

4.11.1. Environmental Officer identifies the operations and activities that are
directly linked with the significant environmental aspects.
4.11.2. An operational control procedure will be prepared for those operations
and activities which are routine activities and not covered by management
program.

4.11.3. Operations Manager will ensure that procedures are prepared and
maintained to identify routine operations and activities related to the significant
environmental aspects. These aspects and their impacts will be recorded in the
Aspects Register, as detailed in the Environmental Aspects Procedure.
4.11.4. These activities including maintenance of plant and equipment are
planned in order to ensure that they are carried out under specified conditions.
4.11.5. Documented operational control procedures are maintained to cover
applicable routine activities. Procedures for specific activities which require
management program are covered by Environmental Management Program.
4.11.6. Documented procedures exist for communicating relevant procedures
and requirements to suppliers and sub-contractors. ABE “Environmental Policy”
will be printed on the purchase order. Environmental Procedure will be attached
with purchase oreder to sub-contractors and suppliers, as relevant.
4.11.7. M.R and Environmental Officer will inspect sites and workshop to ensure
that various operations and activities are carried out as per Operational Control
Procedure. They will also advise corrective actions and/or issue NCR’s as
appropriate.

4.11.8. In order to ensure the Air pollutions are within the limits, Emission
monitoring, Noise level and Waste water disposed shall be measured at
Workshop once in a year. These results shall be reviewed by the CEO against
the DM requirement and discussed during the Management review meeting.

4.11.9. The waste water after the equipment & vehicle wash shall be collected
in a separate tank. Water shall be disposed after the sample testing through
Third party Laboratory and obtaining Dubai Municipality permit.

3.0 RESPONSIBILITY
3.1 HSE Officer is responsible for defining safety and environmental objectives and
targets together with PO and CEO
3.2 Project Managers and Operations Manager are responsible for managing day-
to-day operations for achieving ABE’s safety and environmental objectives and
targets and develop appropriate measures to track progress towards meeting
the objectives and targets.
3.3 Project Engineers and Supervisors are responsible for implementing and
meeting the goals within their operational area. They are also responsible for
communicating objectives and targets and the means for achieving them to
employees working within their operational area.

4.0 PROCEDURE
4.1 Safety & Environmental objectives and targets for each activity will be
established in line with ABE objectives procedure ABE-QA-OB-01. Objectives
and targets will be consistent with ABE’s IMS Policy.
4.2 In establishing objectives and targets, consideration will be given to:

review will endorse the safety objectives and targets.
4.4 The performance against objectives and targets is also communicated to
employees via inter office memos, tool box talks and bulletin board articles.

4.5 Any new or revised legal requirements to be complied shall be made as


objectives and systematically followed-up to achieve.

4.6 The objectives can be formed out of an Incident, Near miss or other lesson
learned in order to mitigate the same. These objectives shall be followed
closely.

INCIDENTS HANDLING PROCEDURE


1.0 INTRODUCTION
An accident is an unplanned, undesired event, which results in personal injury, ill
health or fatality. Measures must be imitated to prevent recurrence in any area of our
operation. In order to achieve this all incidents must be thoroughly investigated.

2.0 PURPOSE AND SCOPE:


The purpose of this procedure is to guide Albanna Engineering management in
reporting, investigating and monitoring follow up actions associated with accidents and
near misses
Scope:
The procedure shall be applied to reporting, investigating, and monitoring of follow up
actions associated with all accidents and near misses.

3.0 DEFINITIONS AND ABBREVIATIONS:


Definitions:
Accident : An accident is an incident which has given rise to
injury, ill health or fatality.
Incident : work related events in which an injury or ill health
regardless of severity, fatality occurred or could
have occurred, near miss.
First aid injury : An incident requiring first aid treatment at site
Lost time injury : An incident where more than one full shift is lost
Medical Aid Injury : An incident requiring medical treatment from doctor
Property Damage : An incident resulting in damage to property
Near Miss : An accident, which did not result in an injury or
injury could have occurred.
Emergency Situation : Particular type of incident.

4.0 RESPONSIBILITIES:
Incident Management is both an individual and shared responsibility of all employees,
subcontractors and subcontractor employees on each work site. Acceptance and
commitment to the following areas of responsibility is essential for the success of
incident management reporting.

Project Manager: The project manager is responsible for assigning one responsible
safety officer for ensuring incident reporting is complied with. The project manager is
also responsible for the sole authorization of incident Report. Project managers will
also participate in investigation of any accident or incident that has a classification of
moderated risk and above.
Safety officer: The safety officer is responsible for ensuring that incident reports are
completed for all report incidents that occur on site in accordance with this procedure.
The safety officer is further responsible for co-ordinating and assisting in any
investigation, which takes place.
Supervisors: Supervisors are fully responsible for the reporting of all incidents within
their work areas, in line with the requirements of this procedure. They are also
responsible for completion of or assisting in the completion of reports to the project
manager.

5.0 REFERENCE:

6.0 PROCEDURE:
6.1 Reporting an Accident
The following procedure will be followed when an incident occurs
 Initiate emergency response procedure if necessary
 If possible, try to secure the safety of any other casualties or persons who
may become affected.
 Arrange medical help is it is needed. if necessary, transport casualties to
nearest clinic and if required make arrangements to transport to nearest
hospital.
 Immediately inform project manager.
 The project manager will take control of the situation and inform ABE
management. He will also inform client of the situation.
 The project manager will send a preliminary accident report to the CEO
along with follow up action plan. This report will be completed on the
company form attached to this procedure.
 Any client documentary requirements further to this will also be complied
with.

