03/10/22(Monday)

1) Shouting from time to time before 12 noon (lunch break)

2) 1:33 p.m.- was refusing to do task and throwing pencil and eraser. Yelling “no” at the
teacher.

Consequence:
Removal of teacher’s attention .

04/10/22( Tuesday)

Antecedent;

Jaycen , was given a prompt to give initial letter of the following objects.

Prompt:

Jaycen ,couldn’t utter at first the correct answer. So he was given ( verbal prompt) a hint . He was
asked if the picture is what being described.

Child’s Response:

Jaycen, was able to give a correct answer after he was given a prompt.

Consequence:

Jaycen, received a direct reward/ immediate ( positive praise ) .

5/10/22(Wednesday)

1) Time to time he likes to roam around the classroom but could be managed to let him sit.
2) When a principal came in to conduct observation, he can’t be remain seated .

Time-out:
He was temporarily removed from the classroom and moved to next vacant room and was
given an (A-Z puzzle card)

6/10/22 (Thursday)

He was just fine . Sitting quietly and finishing task in time . Sometimes standing but could let
him sit if being told.

7/10/22(Friday)

Behaving well in class.
