WASTEWATER DISCHARGE PERMIT

Pursuant to Section 14, Article 2, of the RA 9275 otherwise known as the "Philippine Clean Water Act of
2004", this permit is hereby granted to Charlot Gonzales with office address at Purok 6, Bacnono,
Bayambang, Pangasinan for its establishment:

1. This permit holder shall discharge wastewater generated from the operation of Slaughtering and Meat
Packing to 3 units Septic Tank for Treatment; effluent not to exceed a Discharge Flow Rate of 5.6 cubic
meters per day.
2. Nearest receiving body of water: Agno River, and effluent shall conform to Class C Water Standards.
Significant effluent quality parameters for analysis by DENR recognized environmental laboratory shall
include the following:

4. Install monitoring equipment i.e. water meter, etc., for the determination of water consumption and
effluent discharge.
5. The permit holder shall allow entry of authorized EMB technical personnel to conduct
inspection/investigation of subject establishment or facility and compliance monitoring within your
premises.
6. This permit shall be adequately framed and posted at a conspicuous place at the plant/establishment.
7. Conduct wastewater sampling annually per source and submit analyses of effluent for parameters
tabulated in Condition No. 2. Further, generated domestic wastewater shall be regularly desludged by
an accredited desludger of the DOH. A certificate of treatment or receipt from the desludger must be
secured for the approval of Discharge Permit. Date of desludging shall not be greater than one (1) year

from the date of submission at EMB-1.

8. The permit holder shall appoint/designate a Pollution Control Officer (PCO) and shall secure
accreditation pursuant to Section 8 of DENR Administrative Order No. 2 Series of 2014, the Revised
Guidelines for Pollution Control Officer Accreditation.
9. Application for renewal of this Permit shall be filed on or before its expiry date.

This permit is valid up to Oct 28, 2027, unless revoked or suspended by this Office in writing.

Non-compliance with the above conditions and/or any pertinent provisions of RA 9275 otherwise knows as "Philippine Clean
Water Act of 2004", a corresponding penalty in the amount of P10,000-P200,000 per violation shall be imposed.