6.2 Reporting a Near miss


An unsafe occurrence, which does not result in injury. Damage or loss is called
near miss. All near miss will be investigated to assess the cause in order to
prevent future occurrence.

6.3 Investigation of accident


6.3.1 all incident , near miss and any injury resulted from safety violation will
be investigated.

6.4 On Notification
The location engineer and the supervisor concerned shall immediately visit the
site of the incident
 to ensure that the site is in safe condition
 to identify the cause of accident.
6.4.1 Investigation Team: An investigation team will be formed for all serious
or high potential incidents. The GM will appoint member based on their
experience and areas of expertise. However, in most case, safety officer,
project in-charge, supervisor, injured person, employee representative,
other connected workers will be part of that team to provide advice on
HSE matters.
6.4.2 Investigation: Careful examination of accident scenes is an essential
part of investigation. The following are just some of the areas to take into
account when doing this.
 Positioning of vehicles, plant and equipment.
 Condition of any related injuries.
 Time of occurrence
 Positions of people.
 Weather conditions.
 Quality of illumination – natural or provided.
 Markings on surfaces which are related to the incident.

6.5 Interviewing Witness


All witness will be interviewed in privacy where possible and in as informal a
manner as possible. A record of any statement must be made and the details
verified with witness. Any discrepancies should then be discussed and clarified.

6.6 Procedures and instructions


When written procedures or work permits were in use at the time of an
accident, it should be established to what extent these were followed and
understood. The same applies to any instruction given by supervisor in relation
to that particular task.

6.7 Investigation Analysis


After all relevant information has been gathered the investigation team will
evaluate it. From this a decision on the root cause will be taken.

EMERGENCY PREPAREDNESS AND CONTROL PROCEDURE


1.0 PURPOSE AND SCOPE
The potential for emergencies and disasters at all constructions sites and facilities and
their associated costs can be devastating in terms of employee casualties, business
interruption, loss of capital investment, etc. These events cannot always be avoided
but can reduce their frequency and impact by developing emergency response plans
that address immediate concerns with Albanna Engineering operations and which also
interface as required, with emergency response procedures requested by the Client.
The following instructions specify Albanna Engineering minimum emergency planning
and response instructions.
Albanna Engineering expects that all personnel working on site will adhere to the
following emergency response instructions.

2.0 DEFINITIONS AND ABBREVITIONS

Emergency : Serious, unexpected, and potentially threatening situation
requiring immediate action
CEO : Chief Executive Officer
HR : Human Resources Officer

3.0 RESPONSIBILITITES
3.1 Project Manager: The Project Manager is responsible for assigning one
safety officer with sole authorization for the compliance with these procedures
and the effective operation of associated work activities.

3.2 Safety Officer: the Project Safety officer shall be responsible for supporting
Emergency Response Operations.
Provide technical information, training and support to Project employees.
Maintain an information network with the PO.
Monitor employee adherence to the emergency instructions, and
implementing of this procedure.

3.3 HR Officer: Liaise with local hospital/police/medical insurance company for


emergency medical assistance to employees.
If any employee is injured in the incident inform his / her relatives as requested
/ appropriate

3.4 Chief Executive Officer: Deal with local authorities on key issues involving the
emergency response.

3.4 Finance Controller: Inform insurance companies for claim. Provide adequate
funds for mobilizing emergence response resources

4.0 PROCEDURES
4.1 Emergency: An emergency is an abnormal incident posing a threat to the
safety of workers, residents, the environment or property at a facility or site and
which can be brought under control using the resources and procedures for
emergency response in place for the facility or site.
4.2 Disaster: A disaster is an emergency which poses a more serious threat to the
safety of workers, residents, the environment or property at a facility or site and
which cannot be brought under control using the resources and procedures for
emergency response in place for the facility or site. Whether an emergency
becomes a disaster depends on the following:
 The type of facility;
 The hazards of the facility operations;
 The proximity of neighboring communities or other facilities;
 The capabilities of emergency personnel; and
 Mutual aid capabilities of outside agencies.
4.3 Emergency Planning: This involves the development of a specific plan which
details actions to be taken by trained personnel during an emergency in an
effort to efficiently control it and minimize its net negative impact on workers,
residents, the environment or property at a facility or site. This type of planning
also extends to developing emergency control strategies and instituting training
and drills for all facility personnel.
4.4 Emergency Assembly Point: The location will be designated by the Safety
officer as per the project conditions or as per clients designated assembly
point. Support staffs assemble to respond to an emergency. It is centralized
location for monitoring the facility response and also serves as a command
center for co-ordinating all communications, including the allocation and
distribution of information. Workshop assembly is designated near security
room

make their way to the designated assembly point.
4.5.6 A daily list of all persons working for Albanna Engineering will be kept up
– to – date & used in the event of an emergency to account for everyone
on site. Emergency Response Coordinator to check names of all
persons at assembly point against the list.
4.5.7 No persons to re-enter affected area until notified as being safe to do so
by Project Manger / Safety Officer/Emergency Response Co-coordinator
or relevant emergency services authority.
4.5.8 Develop as appropriate, document and follow the emergency response
training / drill items given below to ensure that all personnel expected to
participate in emergency response are proficient in its theory and
practice.
4.5.8.1 Fire Protection Education
Fire protection education for the workforce typically involves
the following:
 Emergency Response
Emergency response training will be one or more times per
year. The workforce is needed to know how to turn in and
how to respond to alarms. This shall be rehearsed
 Fire fighting
Workers shall be practiced to use of portable fire
extinguishers and hose at least yearly if they are expected
to use the equipment. A yearly review of the fixed fire
protection systems and manual operation of those systems
is necessary
Log book shall be maintained for updating the status of fire
fighting.

