We, the (Grade Level-Section) student, would like to ask for your
permission to conduct a survey among (Respondents) students about the
research titled “TITLE OF RESEARCH”. This survey is a part of our research
report that will be submitted in partial fulfillment on the requirement for our
Mathematics subject. It complies with the following MELCS: (a) Formulates
statistical mini-research (M10SP-Ivf-g-1), (b)Uses appropriate measures of
position and other statistical methods in analyzing and interpreting research
data.

The survey will last only for 10-15 minutes per section and assured
that the data gathered will remain confidential and for academic purposes
only.
