Today I was with Brooke, and I saw a new type of rash today that I haven’t seen yet. It was
called granuloma annulare, which is a type of skin condition that causes ring-like rashes to
appear on the skin with the outer ring being bumpy or raised. The patient that had this was kind
of confusing us because she told the medical assistant she had it for about a month and then she
told Brooke she had it for about 2 months, and then she said it has been there for a year so we
don’t think she knew how long it was there for. It can be caused by a few things including bug
bites, injuries or even certain medications. It isn’t contagious or painful. Left untreated it can
resolve itself after a period of time, but if treated it will fade gradually. Brooke prescribed her a
cream to put on it so hopefully that resolves it. There was another patient who came in, a young
girl with what looked like a bad case of cystic acne, but it was actually impetigo which is a
contagious infection on the skin that can have several causes as well. In this case it was caused
by the tanning bed. This patient had prom coming up soon so she had been using the tanning bed
and probably didn’t clean it beforehand. Brooke prescribed this patient a month's worth of
doxycycline to help treat the infection as well as keep her face clear for prom, so it was nice that
Christine is back from her break so i am in macon with her today. We haven’t been so busy so I
have had plenty of time to work on my reflection paper. Another thing that I have realized while
interning here is how sensitive I am to smells. I already knew I was since I was little but I'm
realizing it a lot now since everyone has their own smell I guess. We had 2 patients in particular
Mikala Monroe
today that had a strong smell that it filled the room. It didn’t stink or anything, they just smelt
heavily of medication or something like that and it made me feel nauseous. But I’m proud of
myself for not having to leave the room because I won’t be able to do that when I am seeing
patients myself. I will however be able to put vapor rub under my nose so I will most likely do
that in the future. One of those patients was in for a contact dermatitis that they were following
up for. I saw pictures of how it looked the first time he visited and it had gotten infected, it
looked so painful. But today it looked so much better, it was almost completely gone and he was
very happy about that. He still had some irritated spots in other places which means it has to be
something he's coming in contact with daily, so hopefully he is able to figure out what is causing
it.
Today I am in Gray with Christine, and I am a little worried today because my grandmother is
coming in for her follow up appointment and treatment. Mostly because looking at her, it looks
like they’re getting worse but I trust that Christine will be able to help her. We had a patient this
morning coming in for warts, he was getting his 2nd MMR injection and usually you don’t see
results until after the 2nd injection but he showed results after his 1st injection because they were
almost gone. Then we had a patient that I had seen before. She was here for itching, but she saw
the cherry angiomas on her and thought that was causing it. Sometimes they can pop up so
quickly so I think she was just trying to look for something to make sense of it. I can sympathize
with her on that since I have bad allergies and if I don’t take my medicine, I will itch all over.
She had a history of thyroid problems and lung cancer so she was nervous that it could be
something else which is understandable. Christine prescribed her allegra and pepcid to reduce
both types of histamines and gave her a coupon for the cerave moisturizing cream or anti-itch
Mikala Monroe
cream, so hopefully she improves. My grandmother's appointment was after lunch, and Christine
treated her very well. She did a C&D on her arm and prescribed a chemo cream for her face and
the rest of the spots on her arm that she didn’t biopsy. There was a patient that came in for a skin
check, and when Christine was looking at his feet, he had spots between his toes that were green
and the skin was breaking down. Christine said it was just caused by moisture but I forgot to ask
Today I was with Christinem, I had plenty of time to work on my project and when I showed her,
she seemed to like it a lot. She was going to suggest for me to take some information off but after
she read all of my information, she said nevermind because she liked everything I put on there. A
drug rep brought us breakfast today from IHOP which was really good. Other than that we were
steady, but had some gaps where there were no patients. On Fridays I like to go through the
sample closet and empty boxes since it gives me a little break from school work and I get to take
some samples home to try so I like that too. We did have a patient in the afternoon who does not
have a history of cancer but she comes every 6 months for skin checks because her brother died
of melanoma in his brain. Its sad when that happens to people but it makes their family take it
more seriously and realize they need to start getting skin checks. We had another patient this
morning that came in to get checked because his sister was about 25 and she just got diagnosed