CONTROL PROCEDURES

1.0 PURPOSE & SCOPE:


Hazard and risk assessment covers all aspects of ABE & its sub-contractor’s works.
The main purpose is to identify the hazards involved, develop and implement the
measures necessary to control or mitigate the risk.

2.0 DEFINITIONS:
The following definitions apply to hazard and risk assessment.

Activities : An ‘activity ‘ is a grouping of tasks at a level where it may


be possible to identify Hazards
Control : A formal control measure to prevent undesired events
occurring.
D M EPPS : Dubai Municipality Environmental Personnel Protection
Section.
Event : Incidents that may occur as result of a hazard being
released.
CEO : Chief Executive Officer
Hazard : The potential for causing injury, damage or Loss
Mitigation : A measure to prevent, or reduce escalation
consequences of an event.
MSDS : Material Safety Data Sheet.
Risk : The severity and the likelihood of the exposure to the
hazard.
Severity : The severity of injury, damage or loss that can result from
event.
Task : A ‘task’ is work, which is conducted following a formal
procedure or set of work instructions. This is the lowest to
which activities are broken down for the purpose of
hazard analysis.

3.0 RESPONSIBILITIES

3.1 Chief Executive Officer: The CEO is responsible for providing the support
necessary for the successful implementation of this procedure.
3.2 Managers: All management employees are responsible for the prevention of
injuries in their respective organizational units through measures such as risk
management.

3.3 Project Managers: Project managers are responsible for risk assessment
on their responsible project sites. They are also responsible for assigning
responsible persons for ensuring risk assessment is carried out on their
project sites.
3.4 Safety officers: The safety officers are responsible for monitoring that this
procedure is implemented and adhered to by all personnel/subcontractors.
They conduct regular workplace inspection and ensure that any
improvements resulting from such an inspection are actioned in the required
time frame.
3.5 Supervisors: Supervisors have key responsibility in the planning and co-
ordination of work activities and assessing potential safety hazards on the job
and instructing employees in risk control methods to accomplish the work.

3.6 Employees Representative: The nominated employees representative will


play major role in collecting information directly from the workmen/site and
report the concerned. Express the OH&S concerns with monthly quality
council meetings.

3.7 Workmen: Workmen shall participate during the risk assessment process.
The necessary inputs regarding the activity related hazards inputs shall be
given.

4.0 PROCEDURES:

4.1 Identifying Hazards, Risk assessment and determining controls:

A. The procedure of Hazard Identification & Risk assessments shall be as


follows.

a. The activities includes routine & non routine.

b. Activities of all persons having access to the work place including


contractors & visitors.

c. Human behavior, skills, capabilities and other human factors.

d. Hazards originating from outside ABE workshop / sites which shall


directly affect the health & safety of ABE employees.

e. Hazards created due to day to day activities of ABE and its sub-
contractors / suppliers.

f. Infrastructures, equipment and materials at the workshops, sites or


customer designated areas.

g. Changes made to company activities, process, machine capacity or


model, machine re-location etc.

h. HSE Management system changes including temporary changes, and


their impacts on ABE activities, process and operations.

i Applicable legal obligations if any to the risk assessment and


implementation of necessary controls.

j Design of work areas, processes, installations, machinery/equipment,


operating procedures, method statements including the adoption to
human capabilities.

k. Hazard identification & risk assessment process shall consider the


necessary inputs from all concerns like concern parties, employees
representatives, workers, operators etc connected to the particular
activities

The nature of methodology for Hazards Identification and risk assessment


shall be to ABE activities including sub-contractors, appropriate in nature and
it shall be proactive than reactive.

All the identified hazards shall be prioritized, documented and control


measures shall be defined. If any management changes corresponding
hazards & risk associated with changes in the organization shall be
identified. These changes shall be checked against the HSE system and its
activities prior to the introduction of such changes.

While determining control measures, all above assessments results are


considered and it shall be ensured.

B. The controls or changes to existing controls shall reduce the risks


according to the following steps.

a. Elimination

b. Substitution.

c. Engineering controls.

d. Administrative controls / signage / warning displays.

e. Personal Protective equipments (PPE).

The results of Hazards identification, Risk assessments and applicable controls


shall be documented. It shall be ensured that the documents are updated
periodically.

 Obtain work permit before starting any work under or near
electrical power lines.

 Administrative controls, i.e. Implement safe work instructions.
The role of management is to create an environment in which risks
can be identified, quantified and controlled and secondly to create a
forum or process in which positive risk assessment can occur.
N.B Risk management is a continuous cyclic process.
After a changing or altering risk control choices, risk assessment
should be repeated for new projects.

After carrying out risk assessment and adopting suitable control


measures there is a format attached to record this process. A
hazard identification record form and register are contained in the
“forms” section of this procedure.

1.0 OBJECTIVE

To identify the environmental aspects and hazards of all activities carried out
by CCCL and assess the significant impact on environment, risk on the work
activity and to adopt suitable control measures.

2.0 SCOPE

This procedure covers all activities performed at project sites and offices.

3.0 RESPONSIBILITY

3.1 Head - HSE is overall responsible for implementing this procedure.

3.2 PC/PM/SE are responsible for identifying the activities with significant impacts
and risks before starting new project and prepares operational control
procedures.

