RE: MEDICAL REPORT FOR SKYNA MOHAMED 21 DAYS OLD, FEMALE

This is Skyna Mohamed 21 days old female delivered by cesarean section 3 weeks ago, no
any complications during and after delivery. Currently she is on formula milk due to the
mother’s illnesses with mother planned for chemotherapy. She has no any complain and she
underwent some investigations such as MRDT which was negative FBP show only mild
anemia of Hb 10.6g/dl.

Her mother is Amina Mdoba Lwikinjiri 38 years old female newly diagnosed patient with de
novo bilateral pulmonary metastasis left breast ca. She is waiting to start chemotherapy at
ORCI after completing the investigations.

With the mother current condition, she’s unable to properly take good care of her new born
daughter, and due to laws we cannot take care her in our facility we are kindly requesting for
your support in taking care of the child until the mother’s condition is stabilized and
improved after completing her prescribed treatment. Kindly assisting in taking care of her and
don’t to hesitate to contact our facility for any inconvenience
