TASK 3: ISSUANCE OF THE FP CLIENT CARD

After filling out the FP Form 1, the client will be issued a FP Client Card. This card serves as the FP service card of the client, which s/he will need to bring every
time s/he seeks any FP service from any facility. Let us help prepare 3 FP Client Cards for the following clients, Ana Cruz, Gemma Bautista, and Julia Cortez.
Gemma Bautista and Julia Cortez are both NHTS members while Ana Cruz in non-NHTS.