4.0 PROCEDURE
4.1 Initial Review

4.1.1 PC/PM/SE to carry out Initial environmental review and assessment of risks to
establish the present organization level with respect to environment and
occupational Health and safety.

4.1.2 The initial review covers the following.

¾ Status of compliance with applicable legislative and regulatory


requirements.
¾ Concerns of all interested parties.
¾ Identification of environmental aspects and hazards of its activities,
products & services during normal, abnormal and emergency situations, to
determine those aspects that have or can have significant environmental
impacts and Major Risk.
¾ Evaluation of performance of existing environmental management,
occupational Health and safety practices and procedures.
¾ Feed back from the previous incident of nonconformity.

4.2 Identification of Aspects

4.2.1 For Identification of aspects and impacts, the following steps are to be followed.

a) Select an activity, product or service. The selected activity, product or


service should be large enough for meaningful examination and small
enough to be sufficiently understood.
b) Identify as many environmental aspects as possible associated with the
selected activity, product or service.
c) Identify as many actual and potential, positive and negative, environmental
impacts as possible with each identified aspect.

4.3 Evaluation of Significant Impacts

4.3.1 The evaluation of the significant impacts from the identified aspects and
impacts is to be carried out by considering the following.

a) Legal concern (LC)

Any of the impacts covered by existing applicable environmental


regulations of the State, Central Governments in India or the country in
which the project is executed and shall be considered significant.

b) Interested Party Concern (IPC)

To consider the feedback on Environmental Performance from interested


parties viz. a) Employees b) Visitors c) Sub contractor and his workmen. d)
Vendors/Suppliers.

The Core Team through Head - HSE will decide the significance based
on assessment.

c) Resource Conservation Potential (RCP)

Any Environmental aspects having Resource Conservation Potential or


Resource Saving Potential worth > Rs. 25,000/year will be rated as
significant.

4.3.2 All impacts having potential emergency situations shall be identified as


significant.

4.3.3 The evaluation criteria using QSPD shall be done for those impacts, which are
not identified as significant under LC, IPC and RCP.

4.3.4 Environmental impact is measured through QSPD criteria as detailed below,


(Refer Format No. : CSF-002A)

4.3.5 The QSPD criteria for evaluation is as per Table-1.


Aspect calculation is done as per the following.
Aspect Score = Quantum (Q) x Severity (S) X Probability (P) x Duration (D)

4.3.6 Any of the impact having a score of QSPD as greater than or equal to 81
shall be considered as significant. This score shall be reviewed once a year.

4.3.7 Basis for Cut-off score :

The basis for cut-off score is taken as 81 or more as significant considering the
following :

Quantum – within the facility = 3


Severity – Moderate release of toxic = 3
Probability – Weekly or more = 3
Duration – Lasts for a week = 3

Apart from the above experience of other industries was also considered.

4.4 Identification of Hazards, Risk Assessment and Determining Controls

4.4.1 Man interface sub-activities are selected for identifying the Hazards/Risks
associated with the following activities (additionally check Lists at Annexure -1
is also used):

a. Routine activités
b. Non Routine activités
c. Obvious hazard locations
d. Auxiliary equipment and operational condition
e. Activities of all personnel having access to the workplace including
contractors, visitors, infrastructure, equipment and materials
f. Facilities at the work place, within the defined scope, nature and timing to
ensure that it is proactive rather than reactive, including human behavior,
capabilities and other human factors

g. Changes or proposed changes in the Organization, its activities or materials


h. Design of work areas, processes, installations and such infrastructure.
i. Hazards originating outside the workplace capable of adversely affecting the
health and safety of persons under the control of the organization.
j. Hazards created in the vicinity of the workplace by work-related activities under
the control of the organization.

4.4.2 All the identified hazards are evaluated for significance of the risk by using
appropriate risk assessment rating criteria (Severity X Occurrence greater than
or equal to 9 as “Major Risk” and any ill health causing hazard as “Major Risk”
irrespective of severity rating).

4.4.3 The Hazards evaluated as “Major Risk” is subjected to Hierarchy of


Determining Risk Control in the order of elimination (remove the hazard
totally), substitution (substitute the hazard with a less hazardous), isolation
(Isolate or separate the hazard from person at workplace) engineering controls
(modification of equipment, guarding, mechanical ventilation etc.),
administrative controls (change work procedure or policies ex. rotating
jobs, limiting time exposed to the hazard or display of signage / warning
boards), and PPE usage practices. The acceptable risk is with a score of 5 or
less as per criteria set in the rating plan given at 4.3. Also for health related
hazards, irrespective of “Severity” Score, Health Surveillance concept is
introduced.

4.4.4 The identified hazards, major risk and hierarchy of risk controls have been
listed in HOD / Departmental File

4.4.5 The master list of the Major Risks is kept updated by considering new process /
equipment, if any, added. For the management of change, identification of
hazards and risks associated with changes in the organization, the OH &
S MS, or its activities, prior to the introduction of such changes
envisaged.

4.4.6 The following evaluation factors are considered to arrive at the Major Risks:
A. Legal

The identified Hazards that are linked to applicable Legal & Other
requirements having relevance to employee welfare.

B. Occupational Health & Safety (OHS) Concerns

The impacts of the identified Hazards are evaluated on `Occurrence’ and


Severity’ (Refer HSE Format No. CSF - 002A) as stated below:

4.5 Evaluation for Major Risk

4.5.1 OHS Hazards that attract Legal and Other Requirements are evaluated as per
4.4.6 A. If the score is 9 and above, or of concern to ill health then the Hazard
is rated as “Major Risk”

4.5.2 If identified Hazards do not attract any Legal & Other requirements, then its
occurrence & severity is evaluated by multiplying (i) x (ii) i.e. Occurrence x
Severity (Refer Table below). Hazard scoring 9 and above attracts “Major Risk”
and the rest are for future considerations.

The Hazard identification / evaluation are done by each Department/Division as


per the Format shown below. In the case of purchase of new equipment,
chemicals and new services, Purchase Department, Safety Department and P
& M Department will assess safety considerations and adhere to specified
procurement practices.

The Major Risk list is prepared and communicated to MR, which forms the
framework for OHS performance improvement. On the basis of this, Objectives
are set aligned to Company-Wide Objectives & Targets. Such Major Risks are
to be discussed during the Management Review Meeting or a special meeting
convened for the purpose.

4.6 Adoption of Control Measures

Significant Impacts as per clause 4.3 and Major Risk as per clause 4.5 will
be reviewed and appropriate Operational Control procedures (OCPs) will
be developed for compliance.

Refer CSP-4.4.6 Procedure for “Preparation & Issue of Operational


Control Procedure”.

4.7 Changes in Activities/Products/Services

Prior to introducing any change in activities/products/services/process


/equipment, the concerned Construction Engineer initiating the change would
undertake identification of significant environmental Impacts and Major Risks
as per the aforesaid procedure for deciding whether,

a) The list of significant aspects & impacts and Major Risks needs to be
modified or
b) New objectives and targets need to be set.

4.8 Document Review

The overall review of the aspects / impacts and hazards / Risk shall be carried
out at least once in a year.

Occupational Health & Safety and Environmental management, communicate
the same to all concerned and project sites.

As defined in the procedure.

4.0 PROCEDURE

4.1 Identification of Legal & Other Requirements

4.1.1 Head - HSE management is responsible for organizing the updating and
maintenance of the records of all applicable legislations and regulations and
communicates to relevant persons.

4.1.2 PC, PM and SE are responsible for identifying the applicable legal
requirements of the particular States and Central Government in India or the
Country in which any project being executed.

4.1.3 The Legal & Other requirements that safeguard welfare of the general
environment vide MOEF, CPCB and local regulatory mechanisms are
considered for ISO 14001. The Legal & Other requirements that safeguard
welfare of the employees and general work environment vide Factories Act,
1948 and such other Acts are considered for OHSAS 18001. Being
Construction activity, the client specific requirements are also verified through
ROR for compliance towards ISO 14001 and OHSAS 18001 requirements

4.1.4 Head - HSE organizes for identification of applicable legislative, regulatory


and other requirements by assessing the information obtained from,

a) CPCB & local PCBs Acts, rules, regulations and notifications.


b) Consents/authorizations/licenses obtained under various Acts.
c) Discussions with safety & environmental professional during meetings,
seminars and conferences.

d) HSE related legislative and other books like Building and Other
Construction workers (Regulation of Employment and Condition of
Services) Act., and other publications.
e) Meetings with clients/customers to whom construction activity is carried
out.
f) Information received from industrial associations and also interacting /
hiring professional services.
g) Meetings with construction engineers to bring in clarity on other
requirements viz CCCL policy guidelines.
4.1.5 The applicable legal and other functional requirements shall be listed in the
Register of Regulations (ROR) and maintained by Head - HSE at Head Office
and PM/SE at Site to ensure compliance.

4.1.6 All sites shall maintain the list of competent persons and their responsibilities
as stipulated in regulatory requirements and also their responsibilities

4.1.7 PM/SE/SS to ensure that all the relevant permits / consents / licenses /
authorizations are available and renewed.

4.1.8 Head - HSE to ensure the communication of legal and other applicable
requirements including specific conditions, if any, to the concerned persons.

4.1.9 PC/PM/SE to identify local applicable legal requirements related to the


environmental aspects specific to the location of the site by interacting with

4.1.10 PM/SE/SS to send compliance report quarterly to Head - HSE management.

4.1.11 Head - SCM to ensure that whenever an item /service is purchased or sub-

purchase order /sub-contract order. The indenter to ensure communication
of such requirements in the indent clearly.

4.1.12 Head - HSE to communicate nonconformities with respect to legal & other

to the Organization or parts there of, that arises from the HSE objectives and
that needs to be set and met in order to achieve those objectives.

4.3.1 CCCL, considers primarily the following for setting ”objectives and targets” :

4.1.3 If any of the stages/steps in HSMP/EMP involves activities to be carried out

4.1.6 HSMPs/EMPs shall be declared completed by PC/PM, only after validating


and demonstrating the achievement of set targets. The status of
implementation of HSMPs/EMPs to be informed to Head - HSE.

4.2 Review and Updating of HSMPs/EMPs

4.2.1 PC/PM to review and update the HSMPs/EMPs as considered necessary


whenever there is new development or change in current activities and
communicates to Head - HSE.

4.3 Monitoring implementation of HSMPs/EMPs

4.3.1 PM/SE to implement the HSMPs/EMPs as per the action plan. PC/PM to
monitor & report the progress to Head - HSE to enable him to present to
Company HSE committee.

4.3.2 PC/PM/SE to review and update the specific aspects/impacts, hazards/risks


related to those HSMPs/EMPs upon completion .The need for developing
OCPs for continuance of benefit as specified in the HSMP/EMP will also be
reviewed.

technical expertise, managerial / supervisory skills and various soft skills

iii) Project Manager to identify competency gaps of other site personnel

4.3 Head - HSE to revise the documents when needed and release revised
versions to the users. He shall update the master list, accordingly.

4.4 The HSE system documents are issued to the users through electronic
system or controlled hard copies as necessary by Head - HSE.

4.5 In case of hard controlled copies issued, holders of the controlled copies are
responsible for removing / destroying obsolete versions of documents on
receipt of revised versions.

4.6 Head - HSE to maintain original signed documents as 'Master Copy' by


identifying them in specific folders marked so.

Control procedures for controlling the significant aspects/hazards.

4.3 SE/SS to prepare Operational Control procedures (OCPs) in areas including


non routine activities where the absence of which could lead to deviations
from the stated HSE - Policy and the objectives and targets.

4.4 OCP to be duly linked to relevant aspects/impacts/hazards/risks.

4.5 SE/SS to prepare the OCP in consultation with the group performing the
activity/service. The OCP will cover purpose, area of control including
permits-to-work, responsibility and description of the operation as per
guidelines detailed in Annexure.

4.6 PM to review and approve the OCPs and issue for implementation.

4.7 PM/SE to communicate the ‘OCP’ to concerned operating personnel /


suppliers and sub-contractors as applicable for compliance.

4.8 PM/SE to identify training needs of the individuals responsible as identified in


the OCP and ensure proper training.

4.9 Construction Engineer can also identify the requirement of making OCP for
non-significant aspects in certain activities to prevent the non-significant
aspect/hazard from becoming significant, if not effectively monitored and

GUIDELINES FOR THE PREPARATION OF OPERATIONAL CONTROL


PROCEDURE

1.0 PURPOSE

The HSE aspect/hazard related to the activity causing the significant


impact/risk is to be addressed. The purpose of OCP is to control/maintain the
significant HSE impacts/risks.

2.0 AREA

Areas for which this OCP is applicable shall be clearly mentioned. There can
be more than one area where this OCP may be applicable in the concerned
area.

3.0 RESPONSIBILITY

a) It shall cover both the supervisory and operational responsibility.


Supervisory responsibility shall be given to the person who
checks/verifies/ ensures that a procedure is being effectively performed.
b) Operational responsibility shall be given to a performer who carries out
an activity in a step by step manner. An overall responsibility shall also
be defined to oversee the implementation of OCP(s).

4.0 DESCRIPTION

4.1 Points of Operation

The step by step function of an activity with the inclusion of key operational
parameters. Essentially, these are the steps that a performer/operator shall
follow.

4.2 Points of checking with periodicity

The list of routine checks along with periodicity. The type of checks to be
done to monitor the procedure written in points of operation. The identification
of person/level also needs to be addressed.

4.3 Permits-to-work

a) The permit work shall be taken for hot work and other hazardous work
activities such as deep excavation, confined space, etc.

4.4 Points of preventive maintenance with periodicity

The list of preventive maintenance checks along with periodicity shall be


maintained. Here not only preventive maintenance for the machines but also
preventive actions to be taken to minimize nonconformity situations shall be
addressed.

4.5 Points of corrective action in case of deviations

In case of deviations, the corrective action to be immediately taken shall be


addressed.

4.6 Records

4.6.1 The records in the form of filled in formats / log books to be maintained shall
include title, location and retention period. The records shall have the
information /data /evidence that an OCP is being effectively practiced.

4.6.2 The place /area where an OCP is required to be implemented, it shall be


displayed and made available to the persons performing and checking the
concerned activity.

and response plans based on the need of technical/ political/ social and work
environment prevailing at sites.

4.2 SE/SS to make arrangements for display of these identified plans at


important locations in site.

4.3 SE/SS to train/ create awareness among all people working at site through
training/safety talks/mock drills with respect to above plans.

4.4 Training and Mock Drills

4.4.1 PM/SE to identify the training needs and provide the training required to the
site personnel for handling effectively any emergency situations.

4.4.2 PM/SE to review the effectiveness of such training through mock drills or any
other suitable means / methods and continue to monitor the effectiveness at
periodic intervals with respect to awareness and preparedness of the
concerned agencies / personnel to meet the emergency

4.4.3 Test the efficacy of the plan and for evolving possible improvement /
alternatives to the existing one.

4.4.4 PM/SE to record the response during the mock drill and review for improving
the emergency preparedness.

4.5 PM/SE to identify the aspects/hazards associated with emergency


situations and evaluate their impact/risk level and to prepare OCPs for
minimizing / preventing the chances of emergency situations happening at
site. Control procedure shall include safety interlocks wherever required,
regular preventive maintenance, monitoring of the personnel working in those
areas.

4.6 PM/SE to consider emergency situations arising out of occupational health


and safety related issues as a result of epidemics etc., and prepare
emergency preparedness and response plans accordingly.

4.7 SE/SS to make arrangements for display of Contact details for emergencies
at site (format no.CSF-007). The in-charge responsible to display the
emergency contact details and emergency outlets (Exits) in the Offices.

4.8 PM/SE office / I/Cs to provide and maintain sufficient first-aid boxes with
necessary contents and specify the locations in format CSF-008.

4.9 PM/SE office / I/Cs to provide and maintain adequate fire extinguishing
equipment as per requirement and specify the locations in format CSF-009.

FIRE & EXPLOSION

In case you notice a fire do the following:

a) Rush to the spot and take charge of the rescue and control work.

b) Be concerned about your own safety and that of others

c) Shout, "Fire Fire", raise an alarm and call for help from colleagues.

d) If the fire is small, attempt it to extinguish by using the nearest fire


extinguishers available, without undue personal risk.

e) At the same time, inform fire station and PM/SE. While reporting state:

f) Instruct security staff to cordon off the affected area and guide the Fire
Brigade personnel to the exact location of the fire and render all possible
assistance to them.

g) Keep all spectators and non-essential employees away from the fire.

h) If required, switch off the electrical supply, taking adequate care.

i) Try to remove flammable/ dangerous materials lying in the vicinity without


endangering yourself.

j) If explosive type materials are involved immediately evacuate all personnel.

k) Count number of people involved and organize rescue of any trapped


personnel with the help of the fire brigade.

l) Make no comments. Refer all inquiries to the company project manager.

EARTHQUAKE
If you are inside the office building when an earthquake occurs:
a) Stay under cover until the shaking stops.

b) Stay away from windows, large panels of glass, ceiling tiles, file cabinets,
book cases, heavy furniture, electrical equipment and appliances.

c) Crawl under a desk or sturdy table and hold onto it. If it moves, move with
it.

d) Watch out for falling plaster or ceiling tiles.

If you are outside in the construction field :

a) Move to a clear area away from unbalanced/ unsupported/ partly erected


structure/ equipments/ buildings and electrical wires and poles that may fall.

b) Stay clear of falling objects and find a place to duck, cover and hold on.

c) Stay near an interior wall and protect your head with your arm.

d) Do not try to leave until the shaking is over.

LIGHTNING, STORM & FLOODS

In case of any lightning, storm and floods:

a) All employees to leave the work areas and take shelter under strong (RCC)
roof.

b) In case of heavy rain and indication of floods, people should take shelter
only in top floors of the buildings.

c) People should not stay by the side of weak walls, loose structures, trees,
electric poles, metal objects etc.

d) People should try to carry drinking water, food stuff available, protective
clothing, battery powered radio, first aid kit etc., to safe locations where
they are going to take shelter.

e) Follow the instructions given by local authorities over radios/TVs/Public


announcements.

f) Avoid using any electrical appliances and disconnect power supply to


electrical appliances.

g) Avoid using telephone, in case of lightning.

BOMB THREATS

When a bomb threat is received for the first time:

a) Local police, fire or bomb disposal authorities will be immediately notified.

b) The project or office shall be evacuated immediately.

c) Evacuation will be made of all personnel on the project or in the office.

d) A personnel count should be made to ensure that all are present and
accounted for.

e) Do not allow anyone except authorized personnel to reenter the area.

f) If necessary to stop or detour traffic away from the affected area, utilize
local police or flag man.

g) A search of the premises will be made by the appropriate authorities. No


company supervisory personnel or other employees shall be involved in the
search.

h) Notify other business people or residents who may be endangered by the


threat.

i) Appoint one person as the company spokes person.

j) Allow no photograph on site.

k) Make no comments.

l) Notify the company's local office immediately.

m) Assist the authorities with information and in every other possible ways to
anyone without danger.

n) If a suspicious article is found do not touch it. Inform the appropriate


authorities to handle the situation.

Enter the premises only after careful evaluation of the circumstances, (to be
made by the company's senior management in consultation with appropriate law
enforcement authorities) and permission of local management.

4.1 Incidents / Accidents

4.1.1 Injured person: Injured person shall intimate to his supervisor personally or
through his colleague (if he is in a position to send message).

4.1.2 Witness: Whenever any employee/worker observes that a person is met with
an accident or any dangerous occurrence, he has to inform the area
supervisor, first aider and if required shift the injured person to safe location.

4.1.3 Supervisor: To arrange first aid, to shift injured person to dispensary/


hospital. To intimate to Safety Engineer (SE), Project Manager (PM) and to
send report to PM/SE.

4.1.4 Supervisor / Safety Engineer

a) To arrange for first aid and treatment of injured person.


b) To intimate all concerned including the insurance company,
Government official (if required).
c) To send accident/incident reports to all concerned.
d) To conduct investigation and submit report as per 'Accident
Investigation Report'.

4.1.7 The concerned SE/SS to keep an accident register where all the incidents
near misses are to be recorded. This shall also include minor first aid done
within the area.

4.1.8 The Register shall have the details such as serial no, date, time and nature of
accident, location, cause of the incident, persons/equipment involved.

4.1.9 PM/SE/SS to report the accident / incident in the format CSF-012 (report of
accidents and dangerous occurrence) and send to

4.1.10 SE/SS to conduct enquiry into accidents/incidents along with sub-contractor’

4.1.11 SE/SS to ensure submission of monthly HSE report in the format CSF-016
(Monthly Safety Report) and submit to PM, PC and Head-HSE. For

4.2 Nonconformities

4.2.1 An Health, Safety & Environmental nonconformity is defined as,

a) Nonconformity to the specified HSE parameters for monitoring and


control.
b) Activity / process causing adverse impacts on the HSE system eg.,
excess release of pollutants, discharges to air, water, land- any
potential nonconformity.
c) Non adherence to defined HSE management system and procedures
and legal requirements.
d) Non-adherence to specified environmental parameters.

4.2.2 PM/SE/SS to take appropriate action, if the impact on HSE caused is minimal
and could be easily remedied. Such ongoing actions are to be acted upon
immediately as and when detected.

4.2.3 The concerned site staff to make efforts to contain the impact/risk on HSE,
when HSE nonconformity is noticed during operation or reported and inform
the actions to PM/SE/SS.

4.2.4 Any nonconformity situations causing /likely to cause adverse impact/risk on


HSE shall be reported immediately to the PM/SE/SS by the concerned
construction engineer/supervisor.

4.2.5 Construction Engineer/Supervisor is responsible for handling and


investigating the cause of such nonconformities.

4.2.6 Nonconformities noticed during internal audits shall be reported and


forwarded to Head - HSE. Necessary corrective and preventive action to be
taken by the respective SE/SS/PM.

4.2.7 Nonconformity reports raised as an outcome of findings from the results of


monitoring and measurements, audits and review of HSE Management
system shall be attended promptly. Necessary corrective and preventive
action to be taken by the concerned SE/SS/PM.

4.2.8 PM/SE/SS to review where the corrective and preventive action identified
new or change in hazards or need for new or changed controls, the
procedure shall require that the proposed actions shall be taken through a
risk assessment prior to implementation.

4.2.9 PM/SE/SS to review the relevant procedures based on the corrective and
preventive actions taken to eliminate actual/potential nonconformities and
revise the procedures if found necessary and communicate to all concerned.

4.2.10 Head - HSE to summarize the corrective and preventive actions taken for
audit nonconformities / potential nonconformities and present to the company

4.1.2 SE/SS to conduct HSE audits at least once a month using the checklists and
the status of implementation / compliance shall be reported to PM.

4.1.3 PM to ensure that corrective actions are taken to rectify non-compliances


reported and informs Head - HSE Management accordingly.

4.1.4 SE/SS to prepare site specific new checklists if new activities other than
identified above are taken up. SE/SS to revise the checklists to incorporate
any new requirements based on the site specific requirements.

4.1.5 The Internal Audit of the HSE Management System is conducted at


least twice in a year. Frequency of internal audit is increased, if
required, based on results of previous audit.

4.2 Audit is conducted by Internal Auditors provided with adequate training


by competent external agency and the list of such trained auditors is
maintained by MR. If necessary, MR also decides to take the help of
external trained auditors to assist in internal auditing.

4.3 MR decides the annual audit plan at the beginning of the year for
Internal HSE Management System Audit.

4.4 Audit Planning & Scheduling

4.4.1 MR/MA identifies areas for audit based on HSE importance, the
current status of HSE system, the results of Aspect & Hazard
assessment of activities concerned and the results of previous audits.
MR/MA prepares a schedule in consultation with various HODs and
circulates the schedule to auditors & auditees.

4.4.2 MR/MA decides the scope of audit for each of HSE Management
System element based on discussion with concerned PMs and
selects the internal auditors who are independent of the area to be
audited.

4.4.3 MR/MA provides the internal auditors with information about the
previous audit findings, if any, to check the effectiveness of corrective
action taken and revalidation.

4.4.4 MR/MA ensures cordial relationship between the auditor and auditee.

4.5 Conduct of the Audit

4.5.1 Internal auditors carry out audit based on


a. HSE Management System Manual, Procedures, OCPs.
b. Relevant HSE records.
c. Aspects / Impacts and Hazards & Risks identified and their
results.
d. HSE Management Programs
e. Relevant Legal regulations & other requirements.
f. Previous audit results.

4.5.2 The auditor, when observes nonconformity, shall raise NCR in format
CSF - 018 which will contain observations, proposed corrective
actions, actions taken. The auditor shall record the nonconformity &
the auditee shall record the disposition & proposed corrective actions
in NCR. Original copy shall be given to MR/MA & a copy to the
auditee. A copy is retained with the auditee for reference.

4.5.3 Classification of observations raised during Audit include

a) Minor non-conformities that can be corrected immediately.


b) Targets not achieved but there is considerable improvement and
system is in place.
c) Documents not updated / signed, but right system is followed.

4.6 Follow-up Action

4.6.1 The auditee on completion of disposition & corrective actions will


intimate auditor/MR so that auditor/MR can verify the implementation
& effectiveness of corrective actions which will be recorded in NCR. If
the corrective action is effective and found to be satisfactory the non-
conformity is closed.

4.6.2 If the auditor could not fully verify the effectiveness of corrective
actions, he will endorse accordingly and which will then be verified in
the follow up audit. In such cases, MR/MA will organise a separate
follow up at a later date or can organise for verification during next
cycle of audit. MR/MA retains the original copy of NCR & a copy
shall be given to the auditee.

4.6.3 Wherever any non-conformity & corrective actions occurring in one


identified area can be extended to other areas, MR will advise the
concerned HODs to implement such corrective actions as preventive
actions.
4.6.4 MR/MA prepares a consolidated report on the internal audit
observations for submission in Management Review Meetings.

4.6.5 Un-scheduled audits may also be undertaken as per discretion of


MR/MA under the following circumstances
a. In case of repetition of same non-conformities in that area
b. If the number of non-conformities are more in that area
c. Introduction of new process / activity.
If any change in the statutory / regulatory requirements.

10 Anchor the unsupported columns with stay ropes
11 Use tag lines or jackets for tools
When placing structured members, the load shall not be released from
12 the hoisting line until the member is secured by at least 50% bolts at
each connection and tight end
Lifting slings shall be as short as possible and gunny packing shall be
13
provided at the friction points
14 No overloading of material at crane or roof
15 Work area below must be barricaded
Weight of the heavy items shall be painted on the items to assist erection
16
engineer to decide about the crane capacity, boom angle of erection etc.,
17 Suspend the work during heavy winds and lower down the crane boom
